Capillary Beds of the Nephron
Every nephron has two capillary beds
o Glomerulus
o Peritubular capillaries
Blood pressure in the glomerulus is high because:
o Arterioles are high-resistance vessels
o Afferent arterioles have larger diameters than efferent arterioles
Fluids and solutes are forced out of the blood throughout the entire
length of the glomerulus
Capillary Beds
Peritubular beds are low-pressure, porous capillaries adapted for
absorption that:
o Arise from efferent arterioles
o Cling to adjacent renal tubules
o Empty into the renal venous system
Vasa recta – long, straight efferent arterioles of juxamedullary
nephrons

Regulation of Glomerular
Filtration
Renal autoregulation
o Myogenic – responds to changes in pressure in the renal blood
vessels
o Tubuloglomerular feedback – senses changes in the
juxtaglomerular apparatus
Sympathetic Nervous System
o When the SNS is at rest: Renal blood vessels are maximally
dilated, Autoregulation mechanisms prevail
o Under stress: Norepinephrine is released by the SNS, Epinephrine
is released by the adrenal medulla, Afferent arterioles constrict
and filtration is inhibited

o Decreased GFR
• prerenal failure: dehydration
• renal failure
Clinical Significance
Decreased urea (BUN)
o Low protein diet, malnutrition
o Newborn
o Severe hepatic insufficiency
o Severe vomiting or diarrhea
o Overhydration
o Pregnancy
o Syndrome of inappropriate anti-diuretic secretion (SIADH)
Disease Correlations
Azotemia: elevated conc. of urea in blood
Very high plasma urea concentration accompanied by renal failure is
called uremia, or the uremic syndrome
Causes of urea plasma elevations are:
o Prerenal
o Renal
o Postrenal
Pre-Renal Azotemia
Reduced renal blood flow -> Less blood is delivered to the kidney ->
less urea filtered
Anything that produces a decrease in functional blood volume include:
o Congestive heart failure,
o shock,
o hemorrhage,
o dehydration
High protein diet or increased catabolism (Fever, major illness, stress)
Renal Azotemia
Decreased renal function causes increased blood urea due to poor
excretion
o Acute & Chronic renal failure
o Glomerular nephritis
o Tubular necrosis
o other intrinsic renal disease
Post-Renal Azotemia
Obstruction of urine flow
o Renal calculi
o Tumors of bladder or prostate
o Severe infections
BUN values can be converted to
urea conc.
Atomic mass of nitrogen = 14 g/mol; Mw of urea = 60.1 g/mol
Urea contains 2 N atoms per molecule (28)
Urea N is 46.6% by weight of urea (28
Therefore
o 100 mg/L of BUN divided by 0.466 = 214.6 mg/L of urea
= 3.6 mmol/L of urea (214.6/60.1)
or
o convert urea to BUN : urea (mg/dl) x (28/60) = BUN (mg/dl)
o convert BUN to urea : BUN (mg/dl) x (60/28) = urea (mg/dl)

Why is creatinine clearance most often used to monitor GFR?
o Creatinine freely filtered by glomerulus
o Creatinine not ‘rehandled’ by tubules
o Creatinine is an endogenous substance
o Amount of creatinine produced per day is constant
o Amount of creatinine produced is proportional to muscle mass

Glomerular disease
o Immunologic mechanism
ม hematuria, azotemia, oligouria (<400 ml/day), edema และ
hypertension

Glomerular disease
Acute glomerulonephritis : acute injury to glomerulus basement
membrane
o Hematuria and anemia
o Proteinuria (<3 g/day), Decreased GFR
o Elevated BUN and Creatinine, Na+ and water retention
Chronic glomerulonephritis
o Lengthy glomerular inflammation -> glomerular scarring
o minor decreases in renal function occur at first and only slight
proteinuria and hematuria
o Gradual development of uremia

Nephrotic syndrome
ผดปกตของ glomerular basement membrane
Massive Proteinuria (>3.5 g/day) and hypoalbuminemia, Edema
Hyperlipidemia and lipiduria (secondary to lipoprotein alteration)

Renal failure
Acute renal failure: sudden onset
o Oliguria, anuria, hyponatremia, azotemia
o แบงสาเหตเปน
• Prerenal azotemia : BUN สง, creatinine ปกต, BUN/creatinine
ratio: Increased (20:1-30:1)
• Renal azotemia : BUN และ creatinine สง, BUN/creatinine
ratio: Normal (10:1-20:1)
• Postrenal azotemia: BUN และ creatinine สง, BUN/creatinine
ratio: Increased (>20:1)

Renal failure
Chronic renal failure: gradual onset
Chronic renal failure

CKD
The stage of CKD
ระยะ GFR
(ml/min/1.73m2) ค าอธบาย

Tubular disease
Fanconi syndrome
Acute tubular necrosis (ATN)
o อาการ: renal failure ม azotemia, hyperacidemia, oligouria

Diabetic patient with renal
failure
Diabetic nephropathy -> End-stage renal disease (ESRD)
Nephropathy occurs in 40 - 50% of IDDM patients
Primarily glomerular lesions

Renal calculi
bladder
