INTRODUCTION ET OBJET DU TEST
La gélose chromID™ MRSA est un milieu chromogène destiné au dépistage des souches S. aureus résistantes à la méticilline (SARM) chez les patients porteurs chroniques ou chez les patients à risque (1, 7). Ce milieu ne se substitue pas aux méthodes d’antibiogramme conventionnelles. Les SARM sont des bactéries multi-résistantes pouvant être responsables d'infections nosocomiales (3, 4, 9). La détection des porteurs de SARM est particulièrement importante pour la prévention et le suivi épidémiologique de ces infections. Dans ce contexte, l’utilisation de la gélose chromID™ MRSA contribue à la surveillance active des SARM.
PRINCIPE
La gélose chromID™ MRSA est constituée d’une base nutritive riche associant différentes peptones. Elle contient également un substrat chromogène d’ α-glucosidase et une association de plusieurs antibiotiques, dont la céfoxitine, permettant (2, 6): • la croissance des staphylocoques résistants à la
méticilline (SARM) y compris les souches hétéro-résistantes.
• la détection directe des souches SARM par la révélation de l’activité α-glucosidase (brevet déposé) : colonies vertes.
Le mélange sélectif permet d’inhiber la plupart des bactéries n’appartenant pas au genre Staphylococcus,ainsi que les levures.

personnes ayant des difficultés d’appréciation des couleurs.
• Les performances présentées ont été obtenues avec la méthodologie et la température d'incubation de 37°C indiquées dans cette notice. Toute déviation de méthodologie peut modifier les résultats.
• L'interprétation des résultats du test doit être faite en tenant compte des aspects macro et microscopiques et éventuellement des résultats d'autres tests.
CONDITIONS DE STOCKAGE
• Les boîtes se conservent entre 2°C et 8°C dans leur coffret jusqu’à la date de péremption.
• La durée de conservation des boîtes hors du coffret, en sachet cellophane, est de 2 semaines à 2-8°C à l’abri de la lumière.
ECHANTILLONS
Les prélèvements étudiés sont de différentes natures (nez, gorge, périnée…..) et sont réalisés à l'aide d'écouvillons. Une étude récente a montré que l'utilisation d'écouvillons en nylon floqué avec milieu de transport (Eswab) permet d'améliorer la détection des SARM sur la gélose chromID™ MRSA (voir paragraphe Performances, 4ème étude).
Pour les prélèvements de nez et de gorge uniquement, il est possible de rechercher la présence de SARM après une phase d’enrichissement de l’échantillon.
Il convient de respecter les bonnes pratiques de prélèvements et de transport, adaptées à chaque type de prélèvement.

MODE OPERATOIRE
A. Ensemencement direct
1. Laisser les boîtes revenir à température ambiante.
2. Ensemencer la gélose chromID™ MRSA directement à partir des échantillons.
3. Incuber à l’étuve, couvercle en bas, à 37°C en aérobiose. Les cultures sont examinées généralement après 18 - 24 heures d’incubation.
Il est possible de rendre un résultat positif dès 18 heures, toutefois en cas de résultat négatif (absence de croissance ou de coloration), il est nécessaire de poursuivre l’incubation jusqu’à 24 heures.
En cas de résultats négatifs à 24 heures, le milieu peut être incubé 24 heures supplémentaires afin d’augmenter la sensibilité de détection.
En cas d’utilisation d'un écouvillon en nylon floqué avec milieu de transport (Eswab), cette phase de réincubation n’est pas nécessaire.
B. Ensemencement après enrichissement1. Ensemencer l’échantillon dans le bouillon
d’enrichissement (bouillon cœur - cervelle ou Todd Hewitt + antibiotiques),
2. Incuber le bouillon à l’étuve à 37°C pendant 18-24h. 3. Laisser les boîtes revenir à température ambiante, 4. Ensemencer la gélose chromID™ MRSA à partir du
bouillon d’enrichissement, 5. Incuber à l’étuve à 37°C, couvercle en bas, en
aérobiose. Les cultures sont examinées après 18-24 heures d’incubation.
Les performances ont été obtenues en suivant la méthodologie et une température d'incubation de 37°C. Tout autre choix de température d'incubation est de la responsabilité de l'utilisateur pour vérifier que les performances sont maintenues.
Remarque : Les conditions de réalisation du test (passage par une phase d’enrichissement et durée d’incubation) doivent être adaptées à l’épidémiologie locale.
LECTURE ET INTERPRETATION
La présence d’au moins une colonie caractéristique verte identifiée donne un statut positif de l’échantillon à SARM. La coloration verte est plus intense en observant les colonies au dos de la boîte.
A. Ensemencement direct
• Après 18-24 heures d’incubation, pour les prélèvements nasaux, la coloration verte des colonies est spécifique des SARM. Pour les autres types de prélèvements, il est nécessaire d’identifier les colonies caractéristiques par des tests biochimiques ou immunologiques (S. aureus). Si l’identification S. aureus est confirmée, la résistance de la souche à la méticilline doit être vérifiée.
• Après 48 heures d’incubation et quel que soit le type de prélèvement, il est nécessaire d’identifier les colonies caractéristiques selon la même méthodologie.
B. Ensemencement après enrichissement
• Après 18-24 heures d’incubation, il est nécessaire d’identifier les colonies caractéristiques .

Les performances du milieu peuvent être testées vis-à-vis des souches suivantes: • Staphylococcus aureus ATCC® 43300. • Staphylococcus aureus ATCC® 29213.

Remarque :
Il est de la responsabilité de l'utilisateur de prendre en compte la nature de l'application et la législation locale en vigueur pour la mise en oeuvre du contrôle de qualité (fréquence, nombre de souches, température et durée d'incubation... ).
LIMITES DU TEST
• Certaines souches de S. aureus possédant le gène mec A mais ayant une CMI faible vis-à-vis de la céfoxitine (≤ 4 mg/l) sont susceptibles de ne pas se développer sur ce type de milieu.
• Certaines souches de S. aureus ne possédant pas le gène mec A peuvent donner des colonies caractéristiques sur ce type de milieu après 24 ou 48 heures d’incubation.
• Certains staphylocoques à coagulase négative peuvent apparaître vert-pâle. A 48 h d’incubation, ce résultat doit être considéré comme négatif.
• Certains germes autres que S. aureus donnent des colonies de couleur verte qui peuvent se différencier des SARM par un aspect phénotypique différent des colonies(Bacillus, bacilles Gram (-), entérocoques, souches de type BLSE).
• Dans le cas d’un antibiogramme réalisé à partir de colonies issues de la gélose chromID™ MRSA, les résultats obtenus pour les glycopeptides ne sont pas interprétables. Pour ces molécules, une dérive vers des résultats trop résistants est observée.
PERFORMANCES
Les performances de la gélose chromID™ MRSA ont été évaluées sur 4 sites et sur des prélèvements cliniques d’origine humaine dans le cadre du dépistage de portage des SARM.

La première évaluation (France) (8) a été réalisée sur 278 écouvillons de nez (dont 28 congelés et présumés positifs). chromID™ MRSA a été comparé à 2 autres milieux : • Un milieu de screening disponible sur le marché dont
les colonies caractéristiques doivent être confirmées par un test de la coagulase.
• Une gélose Columbia ANC + 5% de sang de mouton (CNA) associée à des tests de confirmation des colonies suspectes (coagulase + recherche du gène mecA par PCR).
Les géloses ont été ensemencées directement à partir des prélèvements. Les lectures ont été réalisées après 18-24 h et 48 h d’incubation à 33-37°C en aérobiose.
45 prélèvements (dont 23 congelés) ont été trouvés positifs par au moins un des 3 milieux.

La seconde évaluation (Belgique) (5) a été réalisée sur 491 prélèvements correspondant à des écouvillonnages de nez (363), de gorge (47) et de périnée (46) ou à d’autres types de prélèvements (35). Le milieu chromID™ MRSA a été comparé à une gélose Columbia + 5% de sang de mouton (COS) associée à des tests de confirmation (coagulase + recherche du gène mecA par PCR).Les prélèvements ont été ensemencés directement sur les géloses.Les lectures des géloses ont été réalisées après 18-24 h et 48 h d’incubation à 33-37°C en aérobiose.
55 prélèvements ont été trouvés positifs sur au moins un des 2 milieux quelle que soit la méthode .

La troisième évaluation (Belgique) a été réalisée sur 770 prélèvements correspondant à des écouvillonnages de nez (119), de gorge (355) ou à d’autres types de prélèvements (296).
Les prélèvements ont été ensemencés directement sur la gélose ou après une phase d’enrichissement en bouillon BHI et en bouillon Todd Hewitt + antibiotiques.
Les lectures des géloses ont été réalisées après 22-24 h et 48 h d’incubation à 33-37°C en aérobiose dans le cas de l’ensemencement direct, et après 18-24h dans le cas de l’enrichissement.

La 4ème évaluation (France) a été réalisée sur 167 prélèvements correspondants à des écouvillonnages de nez (144) ou de plaies (23).
Dans cette étude, l’écouvillon Eswab (embout en nylon floqué accompagné de son milieu de transport) a été comparé à un écouvillon sec utilisé en routine dans le laboratoire. Chaque écouvillon d’un même prélèvement a été ensemencé directement sur une gélose chromID MRSA.
Les lectures des géloses ont été réalisées après 18, 24 et 48 h d’incubation à 33-37°C en aérobiose.
59 prélèvements ont été trouvés positifs par au moins un des deux types d’écouvillons sur l’ensemble de l’étude.

ETUDE D’INTERFERENCE
Une étude interne a montré que l'utilisation de l'écouvillon sec en nylon floqué (Réf. 280101) est compatible avec la gélose chromID™ MRSA.
ELIMINATION DES DECHETS
Eliminer les réactifs utilisés et non utilisés ainsi que les matériels à usage unique contaminés en suivant les procédures relatives aux produits infectieux ou potentiellement infectieux. Il incombe à chaque laboratoire de gérer les déchets et les effluents qu'il produit selon leur nature et leur dangerosité, et d'en assurer (ou faire assurer) le traitement et l'élimination selon les réglementations applicables.

SUMMARY AND EXPLANATION
chromID™ MRSA agar is a chromogenic medium for the screening of methicillin-resistant S. aureus (MRSA) in chronic carriers or patients who are at risk for MRSA (1, 7). This medium does not replace conventional antimicrobial susceptibility tests. MRSA are multi-resistant bacteria which may cause nosocomial infections (3, 4, 9). The detection of MRSA carriers is particularly important for the epidemiological prevention and monitoring of these infections. In this context, the use of chromID™ MRSA agar contributes towards the active surveillance of MRSA.
PRINCIPLE
chromID™ MRSA agar consists of a rich nutrient base combining different peptones. It also contains a chromogenic substrate of α-glucosidase and a combination of several antibiotics including cefoxitin, which favour (2, 6): • the growth of methicillin-resistant staphylococci (MRSA)
including hetero-resistant strains. • the direct detection of MRSA strains by revealing
α-glucosidase activity (patent registered): green colonies.
The selective mixture inhibits most bacteria not belonging to the genus Staphylococcus, as well as yeasts.

knowledge of the origin and/or sanitary state of the animals does not totally guarantee the absence of transmissible pathogenic agents. It is therefore recommended that these products be treated as potentially infectious and handled observing the usual safety precautions (do not ingest or inhale).
• All specimens, microbial cultures and inoculated products should be considered infectious and handled appropriately. Aseptic technique and usual precautions for handling the bacterial group studied should be observed throughout this procedure. • Culture media should not be used as manufacturing material or components.
• Do not use reagents after the expiry date. • Do not use reagents if the packaging is damaged. • Do not use contaminated plates or plates that exude
moisture.• The use of this medium may be difficult for people who
have problems recognizing colors. • The performance data were obtained using the
procedure and the 37°C incubation temperature indicated in this package insert. Any change or modification in the procedure may affect the results.
• Interpretation of tests results should be made taking into consideration colonial and microscopic morphology and, if necessary, the results of any other tests performed.
STORAGE CONDITIONS
• Store the plates in their box at 2-8°C until the expiry date.
• If not in the box, plates can be stored in the cellophane sachet for 2 weeks at 2-8°C in the dark.
SPECIMENS
Different types of specimens may be used (nose, throat, perineum, etc.) and should be collected using swabs. A recent study has shown that the use of nylon flocked swabs and transport medium (Eswab) improves the detection of MRSA on chromID™ MRSA agar (see PERFORMANCE section, 4th evaluation).
For nose and throat specimens only, the presence of MRSA can be detected after enrichment of the sample.
Good laboratory practices for collection and transport should be respected and adapted to each type of specimen.

INSTRUCTIONS FOR USE
A. Direct inoculation
1. Allow the plates to come to room temperature.
2. Inoculate the specimens directly onto the chromID™ MRSA agar.
3. Incubate the plates inverted at 37°C in aerobic conditions. The cultures are generally examined after 18-24 hours of incubation. A positive result may be obtained after 18 hours of incubation, however if a negative result is obtained (no growth or coloration) incubation must be extended to 24 hours. If a negative result is obtained at 24 hours, the medium can be incubated for an additional 24 hours to increase the sensitivity of detection.
If a nylon flocked swab with transport medium (Eswab) is used, this additional incubation time is not necessary.
B. Inoculation after enrichment
1. Inoculate the sample in the enrichment broth (Brain-Heart Infusion Broth or Todd Hewitt Broth + Antibiotics).
2. Incubate the broth at 37°C for 18-24 hours.
3. Allow the plates to come to room temperature.
4. Inoculate the chromID™ MRSA agar using the enrichment broth.
5. Incubate the plates inverted at 37°C in aerobic conditions. The cultures are generally examined after 18-24 hours of incubation.
The performance data were obtained using the procedure and a 37°C incubation temperature. The user is responsible if any other incubation temperature is used, and should check that performance is maintained.
Note: Test conditions (use of an enrichment phase, incubation time) must be adapted to the local epidemiology.
READING AND INTERPRETATION
The presence of at least one typical green colony gives the sample a positive MRSA status. The green color is more vivid if the colonies are observed through the agar.
A. Direct inoculation
• After 18-24 hours of incubation, for nose specimens, a green color is characteristic of MRSA. For the other types of specimens the typical colonies must be identified using biochemical or immunological tests (S. aureus). If identification of S. aureus is confirmed, check the resistance of the strain to methicillin.
• After 48 hours of incubation and whatever the type of specimen, the typical colonies should be identified following the same procedure.
B. Inoculation after enrichment
• After 18-24 hours of incubation, identify the typical colonies.

LIMITATIONS OF THE METHOD
• Certain strains of S. aureus which have the mec A gene but a low MIC in relation to cefoxitin (≤ 4 mg/l) may not develop on this type of medium.
• Certain strains of S. aureus which do not have the mec A gene may develop typical colonies on this type of medium after 24 or 48 hours of incubation.
• Certain coagulase-negative staphylococci may develop a pale green color. After 48 hours of incubation, this result should be considered negative.
• Certain organisms other than S. aureus produce green colonies which have a different phenotypic appearance, enabling them to be differentiated from MRSA (Bacillus,Gram-negative bacilli, enterococci, ESBL strains).
• If a susceptibility test is performed using colonies from chromID™ MRSA agar, the results obtained for the glycopeptides will not be interpretable. A tendency towards too resistant results has been observed for these antibiotics.

PERFORMANCE
Performance of chromID™ MRSA agar was evaluated at 4 sites using human clinical specimens, within the context of MRSA carrier screening.
The first evaluation (France) (8) was performed using 278 nasal swabs (including 28 frozen and presumed positive specimens). chromID™ MRSA agar was compared to 2 other media: • A commercially available screening medium whose
typical colonies have to be confirmed by a coagulase test.
• A Columbia CNA agar + 5% sheep blood in combination with tests for confirmation of suspect colonies (coagulase + mecA gene detection by PCR).
The agar were inoculated directly with the specimens. Readings were performed after 18-24 h and 48 h of incubation at 33-37°C in aerobic conditions.
45 specimens (including 23 frozen ones) were found to be positive for at least one of the 3 media.

The second evaluation (Belgium) (5) was performed using 491 specimens corresponding to nasal (363), throat (47) and perineum (46) swabs or other types of specimens (35). chromID™ MRSA agar was compared to a Columbia agar + 5% sheep blood (COS) in combination with confirmation tests (coagulase + mecA gene detection by PCR).
The specimens were inoculated directly onto the agars.Reading of the agars was performed after 18-24 hours and 48 hours of incubation at 33-37°C in aerobic conditions.
Fifty-five specimens were found to be positive on at least one of the 2 media, whatever the method.

The fourth evaluation (France) was performed using 167 specimens corresponding to nasal (144) or wound (23) swabs.
In this study, the Eswab (flocked nylon tip and its transport medium) was compared to a dry swab used routinely in the laboratory. Each swab containing the same specimen was inoculated directly onto a chromID™ MRSA agar plate. Reading of the agars was performed after 18, 24 and 48 hours of incubation at 33-37°C in aerobic conditions.
59 specimens were found to be positive with at least one of the 2 types of swabs during the whole study.

EINFÜHRUNG UND PRODUKTERKLÄRUNG
chromID™ MRSA Agar ist ein chromogenes Medium zum Screening auf Methicillin-resistente S. aureus (MRSA) bei chronischen Trägern oder Risikopatienten (1, 7). Dieses Medium ist kein Ersatz für die klassische Empfindlichkeitsprüfung. MRSA sind multiresistente Bakterien, die nosokomiale Infektionen hervorrufen können (3, 4, 9). Der Nachweis von MRSA-Trägern ist insbesondere für die Prävention und die epidemiologische Verlaufskontrolle dieser Infektionen wichtig. In diesem Zusammenhang trägt der chromID™ MRSA Agar zur aktiven Überwachung von MRSA bei.
PRINZIP
Der chromID™ MRSA Agar besteht aus einer reichen Nährstoffgrundlage mit verschiedenen Peptonen. Er enthält außerdem ein chromogenes -Glucosidase Substrat und eine Antibiotika-Mischung, darunter Cefoxitin (2, 6). Diese ermöglichen:
das Wachstum Methicillin-resistenter Staphylokokken (MRSA) einschließlich der hetero-resistenten Stämme. den direkten Nachweis von MRSA durch den Nachweis der -Glucosidase (eingetragenes Patent): grüne Kolonien.
Die Selektivmischung hemmt die meisten nicht zur Gattung Staphylococcus gehörenden Bakterien und Hefen.

Dieser Kit enthält Bestandteile tierischen Ursprungs. Da durch die Kontrolle der Herkunft und/oder des Gesundheitszustandes der Tiere nicht völlig gewähr-leistet werden kann, dass diese Produkte keine übertragbaren pathogenen Agenzien enthalten, ist es empfehlenswert, diese als potenziell infektiös zu betrachten und unter Beachtung entsprechender Vorsichtsmaßnahmen zu behandeln (nicht einnehmen, nicht einatmen). Alle Proben, mikrobielle Kulturen und beimpfte Produkte müssen als potenziell infektiös betrachtet und unter Beachtung geeigneter Vorsichtsmaßnahmen sachgemäß behandelt werden. Während der gesamten Testdurch-führung müssen aseptische Arbeitsbedingungen und entsprechende Vorsichtsmaßnahmen für die zu untersuchende Keimgruppe eingehalten werden, siehe “CLSI M29-A, Protection of Laboratory Workers from Occupationally Acquired Infections; Approved Guideline– Current Revision“. Weitere diesbezügliche Informationen finden Sie in „Biosafety in Microbiological and Biomedical Laboratories, CDC/NIH – Latest edition" oder in den jeweils gültigen nationalen Richtlinien. Die Kulturmedien dürfen nicht als Materialien oder Bestandteile für die Herstellung verwendet werden. Die Platten nach Ablauf des Verfallsdatums nicht mehr verwenden. Platten mit beschädigter Verpackung nicht verwenden. Kontaminierte oder eingetrocknete Platten nicht verwenden. Der Gebrauch des Mediums ist für Personen mit Problemen bei der Farbwahrnehmung möglicherweise schwierig. Die angegebenen Leistungsdaten wurde gemäß den in der vorliegenden Packungsbeilage angegebenen Anweisungen und einer Inkubationstemperatur von 37°C ermittelt. Jede Abweichung von diesem Verfahren kann die Ergebnisse beeinflussen. Bei der Interpretation der Ergebnisse müssen die Kolonie- und mikroskopische Morphologie sowie gegebenenfalls die Ergebnisse anderer Tests berücksichtigt werden.
LAGERUNGSBEDINGUNGEN
Die Platten sind in ihrem Originalkarton bei 2-8°C bis zum Verfallsdatum haltbar. Außerhalb des Originalkartons beträgt die Haltbarkeit der Platten lichtgeschützt im Zellophanbeutel 2 Wochen bei 2-8°C.
PROBEN
Es können verschiedene Probenarten verwendet werden (Nase, Rachen, Perineum etc.). Die Proben sollten mit Abstrichtupfern abgenommen werden. In einer neueren Studie wurde gezeigt, dass der Gebrauch von Abstrichtupfern mit Nylon-Flockfaser und Transportmedium (eSwab) den Nachweis von MRSA auf chromIDTM MRSA Agar verbessert (siehe Abschnitt PERFORMANCE, 4. Evaluierung).
Nur für Nasen- und Rachenabstriche kann die Anwesenheit von MRSA nach einer Anreicherung der Probe nachgewiesen werden.
Bei der Gewinnung und dem Transport der Proben sollten die GLP-Richtlinien beachtet und auf das jeweilige Untersuchungsmaterial abgestimmt werden.

GEBRAUCH
A. Direkte Beimpfung
1. Bringen Sie die Platten auf Raumtemperatur.
2. Beimpfen Sie den chromID™ MRSA Agar direkt mit der Probe.
3. Inkubieren Sie im Brutschrank mit dem Deckel nach unten bei 37°C in aerober Atmosphäre. Die Kulturen werden im Allgemeinen nach 18-24 h Inkubation abgelesen.
Ein positives Ergebnis kann bereits nach 18 h erzielt werden. Bei einem negativen Ergebnis (kein Wachstum oder keine Färbung) muss jedoch die volle Inkubationszeit von 24 h eingehalten werden.
Bei einem negativen Ergebnis nach 24-stündiger Inkubation kann das Medium für weitere 24 h inkubiert werden, um die Sensitivität des Nachweises zu erhöhen. Bei Verwendung eines Abstrichtupfers mit Nylon-Flockfaserspitze + Transportmedium (eSwab) ist diese zusätzliche Inkubationszeit nicht erforderlich.
B. Beimpfung nach Anreicherung
1. Geben Sie die Probe in eine Anreicherungsbouillon (Hirn-Herz Bouillon oder Todd Hewitt + Antibiotika),
2. Inkubieren Sie die Bouillon im Brutschrank bei 37°C für 18-24 h.
3. Bringen Sie die Platten auf Raumtemperatur.
4. Beimpfen Sie den chromID™ MRSA Agar mit der Anreicherungsbouillon.
5. Inkubieren Sie die Platten mit dem Deckel nach unten bei 37°C in aerober Atmosphäre. Die Kulturen werden im Allgemeinen nach 18-24 h Inkubation abgelesen.
Die Leistungsdaten wurden gemäß der vorliegenden Gebrauchsanweisung und einer Inkubationstemperatur von 37°C ermittelt. Die Wahl einer anderen Inkubations-temperatur liegt in der Verantwortung des Anwenders, da sichergestellt werden muss, dass die Leistungsdaten dadurch nicht beeinträchtigt werden.
Anmerkung: Die Testbedingungen (Durchführung einer Anreicherung und Inkubationszeit) müssen an die regionale Epidemiologie angepasst werden.
ABLESUNG UND INTERPRETATION
Bei Anwesenheit von mindestens einer charakteristischen grünen Kolonie ist die Probe MRSA-positiv. Die grüne Färbung ist deutlicher erkennbar, wenn die Kolonien durch den Agar betrachtet werden (Platte umdrehen).
A. Direkte Beimpfung
Bei Nasenabstrichen ist nach 18-24 h Inkubation die Grünfärbung mindestens einer Kolonie charakteristisch für MRSA. Bei anderen Probenarten müssen die charakteristischen Kolonien zuerst mit biochemischen oder immunologischen Tests (S. aureus) bestätigt werden. Ist der Stamm S. aureus bestätigt, muss die Methicillin-Resistenz geprüft werden.
Nach 48 h Inkubation müssen die charakteristischen Kolonien unabhängig von der Probenart mit demselben Verfahren identifiziert werden.
B. Beimpfung nach Anreicherung
Nach 18-24 h Inkubation werden die charakteristischen Kolonien identifziert.

Anmerkung:
Es liegt in der Verantwortung des Anwenders, den Verwendungszweck des Mediums und die jeweils gültigen Bestimmungen bei der Durchführung der Qualitäts-kontrolle zu berücksichtigen (Frequenz, Anzahl der Stämme, Inkubationstemperatur, Inkubationszeit etc).
LIMITIERUNGEN
Einige S. aureus Stämme, die das mecA Gen besitzen, aber eine geringe MHK in Bezug auf Cefoxitin ( 4 mg/l) haben, wachsen möglicherweise nicht auf diesem Agartyp.
Einige S. aureus Stämme ohne mecA Gen können auf diesem Agartyp nach 24 oder 48 h Inkubation charakteristische Kolonien bilden.
Einige Koagulase-negative Staphylokokken können eine hellgrüne Färbung entwickeln. Nach 48-stündiger Inkubation sollte dieses Ergebnis als negativ bewertet werden.
Einige nicht S. aureus Stämme bilden grüne Kolonien, die jedoch aufgrund ihres anderen phänotypischen Aussehens von MRSA unterschieden werden können (Bacillus, gramnegative Stäbchen, Enterokokken, ESBL Stämme).
Bei der Erstellung eines Antibiogramms mit Kolonien von chromID™ MRSA Agar, kann das Ergebnis für die Glycopeptide nicht verwendet werden. Bei diesen Antibiotika wurde tendenziell eine zu hohe Resistenzrate auf chromID™ MRSA Agar beobachtet.
PERFORMANCE
Die Leistungsdaten des chromID™ MRSA Agars wurden in 4 Laboratorien mit klinischen Proben humanen Ursprungs im Rahmen eines Screenings auf MRSA-Träger ermittelt.

Für die erste Evaluierung (Frankreich) (8) wurden 278 Nasenabstriche (davon 28 eingefrorene, voraussichtlich positive Proben) getestet. chromID™ MRSA Agar wurde mit 2 anderen Medien verglichen:
Einem kommerziellen Screeningagar, dessen charak-teristische Kolonien mit dem Koagulase-Nachweis bestätigt werden müssen. Einem Columbia CNA Agar + 5% Schafblut in Verbindung mit Bestätigungstests für verdächtige Kolonien (Koagulase + Nachweis des mecA Gens durch PCR).
Die Medien wurden direkt mit den Proben beimpft. Die Ablesung erfolgte nach 18-24 h und 48 h Inkubation bei 33-37°C unter aeroben Bedingungen.
45 Abstriche (davon 23 eingefrorene) ergaben mit mindestens einem der drei Medien ein positives Ergebnis.

Die 4. Evaluierung (Frankreich) wurde mit 167 Proben (144 Nasen- und 23 Wundabstriche) durchgeführt.
In dieser Studie wurde der eSwab (Abstrichtupfer mit Nylon-Flockfaserspitze und Transportmedium) mit einem trockenen Tupfer, wie er üblicherweise im Labor verwendet wird, verglichen. Die jeweils für dieselbe Probe verwendeten Abstrich-tupfer, wurden direkt auf einen chromID™ MRSA Agar überimpft.
Die Ablesung der Platten erfolgte nach 18, 24 und 48 h Inkubation bei 33-37°C unter aeroben Bedingungen.
59 Proben ergaben über die gesamte Studie mit mindestens einem der zwei Abstrichtupfertypen ein positives Ergebnis.

INTRODUCCION Y OBJETIVO DE LA PRUEBA
El medio chromID™ MRSA es un medio cromogénico destinado al cribado de cepas S. aureus resistentes a meticilina (SARM) en pacientes portadores crónicos o en pacientes con riesgo (1, 7). Este medio no sustituye a los métodos convencionales de antibiograma. Los SARM son bacterias multi-resistentes que pueden ser responsables de infecciones nosocomiales (3, 4, 9). La detección de los portadores de SARM es particularmente importante para la prevención y el seguimiento epidemiológico de estas infecciones. En este contexto, la utilización del medio chromID™ MRSA contribuye a la vigilancia activa de los SARM.
PRINCIPIO
El medio chromID™ MRSA está constituido por una base nutritiva rica asociada a diferentes peptonas. Contiene así mismo un sustrato cromógeno de -glucosidasa y una combinación de varios antibióticos que incluyen (cefoxitina) que permite (2, 6):
el crecimiento de estafilococos resistentes a la meticilina (SARM) incluidas las cepas heteroresistentes. la detección directa de las cepas SARM por el revelado de la actividad -glucosidasa (patente registrada) : colonias verdes.
La mezcla selectiva permite inhibir el crecimiento de la mayoría de las bacterias no pertenecientes al género Staphylococcus, así como el de levaduras.

PRECAUCIONES DE UTILIZACIÓN
Para diagnóstico in vitro únicamente. Para uso profesional únicamente.
Este equipo contiene componentes de origen animal. El certificado del origen y/o del estado sanitario de los animales no pueden aportar la garantía absoluta que estos productos no contienen ningún agente patógeno transmisible, se recomienda manipularlos con las precauciones de uso relacionadas con los productos potencialmente infecciosos (no ingerir, no inhalar). Las muestras, cultivos bacterianos y productos inoculados deben considerarse potencialmente infecciosos y deben manipularse de forma adecuada. Las técnicas asépticas y las precauciones habituales de manipulación para el grupo bacteriano estudiado deben respetarse a lo largo de toda la manipulación; consultar en “CLSI M29-A, Protection of Laboratory Workers from Occupationally Acquired Infections;Approved Guideline – Revisión en vigor". Los medios de cultivo no deben utilizarse como material o componente de fabricación. No utilizar los reactivos después de la fecha de caducidad. No utilizar los reactivos cuyo embalaje esté deteriorado. No utilizar placas contaminadas o con líquido de exudación. La utilización del medio puede ser delicada para las personas con dificultades en la apreciación de los colores . La interpretación de los resultados de la prueba debe hacerse teniendo en cuenta los aspectos macro y microscópicos, y eventualmente los resultados de otras pruebas. Las prestaciones presentadas han sido obtenidas con la metodología y la temperatura de 37ºC indicada en esta ficha técnica. Toda desviación en la misma puede modificar los resultados.
CONDICIONES DE ALMACENAJE
Las placas se conservan entre 2°C y 8°C en su envase hasta la fecha de caducidad.La duración de la conservación de las placas fuera de su envase, en bolsa de celofán, es de 2 semanas a 2-8°C protegidas de la luz.
MUESTRAS
Las muestras pueden ser de diferente origen ( : nariz, garganta, perineo, etc …)..y se realizan con la ayuda de escobillón. Un estudio reciente muestra que la utilización de escobillones de nylon con medio de transporte(Eswab) permite mejorar la detección de los SARM en agar chromID™ MRSA (ver párrafo Prestaciones , 4º estudio).
Para muestras de nariz y garganta sólo, la presencia de MRSA se puede detectar tras enriquecimiento de la muestra.
Conviene respetar las buenas prácticas en términos de recogida de muestras y transporte, adaptadas a cada tipo de muestra.

MODO DE EMPLEO
A. Inoculación directa
1. Dejar que las placas alcancen la temperatura ambiente.
2. Inocular las muestras directamente en el medio . chromID™ MRSA.
3. Incubar en estufa, tapa hacia abajo, a 37°C en aerobiosis. Los cultivos se examinan generalmente después de 18 a 24 horas de incubación.
Se puede obtener un resultado positivo tras 18 horas de incubación, en caso de resultados negativos (ausencia de crecimiento o de color), la incubación se puede extender a 24 horas.
Si se obtiene un resultado negativo a las 24 horas, se puede incubar el medio otras 24 horas adicionales para incrementar la sensibilidad de la detección.
En caso de utilización de un esobillón de nylon con medio de trasnporte (Eswab),esta fase de reincubación no es necesaria
B. Incubación tras enriquecimiento
1. Inocule la muestra en el caldo de enriquecimiento (caldo corazón-cerebro o caldo Todd Hewitt + antibióticos).
2. Incube el caldo a 37°C durante 18-24 horas. 3. Deje que las placas alcancen la temperatura
ambiente. 4. Inocule el medio chromID™ MRSA usando el caldo
de enriquecimiento. 5. Incubar en estufa, tapa hacia abajo, a 37°C en
aerobiosis. Los cultivos se examinan generalmente después de 18 a 24 horas de incubación.
Los datos de prestaciones se obtuvieron usando el procedimiento a 37ºC de temperatura de incubación. El usuario es responsable si usa otra temperatura de incubación y debe verificar que las prestaciones se mantienen..
Advertencia : Las condiciones de realización de la prueba (paso por una fase de enriquecimiento y duración de la incubación) deben estar adaptadas a la epidemiología local.
LECTURA E INTERPRETACION
La presencia de al menos una colonia verde típica corresponde a un resultado positivo : presencia de MRSA. El color verde es más intenso si las colonias se observa a través del agar.
A. Inoculación directaTras 18-24 horas de incubación, para muestras de nariz, un color verde es característico de MRSA. Para los otros tipos de muestras las colonias se pueden identificar usando pruebas bioquímicas o inmunológicas.(S. aureus). Si se confirma la identificación de S. aureus verifique la resistencia a meticilina de la cepa.
Tras 48 horas de incubación y cualquiera que sea el tipo de muestra las colonias se deben identificar por el mismo procedimiento.
B. Inoculación tras enriquecimiento
Tras 18-24 horas de incubación identifique las colonias

Advertencia :
Es responsabilidad del usuario tener en cuenta la naturaleza de la aplicacion y la normativa local en vigor para poner en marcha el control de calidad (frecuencia, número de cepas, temperatura y duración de la incubación... ).
LIMITES DE LA PRUEBA
Algunas cepas de S. aureus que poseen el gen mec A pero que presentan una CMI baja frente a la cefoxitina ( 4 mg/l) son susceptibles de no desarrollarse en este tipo de medio . Algunas cepas de S. aureus carentes del gen mec Apueden dar colonias características sobre este tipo de medio después de 24 o 48 horas de incubación.Algunos estafilococos coagulasa negativo pueden presentar un color verde pálido. A 48 h de incubación, este resultado debe considerarse como negativo. Algunos gérmenes distintos a S. aureus dan colonias verdes que pueden diferenciarse de los MRSA por su aspecto fenotípico diferente(Bacillus,bacilos Gram (-),enterococos, cepas de tipo BLSE).

En el caso de un antibiograma realizado a partir de colonias obtenidas en el medio chromID™ MRSA , los resultados obtenidos para los glicopéptidos no son interpretables. Para estas moléculas, se observa una deriva hacia los resultados resistentes.
PRESTACIONES TECNICAS
Las prestaciones han sido evaluadas en 4 centros , en muestras clínicas de origen humano dentro del marco de un cribado de portadores SARM .
La primera evaluación (Francia) (8) ha sido realizada con 278 exudados de nariz (de ellos 28 congelados y presumiblemente positivos). El medio chromID™ MRSA bioMérieux ha sido comparado con otros 2 medios :
Un medio de cribado disponible en el mercado cuyas colonias características deben ser confirmadas por una prueba de coagulasa. Un medio Columbia ANC + 5% de sangre de cordero (CNA) asociado a pruebas de confirmación de las colonias sospechosas (coagulasa + detección del gen mecA por PCR).
Los medios fueron inoculados directamente a partir de las muestras. Las lecturas fueron realizadas después de 18-24 h y 48 h de incubación a 33-37°C en aerobiosis.
45 muestras (de ellas 23 congeladas) fueron positivas al menos por uno de los 3 medios.

La segunda evaluación (Bélgica) (5) fue realizada con 491 muestras correspondientes a exudados de nariz (363), faringe (47) y de periné (46) u otros tipos de muestras (35). El medio chromID™ MRSA bioMérieux fue comparado con un medio Columbia + 5% de sangre de cordero (COS) asociado a pruebas de confirmación (coagulasa + detección del gen mecA por PCR).
Las muestras fueron inoculadas en los medios directamente y después de enriquecimiento en caldo BHI + 7,5% NaCl a 33-37°C en aerobiosis. Las lecturas de los medios fue realizada después de 18-24 h (D1) y 48 h (D2) de incubación a 33-37°C en aerobiosis.
Fueron positivas 55 muestras en al menos uno de los 2 medios 2 independientemente del método usado.

La tercera evaluación (Bélgica) se realizó usando 770 muestras de las cuales eran nasales (119) y torunda de garganta (355) u otros tipos de muestras (296).
Las muestras fueron directamente inoculadas en el medio o tras enriquecimiento en caldo de cerebro-corazón o en caldo Hewitt Broth + antibióticos
Se leyó el medio tras 22-24 horas y 48 horas de incubación a 33-37°C en condiciones aeróbicas en el caso de inoculación directa y tras 18-24 horas en caso de enriquecimiento.

La 4ªevaluación (France) ha sido realizada sobre 167 muestras correspondientes a escobillones de nariz (144) o de herida (23). En este estudio, el escobillón Eswab (recubierto de nylon con medio de transporte) ha sido comparado con el utilizado en rutina en el laboratorio. Cada escobillón de una misma muestra ha sido sembrado directamente en agar chromID MRSA.
Las lecturas de las placas han sido realizadas a 18, 24 y 48 h de inubación a 33-37°C en aerobiosis. 59 muestras han sido positivas por al menos uno de los dos tipos de escobillón sobre el conjunto del estudio.

La utilización de Eswab permite aumentar de manera significativa la detección SARM en 18 y 24 horas sobre agar chromID™ MRSA. Una reincubación y lectura a 48 horas es inútil .
ESTUDIO DE INTERFERENCIA
Un estudio interno ha mostrado que la utilización del escobillón seco de nylon (Réf. 280101) es compatible con el agar chromID™ MRSA.
ELIMINACIÓN DE RESIDUOS
Eliminar los reactivos utilizados y no utilizados, así como los materiales de un solo uso contaminados siguiendo los procedimientos relacionados con los productos infecciosos o potencialmente infecciosos. Es responsabilidad de cada laboratorio gestionar los residuos y efluentes que produzca según su naturaleza y su peligrosidad, y garantizar ( o hacer garantizar) el tratamiento y la eliminación según las normativas aplicables

INTRODUZIONE E OBIETTIVO DEL TEST
L’agar chromID™ MRSA è un terreno cromogeno destinato allo screening dei ceppi di S. aureus resistenti alla meticillina (MRSA) nei pazienti portatori cronici o nei pazienti a rischio (1, 7). Questo terreno non si sostituisce ai metodi d’antibiogramma convenzionali. Gli MRSA sono batteri multi-resistenti che possono causare infezioni nosocomiali (3, 4, 9). La ricerca dei portatori di MRSA è particolarmente importante per la prevenzione ed il monitoraggio epidemiologico di queste infezioni. In questo contesto, l’utilizzazione dell’agar chromID™ MRSA contribuisce alla sorveglianza attiva delle MRSA.
PRINCIPIO
L’agar chromID™ MRSA è costituito da una ricca base nutritiva che associa differenti peptoni. Contiene inoltre un substrato cromogeno, l’ -glucosidasi ed una associazione di più antibiotici, tra cui la cefoxitina, che permettono (2, 6):
la crescita degli Staphylococcus aureus meticillino-resistenti (MRSA), compresi i ceppi etero-resistenti. la ricerca diretta dei ceppi MRSA tramite la rilevazione dell’attività -glucosidasica (brevetto depositato): colonie verdi.
La miscela selettiva permette di inibire la maggior parte dei batteri che non appartengono al genere Staphylococcus e dei lieviti.

Poiché i controlli sull’origine e/o sullo stato sanitario degli animali non possono garantire in maniera assoluta che questi prodotti non contengano nessun agente patogeno trasmissibile, si raccomanda di manipolarli con le precauzioni d’uso relative ai prodotti potenzialmente infettivi (non ingerire, non inalare). I prelievi, le colture batteriche ed i prodotti seminati devono essere considerati come potenzialmente infettivi e devono essere manipolati in maniera appropriata. Non utilizzare i reattivi con l’imballaggio deteriorato. Non utilizzare piastre contaminate o trasudanti umidità. L‘utilizzazione del terreno potrebbe essere difficoltosa per persone con problemi nella percezione dei colori. Le performance riportate di seguito sono state ottenute con il metodo e la temperatura di incubazione di 37°C indicati in questa scheda tecnica. Qualsiasi deviazione dal procedimento indicato può alterare i risultati. L’interpretazione dei risultati del test deve essere fatta tenendo conto del contesto clinico, dell’origine del prelievo, degli aspetti macro e microscopici ed, eventualmente, dei risultati di altri esami.
CONDIZIONI DI CONSERVAZIONE
Le piastre si conservano a 2-8°C, nella loro confezione, fino alla data di scadenza.Fuori della confezione le piastre si conservano per 2 settimane a 2-8°C nel loro sacchetto di cellophane, al riparo dalla luce.
CAMPIONI
Si possono utilizzare vari tipi di prelievo (naso, gola, perineo.…) eseguiti tramite tamponi. Uno studio recente ha mostrato che l’impiego di tamponi di nylon fioccato con terreno di trasporto (Eswab) permette di migliorare la ricerca degli MRSA sull’agar chromID™ MRSA(vedere paragrafo Performance, 4a valutazione.
Unicamente per i prelievi di naso e gola, è possibile ricercare la presenza di MRSA dopo una fase di arricchimento dei campioni.
Per il prelievo ed il trasporto si devono rispettare le norme di buona pratica di laboratorio adattate ad ogni tipo di campione.

PROCEDIMENTO
A. Semina diretta
1. Riportare le piastre a temperatura ambiente.
2. Inoculare il campione direttamente sull’agar chromID™ MRSA.
3. Incubare immediatamente la piastra in termostato a 37°C, in aerobiosi, con il coperchio rivolto verso il basso. Le colture vengono normalmente esaminate dopo 18-24 ore di incubazione.
E’ possibile ottenere un risultato positivo dopo 18 ore, tuttavia, in caso di risultato negativo (assenza di crescita o di colorazione), è necessario proseguire l’incubazione fino a 24 ore.
In caso di risultati negativi dopo 24 ore, il terreno può essere incubato per altre 24 ore per aumentare la sensibilità della rilevazione.
In caso di utilizzazione di un tampone di nylon fioccato con terreno di trasporto (Eswab), questa fase di re-incubazione non è necessaria.
4. Il terreno può essere inoculato anche partendo da un brodo di arricchimento.
B. Semina dopo arricchimento
1. Seminare il campione nel brodo di arricchimento (brodo cuore - cervello o Todd Hewitt + antibiotici),
2. Incubare il brodo in termostato a 37°C per 18-24 ore. 3. Riportare le piastre a temperatura ambiente, 4. Seminare l’agar chromID™ MRSA partendo dal brodo
di arricchimento, 5. Incubare in termostato a 37°C, con il coperchio in
basso, in aerobiosi. Le colture sono esaminate dopo 18-24 ore di incubazione.
Le performance sono state ottenute seguendo il procedimento indicato e una temperatura di incubazione di 37°C. Se viene scelta una diversa temperatura di incubazione, è responsabilità dell’utilizzatore verificare che le performance siano mantenute.
Nota : Le condizioni con cui viene realizzato il test (esecuzione di un arricchimento, durata dell’incubazione) devono essere adattate all’epidemiologia locale.
LETTURA E INTERPRETAZIONE
La presenza di almeno una colonia caratteristica verde attribuisce al campione uno MRSA status positivo. La colorazione verde è più intensa se le colonie vengono osservate dal retro della piastra.
A. Semina diretta
Per i prelievi nasali, la colorazione verde delle colonie dopo 18-24ore di incubazione è specifica degli SARM. Per gli altri tipi di prelievo, è necessario identificare le colonie caratteristiche con test biochimici o immunologici (S. aureus). Se l’identificazione S. aureus è confermata, deve essere verificata la resistenza del ceppo alla meticillina.
Dopo 48 ore di incubazione, per qualunque tipo di prelievo, è necessario identificare le colonie caratteristiche seguendo lo stesso procedimento.
B. Semina dopo arricchimento
Dopo 18-24 ore di incubazione, è necessario identificare le colonie caratteristiche.

E’ responsabilità dell’utilizzatore tenere conto della natura dell’applicazione ed assicurarsi che il controllo di qualità corrisponda a quanto previsto dalla legislazione locale vigente (frequenza, numero di ceppi, temperatura d’incubazione...).
LIMITI DEL METODO
Alcuni ceppi di S. aureus che possiedono il gene mec Ama che hanno una MIC bassa nei confronti della cefoxitina ( 4 mg/l) possono non svilupparsi su questo tipo di terreno. Alcuni ceppi di S. aureus che non possiedono il gene mec A possono formare colonie caratteristiche su questo tipo di terreno dopo 24 o 48 ore di incubazione. Alcuni stafilococchi coagulasi negativi possono apparire verde pallido. Dopo 48 ore di incubazione questo risultato deve essere considerato negativo. Alcuni germi diversi dallo S. aureus danno colonie di colore verde che possono differenziarsi dagli MRSA per un aspetto fenotipico delle colonie differente (Bacillus,bacilli Gram (-), enterococchi, ceppi di tipo BLSE). Nel caso di un antibiogramma eseguito con colonie cresciute sull’agar chromID™ MRSA, i risultati ottenuti per i glicopeptidi non sono interpretabili. Per queste molecole si osserva una deriva verso risultati troppo resistenti.
PERFORMANCE
Le performance dell’agar chromID™ MRSA sono state valutate in 4 Centri, su prelievi clinici di origine umana, nel quadro dello screening dei portatori di MRSA.
La prima valutazione (Francia) (8) è stata realizzata su 278 campioni nasali (di cui 28 congelati e presunti positivi). L’agar chromID™ MRSA bioMérieux è stato comparato ad altri 2 terreni :
Un terreno di screening disponibile sul mercato, le cui colonie caratteristiche devono essere confermate con un test della coagulasi. Un agar Columbia CNA + 5% di sangue di montone associato a test di conferma delle colonie sospette (coagulasi + ricerca del gene mecA con la PCR).

Gli agar sono stati seminati direttamente con i campioni. Le letture sono state eseguite dopo 18-24 ore (G1) e dopo 48 ore (G2) di incubazione a 33-37°C in aerobiosi.
45 prelievi (tra cui 23 congelati) sono risultati positivi con almeno uno dei 3 terreni.

La seconda valutazione (Belgio) (5) è stata condotta su 491 prelievi costituiti da tamponi nasali (363), faringei (47) e perineali (46) o da altri tipi di prelievo (35). Il terreno chromID™ MRSA è stato comparato ad un agar Columbia + 5% di sangue di montone (COS) associato a test di conferma (coagulasi + ricerca del gene mecA con la PCR).
I prelievi sono stati seminati direttamente sugli agar.Le letture degli agar sono state eseguite dopo 18-24 ore e dopo 48 ore di incubazione a 33-37°C in aerobiosi.
55 prelievi sono risultati positivi su almeno uno dei 2 terreni con qualunque metodo.

La terza valutazione (Belgio) è stata eseguita su 770 campioni di tamponi nasali (119), faringei (355) o di altri tipi di prelievo (296).
I prelievi sono stati seminati sull’agar direttamente o dopo una fase di arricchimento in brodo BHI ed in brodo Todd Hewitt + antibiotici.
Le letture degli agar sono state eseguite dopo 22-24 ore e 48 ore di incubazione a 33-37°C in aerobiosi nel caso della semina diretta e dopo 18-24 ore nel caso dell’arricchimento.
