INCREASING GRAFTS: CURRENT STRATEGIES
1. Adequate policies:- Health system & authorities
- In-hospital donation coordinator
2. Use of expanded criteria donors:
- Older age
- Donation after circulatory death
- Marked graft steatosis (30 – 60%)
- Donors with specific tumors (past or present)
- HBc Ab +ve donors (’latent infection’)
- HCV +ve donors (HCV-infected recipients)
3. Living donation

FARMACOS INMUNOSUPRESORES
Y FACTORES DE RIESGO CARDIOVASCULAR
Minimización de la inmunosupresión:
Modalidades:
- Reducción de ICN y sustitución con inmunosupresores menos tóxicos
- No administración / retirada rápida de corticoides
- Minimización global de la inmunosupresión
- Retirada total de inmunosupresión, tolerancia operacional
Tolerancia operacional espontánea:
- ≈ 20% pacientes seleccionados (5-7% del total de pacientes)
- Identificación de pacientes tolerantes no bien establecida
- Tardía en el post-trasplante (>5-6 años)
- Utilidad clínica real para minimizar toxicidad de inmunosupresores ?

TRASPLANTE HEPATICO
Inducción de tolerancia precoz en clínica humana:
1) Alo-trasplante médula ósea / precursores hematopoyéticos
simultáneo al trasplante de hígado: (a)
- Microquimerismo. Eficacia? Problemas: pre-condicionamiento, ‘GvsHD’
2) Anticuerpos anti-linfocitarios policlonales, ATG, alemtuzumab: (b)
- Linfodepleción inicial, reconstitución preferencial de T-regs. Eficacia no probada
3) Infusión células mesenquimales estromales: (c)
- Inmunomodulación. Tres RCT en marcha (sin noticias desde hace años)
4) Infusión de T-regs expandidas in vitro, donante-específicas: (d)
- Inmunosupresión standard inicial, weaning progresivo hasta mes 18
- Eficacia: 100% tolerancia en pacientes sin enfermedades autoinmunes
- Problema: complejidad tecnológica

TRASPLANTE HEPATICO: OTROS DESAFIOS
Candidatos a trasplante:
• Riesgo incrementado:
- Más viejos, más enfermos
- Expansión de criterios en HCC
• Indicaciones cambiantes:
- Erradicación de VHC
- Aumento de NASH
• Priorización en lista de espera:
- MELD vs. beneficio
- “Matching” donante-receptor
• Distribución de injertos
Respuesta inmune:
• Inmunosupresión personalizada:
- Según el riesgo de:
· Rechazo
· Efectos adversos
· Recidiva de la hepatopatía
- Biomarcadores / monitorización:
· Alorreactividad
· Grado de inmunosupresión
• Rechazo mediado por anticuerpos
Calidad de vida post-trasplante
