    Nagler`s reaction: is used to detect alpha toxin(lesithinase activity) of Clostr id ium perfr ingens.

    Test principle: (Precipitation)

    Egg yolk agar is inoculated with alpha toxin Abs,

    once the toxin is produced, it will bind to the Abs

    and will precipitate.

    erythema marginatum, carditis = murmur, subcutaneous nodules.

    Must have: History of sore throat 4-8 weeks prior to the presentation

    Migratory arthritis: The inflammation travel from one joint to another

    Diagnosis: Rheumatic Heart disease if murmur existed (carditis)

    Rheumatic Fever if no murmur

    Non-specific symptoms: Fever, malaise

    Upper resp. tract symptoms: sore throat, cough (URTI and LRTI).

    O/E: Pharyngitis: pharynx is inflamed, Tonsillitis: tonsils enlarged and

    inflamed

    Lower resp. tract symptoms: can have URTI symptoms in addition to

    Tachypnea (fast breathing), shortness of breath, chest pain.O/E: Subcostal/intercostal recession, Auscultation: Crackles, crepitation

    Chest x-ray: always taken for LRTI- haziness, consolidation
