Respectfully requesting the Sanggunian to pass a resolution authoriz_ing the Local Chief Executive to sign

the Memorandum of Agreement between the Rotary Club of Sta. Rosa and the City Government of

Santa Rosa.

The city is fortunate that the Rotary Club of Sta. Rosa will donate Physical Rehabilitation equipment to

our Santa Rosa Physical Therapy Rehabilitation Center under the supervision/ management of the Santa

Rosa Community Hospital.

The Memorandum of Agreement is attached for your reference.

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreem~nt ("MOA") made this day of 2014 in the City of Santa Rosa, Laguna, Philippines, and entered between:

The CIT\ GOVERNMENT OF SANTA ROSA, a local government unit, duly organized and existing under Republic Act No. 7160, also known as the Local Government Code of

1991, with seat of government at the Santa Rosa City Hall, J.P. Rizal Blvd., City of Santa Rosa, Laguna, represented in this act · by its · Mayor, HON. ARLENE B. ARCILLAS, hereinafter referred to as "LGU-SANTA ROSA";

and

ROTARY CLUB OF STA. ROSA, a duly constituted Rotary International Club under District 3820, a non-government civic organization, and represented herein by its President MR. PAULO VERA CRUZ, hereinafter referred to as "ROTARY CLUB";

WITNESSETH THAT:

WHEREAS, the Santa Rosa · Community Therapy Rehabilitation Center . is being

established thru coordination between the national government providing the 2-storey

building and the City Government of Santa Rosa providing the finishing of the

building, manpower, management of its daily operation;

WHEREAS, there are no publicly or government-run Physical Therapy Rehabilitation

Center in the City of Santa Rosa which has a population of 300,000, and people in need

of physical therapy services will need to pay Php300-500 per session for such services in

private clinics or therapist;

WHEREAS, the Rotary Club of Sta. Rosa in partnership with its sister club Rotary Club

· of Gtimi Geumo, South Korea is· willing to donate and provide Physical Therapy

equipment ("PT equipment") to the Santa Rosa Community Therapy Rehabilitation

Center in order to speed-up· and facilitate it~ operation in providing qua~ity healthcare

to the people of Santa Rosa as well as to its neighbouring local government units;

WHEREAS, the following Physical Therapy equipment (see Annex 1) is being donated

to the City Government of Santa Rosa to be used by the . Santa Rosa Community

Hospital Physical Therapy Rehabilitation Center;

WHEREAS, the Sanggunian Panlungsod of the City of Sta. Rosa Laguna, through its

SP# . has fully authorized the Honorable Mayor Arlene B. Arcillas to execute, sign and deliver this Memorandum of Agreement with the ROTARY CLUB.

IN VIEW THEREOF, the Parties hereby agree on the following covenants:

I. Undertaking of LGU Santa Rosa

I. The LGU Santa Rosa shall:

a. Use the PT equipment solely for medical use and physical therapy rehabilitation;

b. Use the donated PT equipment with due care and diligence;

c. Allow Rotary Club of Santa Rosa to put-up a Rotary Logo in front of the Santa

Rosa Community Therapy Rehabilitation Center. Provided, that the size and

location of the said logo must be with the prior approval of the LGU Santa Rosa.

Provided further, that any expense or cost in the putting up of the said logo shall

be for the account of ROTARY CLUB;

d. Upon acceptance of the PT equipment, cost of repairs and maintenance of the

said equipment shall be for the account of LGU Santa Rosa; on the other hand;

II. Undertaking of Rotary Club

II. The Rotary Club shall:

a. Regularly monitor the progress of the beneficiary's maintenance . as to the

usefulness of the donated equipment;

b. Whenever necessary, shall assist in the effective use of the equipment;

c. Promote the Santa Rosa Community Therapy Rehabilitation Center's services to

component barangays of the city and nearby local government units.
