Introducción
• Los defectos del tubo neural
corresponden a un grupo de
malformaciones, en su mayoría
ligados a una alteración en el cierre
del tubo neural, o neurulación
primaria.
• Corresponde a la segunda causa de
malformaciones congénitas:
– 323.000 rnv en el mundo (2001)
The March of Dimes Global Report on Birth Defects: The Hidden Toll of Dying and Disabled Children. 2006.

Introducción
• Prevalencia histórica 1/1000 rnv
– 0,5-0,8/1000 en EEUU y Europa
• 0,2/1000 post fortificación con ácido fólico
– 7/1000 en norte de China
• Riesgo de recurrencia: 2,5-4,5%
– Se reduce en 66% con suplementación
de a. fólico -Frey L, Hauser WA. Epidemiology of neural tube defects. Epilepsia 2003; 44 Suppl 3:4.

Etiología
• La mayoría de las veces aislada y
multifactorial
• Síndromes genéticos: Meckel-Gruber,
Roberts
• Trisomias 13 y 18, triplodias
• Asociada a extrofia cloacal, teratoma
sacrococcígeo o bandas amnióticas

Factores de riesgo en forma
aislada • Bajos niveles de folato
– Baja ingesta
– Antagonistas del folato: metotrexato
• Uso de antiepiléticos: ácido valproico, carbamazepina
• Antecedentes familiares
• Diabetes mellitus
• Obesidad
• Hipertermia
• Bandas amnióticas

Screening
• 90-95% de casos de DTN ocurre en pacientes sin FR screening universal
• Alfa fetoproteína – Globulina fetal específica, sintetizada en
saco vitelino, tracto GI e hígado
– Screening ideal 16 a 18 semanas (15 a 20 sem)
– Alterado > 2 a 2,5 MoM
– Requiere dos valores alterados
– Sensibilidad 75%, especificidad 97%
– Elevada: US vs Amniocentesis (aumento de AFP + presencia acetilcolinesterasa)

Screening
• Ultrasonografía
– Dado avances puede utilizarse como
técnica de screening y diagnóstico.
– Expertos
• 97% sensibilidad
• 100% especificidad
– ¿Uso combinado si ecografistas no
expertos?

Diagnóstico 22 sem
• Imagen quística similar a lesiones
abiertas en zona de defecto
• Ausencia de signos indirectos
• Post natal: cambios cutáneos como
lipomas, hemangiomas, hipertricosis

Espina bífida cerrada
Diagnóstico 22 sem b) Lipomielomeningocele
-Alteración de la disyunción del ectodermo epitelial y tubo neural, por lo que mesénquima contacta TN abierto
-Éste induce formación de tejido lipomatoso
-Impide cierre de TN

Espina bífida cerrada
Diagnóstico 22 sem c) Diastematomelia
-División de médula espinal en dos
-Tipo A: un solo saco meníngeo. Sin dg prenatal
-Tipo B: dos sacos, separados por un septo sagital óseo, cartilaginoso o fibroso. Rol eco 3D. Asocia médula anclada.

Profilaxis – Suplementación
post fortificación
• Estudios de cohorte históricos muestran
beneficio de suplementación
– Sin antecedente DTN (RR 0,07 a 0,27)
– Con antecedente DTN uso altas dosis (RR 0,31)
• Post fortificación
– No queda claro beneficio adicional de
suplementación

Cirugía fetal
MOMS Study • Aleatorización cirugía 18-25 semanas
(n=91) vs cirugía post natal (n=92) – Necesidad shunt VP 44% vs 84%
(OR 0,53, 95% IC 0,41-0,67 )
– Mayores puntajes en evaluaciones cognitivas y motoras a los 30 meses de vida
– Mortalidad similar en ambos grupos (?)
– Aumento de parto prematuro, complicaciones respiratorias, DPPNI, dehiscencia histerorrafia, necesidad de transfusión
