ACUTE DIARRHEA
แนวทางการใชยาปฏชิีวนะในการรักษาโรคทองรวง
เฉียบพลัน (ยกเวนอหวิาหตกโรค)
1. โรคทองรวง หมายถึง การถายอุจจาระเหลวจํานวน 3 ครั้งตอวันหรือมากกวา หรือถายมีมูกหรือมูกปนเลือด หรือถายเปนน้ําอยางนอย 1 ครั้ง40a, 40b
2. หากผูปวยมีอาการอาเจียนเปนอาการเดนหรือถายเปนน้ํามักหมายถึงโรคอาหารเปนพิษ41a ซึ่งมีสาเหตุจากการกินสารพิษของแบคทีเรียที่ปนเปอนอยูในอาหาร จึงไมตองใชยาปฏิชีวนะในการรักษา41a
3. ในจํานวนผูปวยโรคอุจจาระรวง 1,433,230 รายที่กรมควบคุมโรคไดรับรายงานจากสถานพยาบาลตางๆ ในปพ.ศ.2550 มีผูปวยเพียง 19,026 ราย (1.3%) ที่เขาขาย “โรคบิด” และควรกินยาตานจุลชีพนอกนั้นเปนผูปวย “โรคทองรวง” หรือ “อาหารเปนพิษ” ที่ไมควรกินยาตานจุลชีพ42a

4. เชื้อแบคทีเรียที่เปนสาเหตุสาํคัญของ infectious diarrhea
ไดแก Campylobacter jejuni, Escherichia coli, Salmonella
และ Shigella44a
5. ควรใหยาปฏิชีวนะเฉพาะการติดเชื้อ Campylobacter,
Escherichia coli (ที่ไมใช enterohemorrhagic type หรือ STEC)
และ Shigella45a โดยเฉพาะอยางยิ่งหากมีอาการรุนแรง
หรือเปนผูปวยที่มีภูมิตานทานโรคต่ํา
The routine use of antimicrobial agents fortreating diarrhea wastes resources and might leadto increased antimicrobial resistance. Even whena bacterial cause is suspected in an outpatientsetting, antimicrobial therapy is not usuallyindicated among children because the majority ofcases of acute diarrhea are self-limited and notshortened by antimicrobial agents.

7. การใชยาปฏิชีวนะในกรณีการติดเชื้อ E. coli O157:H7
(Shiga toxin producing E. coli O157-STEC O157) เสี่ยง
ตอการเกิด HUS (Haemolytic Uremic Syndrome) ซึ่งอาจ
เปนอันตรายถึงชีวิตจึงไมควรใชยาปฏิชีวนะในกรณีนี้47a
8. เชื้อ Campylobacter, Enteroinvasive Escherichia coli
และ Shigella จัดเปนทองรวงชนิด dysentery คือมี
อาการไข ปวดทอง ปวดเบงและถายเปนเลือด การตรวจ
อุจจาระอาจพบเม็ดเลือดแดงและเม็ดเลือดขาว48a
นอกเหนือจากอหิวาห องคการอนามัยโลกระบุวา การ
ใหยาปฏิชีวนะเปนประโยชนในเด็ก เฉพาะเมื่อเด็กถาย
อุจจาระมี เลือดปน49a สําหรับผูใหญก็ใชหลักการ
เดียวกัน
9. การใหยาปฏิชีวนะควรใหเฉพาะผูปวยทองรวงที่มี
อาการรุนแรง ซึ่งเขาขายดังตอไปนี้คือ48b, 49
9.1 มีไข มากกวา 38oC
และ
9.2 อุจจาระเปนมกูหรือมีเลือดปนเห็นไดดวยตา
เปลา หรือ ตรวจอุจจาระพบเม็ดเลอืดแดง และ/
หรือเม็ดเลอืดขาว
ขอสังเกต หากถายเปนเลอืด โดยไมมีไข อาจเกิดจาก STEC45b
10. ยาปฏิชีวนะที่ควรใช 45c
10.1 ในผูใหญ ใช norfloxacin 400 มิลลิกรัม วัน
ละ 2 ครั้ง ขณะทองวาง นาน 3-5 วัน
10.2 ในเด็กอนุโลมใหใช norfloxacin ในขนาด
15-20 มก./กก./วัน แบงใหวันละ 2 ครั้ง ขณะทองวาง นาน
3-5 วัน ในกรณีที่คาดวาเชื้อดื้อตอ co-trimoxazole
หมายเหตุ ปจจุบันเชื้อ Shigella ซึ่งเปนเปาหมายในการ
รักษามีความไวตอ norfloxacin รอยละ 100 และไวตอ co-
trimoxazole ประมาณรอยละ 3-26 (ขอมูลป 2007)50
หมายเหตุ ไมควรกิน norfloxacin รวมกับแคลเซียม
แมกนีเซียม ธาตุเหล็ก ธาตุสังกะสี หรือยาลดกรด เพราะยา
เหลานี้จะจับกับ norfloxacin ทําใหยาดูดซึมไดนอยลง ถา
จําเปนตองกินรวมกัน ใหกินยาเหลานี้หางจาก norfloxacin
ไมนอยกวา 60 นาที51
11. ยาปฏิชีวนะอื่นๆ เชน co-trimoxazole และ tetracycline
มีความไวต่ําตอเชื้อเปาหมาย50 จึงไมควรใชยกเวนเพาะ
เชื้อแลวพบวาเชื้อไวตอยา (รายละเอียดของแนวทางการ
รักษาดูจากภาคผนวก ข)
A hospital survey of more than 30,000 stool specimens sent for testing revealed a specific pathogen in only 5.6 percent of cases. In descending order, the most commonly identified pathogens were C.jejuni, Salmonella, Shigella and E. coli O157:H7. (จาก Ref 47a)

This study explores Thai physicians’ rationales about their prescribing practice for treating childhood diarrhoea within the public hospital system in central Thailand. A prospective clinical audit and observations of 424 cases treated by 38 physicians. Antimicrobial use was the most serious area of inappropriate prescribing; of the 363 children who received antimicrobials, 75.5% were incorrectly treated according to standard treatment guidelines. Most doctors studied routinely prescribed antimicrobials even for non-bloody diarrhoea

Selective antibiotic treatmentwas recommended for traveler's diarrhea, Shigellosis andCampylobacter infection. The role of antibiotic therapy in Salmonellosisand E. coli O157:H7 infection remains unclear. Avoidance ofantimotility agents in bloody diarrhea is emphasized, especiallywhen illness is caused by E. coli O157:H7, which could increasethe risk of subsequent hemolytic-uremic syndrome.

INFLAMMATORY DIARRHEAThe presence of fever and bloody diarrhea (dysentery)indicates colonic tissue damage caused by invasion(shigello salmonellosis, Campylobacter or Yersiniainfection, amebiasis) or a toxin (C difficile, E coliO157:H7). Because these involve predominantly the colon,the diarrhea is small in volume (< 1 L/d) and associatedwith left lower quadrant pain and tenesmus. Fecalleukocytes or lactoferrin usually are present in infectionswith invasive organisms.

and 91% were from patients with a history of bloody
diarrhea; such specimens tended to be from patients with
less severe fever but more abdominal pain than specimens

species.
Right-side abdominal pain without high fever but with
bloody or nonbloody diarrhea should prompt culture for
Shiga toxin–producing Escherichia coli (STEC) O157.

Early refeeding with milk or food after rehydration does not prolong diarrhea; there is evidence that it may reduce the duration of diarrhea by approximately half a day and is recommended to restore nutritional balance as soon as possible.
Data on antidiarrheal agents were not sufficient to demonstrate efficacy; therefore, the routine use of antidiarrheal agents is not recommended, because many of these agents have potentially serious adverse effects in infants and young children.

10.2 "Antidiarrhoeal" drugsThese agents, though commonly used, have no practical benefit and are never indicated for the treatment of acute diarrhoea in children. Some of them are dangerous.Adsorbents (e.g. kaolin, attapulgite, smectite, activated charcoal,cholestyramine). None, has proven practical value in the routine treatment of acute diarrhoea in children.Antimotility drugs (e.g. loperamide hydrochloride, diphenoxylate with atropine, tincture of opium, camphorated tincture of opium,paregoric, codeine). They can cause severe paralytic ileus, which can be fatal, and they may prolong infection by delaying elimination of the causative organisms. None of these agents should be given to infants or children with diarrhoea.Combinations of drugs. Many products combine adsorbents,antimicrobials, antimotility drugs or other agents. Such combinations are irrational and their cost and side effects are substantially higher than for individual drugs. They have no place in the treatment of diarrhoea in children.
