Appointments with Doctors & NursesWe aim to offer appointments on the day of your choosing, though your practitioner of choice may not be immediately available. Please book your appointment online, via interactive TV, by telephone (automated or manual) or by calling in at the Practice. Please note that not all appointments are available on-line. Appointments may be booked in advance.

Nurse Practitioners and Practice NursesThe Practice Nurses run surgeries every day. They liaise very closely with the Doctors so can refer patients to them as appropriate. They deal with a wide range of problems including:-routine smears, foreign travel advice and vaccinations, general health issues, asthma, management of wounds and injuries, ear syringing, wart treatment, emergency contraception, follow-up for other contraceptive services, male/female sexual health, Chlamydia screening, eating disorders and minor ailments.

The following Clinics and Services are offered:-Asthma Clinic: Appointments available Monday – FridayDiabetes Clinic: Alternate Wednesday and Thursday afternoons 2.00pm – 4.40pm

Minor Ailments: Nurse Practitioners, who are able to prescribe for minor ailments, are available for telephone advice Monday-Friday between 08.45 and 10am. Appointments are also available throughout the day.

Travel Clinics: Monday - Friday Travel appointments are available throughout the year with the nursing staff. It is important that you submit your completed travel questionnaire at least 48 hours prior to your 1st appointment. This can be downloaded from www.sheffield.ac.uk/health, or obtained from our reception and sent to the University Health Service. Please ensure that your 1st appointment is at least 8 weeks before your departure and bring with you any record of your immunisations. The University Health Service is a certified Yellow Fever Centre. Please note that payment should be made via the University on-line store prior to vaccinations being given. www.shef.ac.uk/health/information/charges Payment will include a £5 administration charge for non NHS items of service.

Male Sexual Health Advice: Clinics are held each week with our male Nurse Practitioner or, if you prefer, you can see a male doctor. Appointments are available on-line.

Female Sexual HealthThese clinics are run by our female practice nurses. Appointments are available Monday – Friday.

• ContraceptivePillClinicDoctors and Practice Nurses are available to discuss contraception and give advice on all aspects of sexual health. To discuss a new method of contraception, please make a routine appointment with a Doctor or our Nurse Practitioner. To renew your current method, you may consult one of the Practice Nurses.

• Emergency ContraceptionThe emergency contraceptive pill is used to prevent pregnancy after unprotected intercourse. It is best taken and most effective within the first 24 hours but can be taken up to 72 hours after sex. A prescription can be obtained from the practice nurses. It is also free of charge at the Walk in Centre and Family Planning Clinics or may be purchased over the counter at pharmacies. | 2 |

• CervicalSmears(female patients)It is recommended that all women over the age of 25 have a regular cervical smear test. This can be undertaken at any time when you are not bleeding. We also offer routine chlamydia screening at this test. Please make a 20 minute appointment with a female Nurse.

ChaperonesIf you would like a chaperone during your consultation/examination, please ask at reception.

Pregnancy and maternal healthIf you are planning a family or are pregnant, please see a doctor and visit our website for helpful information and advice.

Eating Disorders We run an eating disorders clinic at the practice each Tuesday and Thursday throughout the year. New patients may self refer and can book an initial appointment at reception. Clinics are held on Tuesday and Thursday between 9.00am and 4.00pm.

Treatment for WartsLiquid nitrogen treatment for warts (and other lesions) is available in the practice on Tuesdays and Thursdays – after 10am.

Minor OperationsSeveral of the doctors perform minor surgery at the practice. Please make an appointment to discuss this with a doctor.

Phlebotomy ServiceThis is provided by the Practice Health Care Assistant.

PrescriptionsPlease allow 48hrs for collection. Prescriptions which have not been collected by 5pm on a Friday evening, are lodged at the weekend with Weldrick’s Pharmacy (below the Practice) on Glossop Road - and may be collected and/or dispensed from there on a Saturday morning before 1pm. Tel 0114 2761321. Prescriptions not dispensed will be returned to the surgery.

MedicalsAs medicals required for travel, employment or leisure are not provided by the NHS, there is a charge for these services, which are payable online. Details of fees are available at Reception and on our website.

The University Counselling ServiceTel 22 24 134 Hours 9.00 - 12.30 & 2.00 - 5.00pm in term time. Hours may vary during vacation.

The University Counselling Service, situated at 36 Wilkinson Street, offers a free of charge, confidential service to all members of the University community. It provides an opportunity to think and talk, in confidence, about difficulties, experiences, feelings, fears, thoughts anxieties and problems that may be concerning you now or have done in the past.

Out of Hours Services Nights, Weekends & Bank HolidaysPhone: 22 22 100 (24 HOURS)Overnight, at weekends and on public holidays, a doctor is always on call for emergencies – to give advice or visit as appropriate. As we use the Out of Hours Collaborative, it will not be a University Health Service Doctor who deals with your call. Calls will be answered by Local Care Direct then transferred to the GP Collaborative where all calls will be triaged by a Doctor. When appropriate, the Out of Hours Collaborative may ask you to attend their surgery premises at The Northern General Hospital or arrange to visit you, as appropriate.

Alternatively, you may phone NHS Direct on 0845 46 47, who will help you to determine the service you require or phone or visit the Sheffield GP Health Centre on Broad lane (0114 241 2700) which is open until 10pm daily.

Advice and InformationMeningitis and Meningococcal Disease www.Meningitis-Trust.Org.UkMany of you coming to University will have been vaccinated against Meningitis C. For those who have not, vaccination is free and is available at the University Health Service.

Meningitis is inflammation of the tissues that cover the brain and spinal cord. It can be caused by bacteria or viruses. Bacterial meningitis is the most serious form and is most usually caused by the meningococcal bug. This bug can also cause blood poisoning (septicaemia). Although these illnesses are rare, they are serious and need treatment. Symptoms of meningitis include:- headache, fever, vomiting, stiff neck and dislike of bright lights. Symptoms of meningococcal septicaemia include:- fever, cold hands and feet, muscle and joint pains, drowsiness and a rash. Symptoms can appear in any order and not everyone gets all these symptoms. Meningococcal septicaemia can occur with or without meningitis. Someone with meningitis or septicaemia will usually become ill very rapidly. CONTACT YOUR DOCTOR IF YOU ARE CONCERNED ABOUT SOMEONE WHO IS ILL, particularly if their condition is deteriorating.

ASK YOUR FRIENDS TO KEEP AN EYE ON YOU IF YOU ARE STAYING IN BED WITH FLU LIKE SYMPTOMS AND CONTACT A DOCTOR IF YOU ARE WORRIED.

Rights and ResponsibilitiesAll medical records are strictly confidential and information will not be given to anyone without your express consent.

You may also view your medical record or obtain a copy. Any copies requested are charged for. A Request to View Medical Records form is available on the UHS web pages. If for any reason you wish to view your Medical Record, please complete, sign and return this form to the Reception Office Manager, Mrs Gina Down who will contact you to make the necessary arrangements.

For requests for information from your medical record, please allow 7 days notice.

The Practice occasionally has young people present in surgery with a GP or Nurse who is intending to become a health care professional. You will be advised of this prior to your consultation and your consent will be sought. Please express your preference when checking in at reception.

Patients are asked to keep appointments or cancel in good time if necessary. Patients who habitually do not attend their appointments, may be asked to leave the Practice. Patients who are violent or abusive to staff or other patients will be excluded from the Practice List. The Practice has a written policy for the removal of patients from the List.

NHS RegistrationREGISTRATION usually takes place during Intro/Freshers’ Week, prior to the first week of the first semester. Registration with the University Health Service is open to all students registered with the University who have a Sheffield address, and their partners and children, who live within the boundary detailed in this leaflet.

At Registration, if an NHS number is provided, you will be issued with YOUR PIN NUMBER to access the on-line appointment system. Students who do not have an NHS number (ie new international students) please call at reception, approximately 6 weeks after registration, by which time the NHS number should have been issued to the University Health Service.

New patients registering at other times, please go to the UHS web site and follow the Registration Instructions.

The NHS Direct and NHS Choices websites can help you manage self-limiting minor illnesses (see web addresses).

Home VisitsIf you feel very unwell, please telephone the practice for advice before 10.30am. A Doctor or Nurse will discuss your symptoms with you and arrange for a doctor to visit if necessary. However, if at all possible, we prefer to see patients at the surgery where full investigation and treatment facilities are available. If you become ill later in the day, please try to telephone before 5.00pm.

Contacting the GP by PhonePatients needing to speak to a doctor should contact reception and the necessary arrangements with be made. The GP will telephone you back at a convenient time.

Sickness AbsenceUniversity regulations only require a Doctor’s certificate for continuous absence of more than 7 days, or in the case of exams, extension to work deadlines or recurrent illness. Please complete the special circumstances form available from the Student Services Information Desk (SIDS) in the Union, or on line at www.sheffield.ac.uk/health. If you are ill during an examination, you must see a Doctor or Nurse Practitioner the same day.
