I- INTRODUCTION

The Operations Manual of Rosales Water District (RWD) contains the general information about the agency, its underlying function, mandates, operating procedures and organization.

The Manual aims to provide knowledge and understanding about the district’s responsibilities and structures, water distribution system and the operational controls to its readers especially the district’s employees and concessionaires.

Rosales WD Operations Manual is divided into several parts as follows:

General Information – It contains the company profile of the district such as it’s history, mission, vision, service pledge, strategic objectives, mandates and functions in its areas of operation.

Organization and Responsibilities – This indicates the organizational structure using data as of December 2015, duties and responsibilities of every division in the district.

Operational Control and Supervision – This part describes the powers of authority, supervisory and operational controls of the district

Operating Procedures – This describes the step-by-step procedures and work instructions in narrative or with use of activity flow charts including the flow of forms.

II- DEFINITION OF TERMS AND ACRONYMS

Category of LWD – The categorization is a two-stage process. The initial stage is categorization based on the Number of Active Service Connections. The second stage of categorization considers the following factors: Gross Revenues, Total Assets, Net Income before Interest and Depreciation and Staff Productivity Index, These factors will determine the Point-Rating Category Points. The resulting Point-Rating Category is compared with the Service Connection Categroy; whichever is LOWER is the FINAL Category of the LWD.

COA - Commission on Audit, responsible in the audit of district’s transaction CSC- Civil Service Commission, central personnel agency of the government DBM- Department of Budget and Management, promote the sound, efficient

and effective management and utilization of government resources ERC- Energy Regulatory Commission; the agency where the district received

its COC for the generator sets

LWUA - Local Water Utilities Administration with a specialized lending function mandated by law to promote and oversee the development of water supply systems in provinces, cities and municipalities outside of Metropolitan Manila.

CCC - Conditional Certificate of Conformance issued by LWUA

NWRB - National Water Resources Board, the Policy-Making Body for the Philippine Water Sector

III- GENERAL INFORMATION

Profile

The original water supply system of Rosales was constructed in the early 1920’s by the now

defunct National Waterworks and Sewerage Authority. On August 28, 1986 as contained in the local

Sangguniang Bayan Resolution No. 71 s. 1986, under the administration of the then Officer-In-Charge

Hon. Mayor Alfonso C. Bince, the Rosales Water District (RWD) was officially formed and organized,

Sangguniang Bayan Resolution No. 61 s. 86 authorized Hon. Mayor Alfonso C. Bince for the transfer of

Rosales Water Works System to Rosales Water District. Eventually, Local Water Utilities Administration

(LWUA) issued a CCC No. 270 on December 4, 1986. Rosales Water District is a government-owned and

controlled corporation as resolved by the Supreme Court November 12, 1992 , governed under

Presidential Decree No. 198 (as amended by Presidential Decree Nos. 768 and 1479, R.A. 9286. It is an

autonomous agency free from political influence and independent of any LGU and which entity takes

charge and operates the local water utility on a self supporting and businesslike manner. The Rosales

Water District was initially categorized by LWUA as “Small” then “Medium” and “Big” under the

Salvador M. Enriquez (SME) Manual on Categorization/ReCategorization of LWDs Model Organizational

Structure/Staffing Pattern, Compensation Policy and Rules for Implementation thereon. Effective

March, 2012 the LWUA categorized the RWD as “C” under the Revised Local Water District Manual on

Categorization, Re-Categorization and Other Related Matters (LWD-MaCRO) of the DBM. The staffing

pattern for Category “C” was approved by DBM dated April 30, 2015 and implemented by the district on

June 2015.

On August 1997, LWUA took over the policy-making and management of the district. which

ended on December, 2000. The General Manager at the time of take over was Mr. Desiderio L. Elio, Jr.

The members of the BOD were Mr. Antonio Rosete, Mr. Aquilino Santiago, Mr. Fidel Adriano, Mr.

Ceferino Garcia and Mrs. Felicisima Cargue. Behind the dramatic growth of the Rosales Water District

are the loyal employees, John A. Ulpindo, Laura S. Causin, Benjamin C. Sayson, Jr., Edmundo S.

Alcantara, Angelo C. Francisco III, Ismael G. Tuaño, George B. Sagun and Ronaldo C. Galvez, who

devoted time and effort even at the times of negativity almost bankruptcy when it experienced financial

difficulty. Interim General Manager (IGM) Edwin R. Reyes, Technical advisers, Engr. Alfonso Basco and

Engr. Armando Laroco also contributed priceless effort to the establishment. The members of the take-

over BOD were Engr. Simplicio Belisario, Engr. Antonio Magtibay, Engr. Antonio Calvelo, Mr. Dexter

Bragas, Atty Elena Te and Recording Secretary Sikatala Zamora.

The Rosales WD is geographically located in the municipality of Rosales, 6th district, province of

Pangasinan with a latitude of 15-53-43.94 and longitude of 120-32-55.33. The landmark in Rosales

known as “Tangke” constructed on 1930 laid the office of the Rosales Water District addressed at No. 5

Bonifacio St., Zone II, Rosales, Pangasinan. The Rosales Water District has one (1) 150 cu m Steel Tank,

one (1) 380 cu m Concrete Reservoir and seven (7) wells sources catering 36 out of 37 Barangays in

Rosales, Pangasinan. As of December 31, 2015, RWD active service connections were 7,831

The Rosales WD is headed by Mr. JOHN A. ULPINDO as the General Manager, assisted by two (2)

Department Managers, two (2) Division Managers, twenty two (22) regular personnel and twelve (12)

recruits hired on Job Order basis to be able to effectively and effciently serve the public particularly its

concessionaires.

Mandate and Function

Pursuant to Presidential Decree No. 198 (Provincial Water Utilities Act of 1973), the Local Water

District Law, Rosales WD mandate and function:

(a) Acquiring, installing, improving, maintaining and operating water supply and distribution systems for

domestic, industrial, municipal and agricultural uses for residents and lands within the boundaries of

such districts,

(b) providing, maintaining and operating wastewater collection, treatment and disposal facilities, and

(c) conducting such other functions and operations incidental to water resource development,

utilization and disposal within'sueh distrietsras. are' necessary or incidental to said purpose.

We, at the Rosales Water District, are committed to provide our concessioners with safe and potable water that conforms with the existing principles of the Philippine National Standards for Drinking Water (PNSDW).

We shall comply with all statutory and regulatory requirements in the course of conducting our business operations.

Provide uninterrupted 24 hours potable and safe water supply and service

Maintain minimal water rate in the most possible techniques for the convenience of our concessioners

Preserve the water resources, development and uphold watershed

Provide fast and efficient billing and collection services to our clients through an enhanced electronic billing and collection system

Install new service connections with the set standards and policies

Promptly attend to requests for connection of our water service

Provide timely technical maintenance in the most economical cost and methods, preservation/rehabilitation of old water pipe main and service lines

Expand and extend water supply and service in the most remote area of the town, observe periodic preventive maintenance, flushing and blowoffs

Provide quality Administrative services

Give priority to feedback and concerns.

We shall strive to exceed concessioners expectations by keeping in stride with the latest technology to improve our water quality, pressure, supply availability and to increase our efficiency in handling concessioners concerns and issues.

IV- ORGANIZATION AND RESPONSIBILITIES

Organizational Structure

The organizational structure was based on the implementation of the Re-categorization

of the approved Position Allocation List (PAL) of Rosales Water District in accordance with DBM

Circular Letter No. 2011-10 dated Novembert 18, 2011 effective not earlier than May 01, 2015.

Duties and Responsibilities

A. Board of Directors – functions are limited to policy-making body per Sec. 18 Chapter V – Local

Water District Law of PD 198.

B. Office of the General Manager- linkages between policy-making body, the personnel, the government regulatory bodies and other outside sources and clients. Directs the agency workforce to attain the mission, vision and goals.

1. Observes, controls, maintains and oversees the day to day affairs/operation of water district towards achieving the district’s goals.

2. Recommends/reports matters in connection to water system operation and objectives for consideration/approval/inofrmation of the Board of Directors. Communicates effectively with the BOD

3. Implementation of policies/rules and regulations as set/approved/considered by the Board of Directors and by the regulatory commissions of the government

4. In behalf of the district, approves disbursement of funds and order payments made to and by the RWD, executes conrtracts and agreements mande into and by the RWD. Responsible for carrying-out the mandate of the RWD with its staff as prerequisites. Approves appointments of RWD employees subject to the CSC regulations. Evaluates staff performance

5. Receives reports/suggestions from the two Department Heads of the RWD, analyzes the details and prepares clear and concise reports for the BOD.

Administrative/General Services/Finance and Commercial Department covers the Administrave and General Services Division and the Finance and Commercial Division of the district which are responsible for the efficient and quality services to be conveyed to the general concessioners of the water district in terms of trade, source, usage and reportorial of monetary aspects, human resources, information dissemination and the business requirements transactions center for the continuity of the water district operation.

Department Manager C

Supervises basic efforts of the Administrative/Finance/General Services Division and Commercial Division functions, activities, affairs and directs tasks which are complicated in nature as deemed necessary.

Observes the effectiveness of the direction, supervision and control of personnel, office and security services and the protection of good district’s image

Analyzes WD operations in order to formulate genuine, efficient and economical recommendation for betterment of the WD. Monitors and directs the preparation of accurate financial reports and records of the WD.

Monitors the implementation of District’s policies, directives relative to its goals and objectives to enhance an effective organization. Monitors the establishment of performance evaluation standards. Carry through, supports and monitors its implementation. Analyzes and formulate ideas for effective harmonious organization connection as may deemed necessary

Handles and oversees human resources, records and other pertinent documents relative to the source of manpower.

Performs finance and administrative duties as required. Performs other tasks as maybe assigned from time to time as the need arises.

C.1 . Administrative and General Services Division

Responsible for Property and Supply Custodian for the purchasing, storekeeping, issuance and report preparation for supplies, materials, equipments for the general operation of the district. Responsible for employees the best assets of the district, Human Resources Management ie selection of employees on Job Order basis training, development and improvements, the appointments, plantilla, relation to other government regulatory bodies, information dissemination and technology. RWD records keeper, filing and keeping of important WD documents. Provide assistance to the GM for the negotiation with the government regulatory bodies. Serve as assistant and the moderator of all divisions within RWD. Maintains cleanliness and provides general services wihtin the WD. Assist in upholding water shed program.

C.2. Finance and Commercial Division

Administer the business center system operation and trade promotion of water supply. The avenue for the generation of new service connection which will be added to the resources of the Rosales Water District ie. Marketing, orientation and the process of application of new service connection Preparation of Water Billing Statements and all other activities related thereto for the computation of the monetary equivalent of the main resource of the district Ie Meter Reading, Encoding, Distributing. Sensible with the implementation of the extensive/intensive collection of water bills current and arrearages. Dealt with the general concessioners concerns bordered on customer service, records and response to customers’ complaints and inquiries. Observes the no lunch break policy for the concessioners’ payment convenience. The front act liner for concessioners issues, and concerns. Distribution of disconnection notices and other notices for the information of the general concessioners of the water district. Responsible for the financial operation of the district: Budgeting and Accounting ie Disbursements Vouchers and its supportive attachments, Payrolls, Financial Statements Reports, Bank Reconciliation Statements and other related financial and accounting transactions and reports. Cashiering for the collection, deposits, checks issued, checks disbursed to payees, related cashiering reports.

Engineering, Construction, Production and Water Quality Department covers the Engineering and Construction Division and Production and Water Quality Division of the district which are responsible for the efficient and quality services to be conveyed to the general concessioners of the water district in terms of construction, field technical/structural operation and maintenance of the water source of the WD.

Department Manager C

Supervises and directs the Technical/Engineering, Production and Water Quality Division skilled personnel functions.

Responsible for monitoring the reports and activies in the production treatment and disinfection supply, scheduling and delivery of safe, potable, reliable and 24-hours water supply service. Establishes programs and procedures for economicaql operations of production facilities and other water appurtenances.

Monitors the maintenance of accurate records as to daily Equipment operation, machinery, histroy, water level and pressure including general field operations.

Directs, supervises and monitors all engineering functions for the growth and improvements of the WD as to construction programs. Based upon job maintenance orders, inspects and checks the accuracy of all the related jobs done.

Monitors the implementation and effectiveness of the performance standards procdures.

Performs complicated managerial tasks as requiired from time to time.

D.1 . Engineering and Construction Division

Engaged in the expansion and construction development projects and/or manage the outside contractors who will perform the improvement works/activities of the expansion/extension of pipelines project. Monitoring and implementationof the well drilling, pumphouse construction and all other related technical and engineering activities for the developments and improvements to support and enhance the main resources of the water district. Responsible in the general sustainability of water supply and system , the estimate, installation and maintenance of water meter, transmission and distribution, the water main and service lines leakages, the fire hydrants, blow offs and flushing. The transmission and distribution system maintenance. Install new service connections with the set standards and policies. Timely technical maintenance in the most economical cost and methods. Preservation/rehabilitation of old water pipe main and service lines. Expand and extend water supply and service in the most remote area of the town. Observe periodic preventive maintenance, flushing and blowoffs.Engineering functions and monitoring of contractor’s performance.Uphold watershed development and improvements for greening and forest reservation program.

D.2. Production and Water Quality Division

Respondent and assistant to Local Water Utilities Administration in the conduct of feasibility study as site of ground well drilling and/or surface water to enhance the sources of water as the main resources to sustain the delivery of water district supply. Compliant to water permit application in National Water Resources Board and reportorial requirements of well/surface water data. Responsible to the ground water and/or surface reservoir to augment and or boost the resources of the water district. Responsible for the maintenance, in general, like the wells preservation, the reservoir, the pumping operation and its facilities, structures, mechanisms and conduits, the water treatment operation and its apparatus. Responsible for the adequacy, availability, stability and continuity of uninterrupted 24 hours potable water supply in the whole area of Rosales Water District coverage such as hourly monitoring of water pressure, observation of the behavior, function and vital operations efficiency of the panel board, the pumping facilities and appurtenances. Sustain good pumphouse keeping. Overseer of the utilization and operation of the power production and the equipment for alternate power source. Administering right amount of water treatment chemicals and overseer of the water treatment equipments.

V- OPERATIONAL CONTROL AND SUPERVISION

Board of Directors

Functions limited to Policy-Making, to establish policy and shall not engaged in the detailed management of the district

General Manager The General Manager shall exercise operational control and supervision over the following:

Over-all operation of the WD and general supervision of personnel

Plans, proposes and recommends policies, rules and regulations including budget for board reference, consideration and approval actions

Preparation of agenda for BOD meeting

Represents the WD in all actions and undertakings

Implementation of agency’s policies, rules and regulations

Appointing authority

Administrative/General Services/Finance and Commercial Department

Supervises and controls the Administrative/General Services Division and the Finance and Commercial Division personnel in carrying-out it’s functions, duties, responsibilities, performances and over-all undertakings:

Administrative and General Services Division

Controls and supervises the Human Resource, Property and Supply Management and General Services Section

Human Resource Section -

Maintainance of Personnel Files such as 201 Files, leave records, service records and other related documents

Assistance to the Personnel Selection Board (PSB) of the district

Preparation/Maintenance of all documents relative to recruitment , promotion, retention, rewards and recognition, personnel relations and retirement, ie. Appointments for employees and BOD, List of Officials and employees granted step increment (LOEGSI) , Plantilla etc...

Provides/Review/Consolidation/Submission of all undertakings/data relative to personnel, ie: the SALN to the Office of the Ombudsman, BIR Forms 2316, 2305, GSIS, HDMF, Philhealth, DBM, other regulatory bodies and the like.

In-charge in the maintenance/keepers of chronicles of all actions, reports and monitoring relative to the manpower of the RWD

Assists in the preparation of re-categorization documents

Directly reports/communicates/participates/represents the RWD and its personnel in CSC as the central personnel agency of the government

Implements CSC programs relative to personnel, ie: Gender and Development Activities, Health and Wellness Program, Conducive working area, Benefits and Priveleges for employees.

Property and Supply Management Section -

Control over property and supply physical inventories good housekeeping

Preparation of documents relative to the procurement process

Prepares Annual Procurement Plan

Administers Philgeps posting and other related process

Maintains inventory-taking records, Inventory Reports, Unserviceable properties records , report of waste materials and those supplies and materials for disposal process.

Prepares Inspection and Acceptance Reports, Report of Supplies and Materials Issued, Report on the Physical Count of Property, Plant and Equipment, Report on the Physical count of Inventories, Report of Accountability for Accountable Forms.

Maintains Property, Plant and Equipment Ledger Card, Property Card, Supplies Ledger Card and Stock Card, Prepares Property Accountability Receipts (PAR)

In-charge with PR, RFQ, PO, SRS, PPE and other equipment and supplies related forms and reports, procurements documentations and deliveries.

General Services Section –

Control over security and protection of the district

Maintains cleanliness and orderliness of office and its premises as well as the office furnitures and equipments

Errand and messengerial activities

Maintains RWD office kitchen, cooking area its premises and utencils in good housekeeping

Provides assistance to GM’s office in filing records, resolutions etc.

Accomodates visitors of the RWD

Maintains cleanliness and ordeliness of mini gym and its sports equipments

Asssits in the good condition maintainance of service vehicles Finance and Commercial Division

Controls and supervises the Sections of Cash Management Collection, Accounting/Budgeting, Billing and Commercial

Cash Management Collection Section

Safekeep cash and all other cash items in behalf of the WD

Issue Offical Receipts for the collections of water bills payments and other collections due for the water district and Prepares checks for disbursements. Cash Disbursing Officer.

Prepares Daily Cash Collection and Daily Cash Position Report

Maintains Cashier’s books of records relative to Cash, Banks and safekeeps Bank passbooks and records

See-to-it that collections for the day are deposited in-tact

Petty Cash Custodian

Prepares and Maintains Report of Collection and Deposits, Report of Checks Issued, Report of Accountability fort Accountable Forms, Liquidation Report of Cash Advances, Cash Receipts and Deposits Records, Petty Cash Fund Records, Cash Disbursement Records, Check Disbursement Records and Cash Bank Records.

Accounting/Budgeting Section

Preparation of FS, MDS and other financial related reports

Preparation of Disbursement Vouchers and related documents

Update Book of Accounts such as GLs, Subsidiary Ledgers, MSIJ, CashDJ, CDJ, GJ and CRDJ

Update PPE and stock inventory card

Preparation of Bank Reconciliation Statement

Preparation of Payroll

Preparation/Consolidation of Annual Budget

Maintains BUS and other related Budgeting data and needed reports

Maintains Files of submitted reports to LWUA, COA and other financial regulatory bodies Billing and Commercial Section

Front liners to serve the concessioners in the public assistance information and complaints desk

Attends to all activities/actions pertaining to concessioners needs such as application for new service connection , requests, complaints , suggestions and the like

Provides water supply, service, utility rules and regualtions orientation for applicants of new service connections

Maintains an accurate and up-to date conscessioners records

Provides Senior Citizens, Persons with Disability and Pregnant Women priority lane

Meter reading of water consumption. Checks the accuracy and veracity of water meters function

7. Process for Application of Senior Citizen Discount

Section. 1. To avail of the 5% discount, the senior citizen shall: 1. apply for the discount personally or thru a representative. There shall be an annual renewal of

12. Water Production , Treatment, Transmission and Distribution Flow Process

VIII. STANDARD RULES, POLICIES AND REGULATIONS FOR THE ROSALES WATER DISTRICT PERSONNEL

COVERAGE AND DEFINITION OF TERMS SECTION 1. Except as otherwise provided, the following rules stated herein shall apply to all employees of the Rosales Water District both in the career and non-career service, to wit;

1.Attendance to Flag Ceremony 2.Attendance to all the programs and activities of the RosalesWD. 3.Wearing of the Prescribed Uniform and Dress Code 4.Rendering of the Prescribed Working Hours 5.Wearing of the prescribed Office Identification Card 6.Attendance to Trainings/Seminar

SECTION 2. Definition of Terms 2.1 Attendance to Flag Ceremony – refers to the mandatory attendance of all officials and employees of the Rosales Water District to the flag raising ceremony every Monday at 8:00 in the morning. 2.2. Attendance to all the programs and activities of the Rosales WD – refers to the mandatory attendance of all officials and employees of the Rosales Water District to all the programs and activities of the Agency whenever required. 2.3. Wearing of the Prescribed Uniform – refers to the use of the prescribe uniform for the year. 2.4. Office Hours – refers to the prescribed government working hours which is “equivalent to eight hours of work a day for five (5) days a week (Monday to Friday) or a total of forty (40) hours a week, exclusive of time for lunch from 8 o’clock in the morning to 12 noon and from 1 o’clock to 5 o’clock in the afternoon on all days except Saturdays, Sundays and Holidays as per Section 5, Rule XVII of the Omnibus Rules Implementing Book 5 of Executive Order No. 292. However, employees who render services on a shifting of work basis shall be exempted to the required office hour provided that employees will still render the required forty (40) hours a week for the duration of the entire workweek. 2.5 Wearing of the Prescribed Office Identification Card – refers to the wearing of the Official ID issued while on tour of duty or at place of work. 2.6. Attendance to Trainings and Seminars - Trainings/Seminars attended through official invitation from different offices, departments and agencies. 2.7. Semester- refers to the period from January to June and from July to December in a year.

II. ATTENDANCE TO FLAG CEREMONY SECTION 3. All employees of the Rosales Water District, career and non-career such as casual and co-terminous employees must attend the flag ceremony every Monday at 8:00 in the morning.

ATTENDANCE TO ALL THE PROGRAMS AND ACTIVITIES OF THE ROSALES WD SECTION 4. As will be required through the issuance of a Memorandum, all officials and employees of the RosalesWD MUST attend to the regular and special programs and activities of the Agency such as follows;

Independence Day

Employees Sports festival

LGU activities during Town Fiesta

Wreath laying activities to honor UnknownSoldiers

And all Other Activities which the RosalesWD Needs to participate example as : Tree Planting, FaMEALy Day, Others as required

WEARING OF THE PRESCRIBED UNIFORM/DRESS CODE Section 5. All RosalesWD officials and employees must wear the prescribed office uniforms which is the official attire to be worn in accordance with the assigned schedule. Section 6. Consistent with the rule specified in Section 2, paragraph 2.3, the prohibitions enumerated in Memorandum Circular No. 14, series of 1991 of the Civil Service Commission shall apply to this Rule, as such;

On those days when employees are exempted from wearing the prescribed office uniforms, they must be dressed appropriately, preferably business clothes but not party attire, picnic clothes, sandos nor t-shirts should be worn at work.

The use of tight fitting, seductive, micro-mini and gauzy/flimsy/transparent dresses by female employees shall be prohibited. Walking shorts, pedal pushers, leggings, tights, jogging pants shall likewise be prohibited.

The use of too much costume jewelry, flashy bangles and similar accessories shall likewise be prohibited. Conversely, ostentatious display of expensive jewelry is strongly discouraged and prohibited except for special occasions and official celebration.

The wearing of heavy or theatrical make-up is likewise prohibited.

The wearing of slippers, sandals, bakya, etc. in the office premises is banned. Only appropriate footwear shall be allowed.

It is likewise considered taboo to use curlers, turbans and bandannas during office hours within office premises.

Exemptions may be allowed on the following reasons:

when by the nature of work of the employees concerned ,they necessarily must wear other appropriate clothing;

religious affiliation or creed or any practice in relation thereto;

physical disabilities;

other analogous circumstances of any of the afore-mentioned.

Any employee who failed to comply with to the prohibitions mentioned in this Section for three (3) times in a month in a semester without valid cause shall be a ground for disciplinary action. The penalty for violation committed is as follows; (CSC MC No. 14, S.1991).

1st Offense – Reprimand 2nd Offense – Suspension of 1-30 days 3rd Offense – Dismissal

Section 7. The Rosales WD HRMO, shall assign authorized personnel to conduct separate monitoring in all employees of RosalesWD.

V. OFFICE HOURS IN THE ROSALES WATER DISTRICT

Section 8. All RosalesWD employees must strictly observe the prescribed office hours which shall be from 8 o’clock in the morning to 12 o’clock noon, and from 1 o’clock in the afternoon to 5 o’clock in the afternoon on all days (Monday to Friday) except for Saturdays, Sundays and Holidays. A grace period of thirty minutes (30) shall be allowed. Beyond 8:30 AM will observe the Flexi time with the notification to the General Manager and monitored by the HRMO. Skeletal Force in Collection and Repairs and Maintenace must be observed during Saturdays, Sundays and Holidays. Production and Techncial Personnel must observed the compensatory time-off procedure. Section 9. Agency Head shall require all employees including those serving in the field to accomplish and submit the Daily Time Record (DTR) of attendance. Section 10. Flexible working hours may be allowed to complete the 8 hours working time. Request for re-scheduling or shifting of work schedule maybe allowed provided that employees will still render the required forty (40) hours a week for the duration of the entire workweek. Flexible working hours will be monitored by the HRMO with the General Manager’s instruction and approval. Section 11. Falsification or irregularities in the keeping of time records will render the guilty officer or employee administratively liable. Section 12. Employees who have incurred tardiness and under time regardless of the number of minutes per day in a month shall be deducted from leave credits earned. Section 13. Officers and employees who have incurred tardiness and under time regardless of the number of minutes per day, ten times a month for two consecutive months in a semester shall be subject to disciplinary action. The penalty for such offense is as follows;

1st Offense- Reprimand 2nd Offense- 1 to 30 days suspension 3rd Offense- Dismissal

Section 14. Off-setting of tardiness or absences by working for an equivalent number of minutes or hours by which an officer or employee has been tardy or absent, beyond the regular or approved working hours of the employees concerned, shall be allowed. Section 15. When the interest of public service so required, the daily hour of work for officers and employees may be extended by the head of Agency, which extension shall be fixed in accordance with the nature of work. Provided, that work in excess of eight (8) hours must be properly compensated. Section 16. As a rule, Officers and employees who incurred under time regardless of the number of minutes/hours, ten (10) times a month for at least two (2) consecutive months in a semester shall be liable for Simple Misconduct and or Conduct Prejudicial to the Best Interest of the Service as the case maybe (CSC MC 16, s. 2010). 1st Offense- Suspension for 6 months and 1 day 2nd Offense- Dismissal

VI. WEARING OF THE PRESCRIBED OFFICE IDENTIFICATION CARD Section 17. All employees including those in the field must wear the official ID at all times during office hours. Non-wearing of ID shall ban the employee from entry or access of the office.

Section 18. Any employee who failed to comply with the provisions of Section 23 of this Rule three (3) times in a semester without valid cause shall be subject for appropriate action. The penalty for offense committed as follows; 1st Offense- Warning 2nd Offense- Reprimand 3rd Offense- Community Service Section 19. Lost or damaged ID must be properly reported to the Human Resource Management Officer for replacement and for record purposes.

VII. ISSUANCE OF PASS SLIP Section 20. Agency head shall issue Pass Slip to employee who will go under time and/or during emergency cases that will need for him to leave post while on duty, provided, such employee shall return to the office for the duration of the subscribed office hour.Employee performing official business outside of his station shall also be issued a Pass Slip.

VIII. MISCELLANEOUS PROVISIONS Section 21. The Human Resource Management Officer shall be responsible for the monitoring and implementation of this rule, regulations and policies for the employees of the Rosales Water District. Section 22. Consistent to the rules herein, Memoranda, Circulars and issuances of the Civil Service Commission, the pertinent provisions of RA 6713 and other pertinent laws shall form part of this Rule. Section 23. If any section or part of these Rules shall be held to be invalid, the remaining provisions shall be given full force and effect as completely as if the part held invalid not been included therein. Section 24. This Rule shall take effect immediately.
