• ♂36ª, fumador, enol. Filipino. Cocinero. • Tentativa previa de suicidio con arma blanca hace
3 años. • Vía pública. Dolor abdominal. Ingesta de
producto de limpieza. • PA: 115/80 mmHg; FC: 110´x; Tª: 36,3ºC. Afectación
estado general. Sialorrea. Lesiones eritematosas en lengua. ACV: taquicardia rítmica. AR: MVC. Abdomen: distendido, peristaltismo disminuido, defensa abdominal.

• Analítica: – EAB: 7,28; pCO2: 27,5 mmHg; bic: 12,9 mmol/L. – Hemograma: leucocitos: 25.250 (NS: 78%; NNS: 5%; L: 10%);
Hto: 54%; Hb: 180 g/L; VCM: 95 fl; plaquetas: 314000. TP: 93%. Bioquímica: glucosa: 186 mg/dl; Cr: 0,86 mg/dl; Na/K: 142/3,9 mEq/L. Ca/Mg: 8,5/2,5 mg/dl; ALAT: 18 UI/L; GGT: 160 UI/L; FA: 110 UI/L; bil total: 0,8 mg/dl.
• TC abdominal: engrosamiento de pared gástrica (cuerpo y fundus) y en la unión gastro-esofágica. Sin signos de perforación o mediastinitis.

ni estenosis. • IOT+VM. • Fibrogastroscopia urgente.

• Fibrogastroscopia urgente: – Esófago: La mucosa de todo el esófago, desde la boca
hasta el cardias, presenta úlceras superficiales con fibrina y circunferenciales.
– Estómago: Cardias, fundus y cuerpo gástrico con abundantes restos sólidos de color marronáceo que tapizan la pared gástrica y que traducen necrosis. No se puede evaluar la profundidad de la necrosis si bien por el aspecto endoscópico parece una necrosis superficial y extensa de la pared gástrica.
– Solo se visualiza parcialmente mucosa antral que está edematosa sin otras alteraciones.

cada 8h ev. • +12d: traqueostomía. • +13d: Fibrogastroscopia:
– Esófago con fibrina y puntos rojos. Estómago con necrosis de la mucosa. Duodeno con inflamación de la mucosa.

Cáusticos• Corrosivos.• Irritación ulceración
necrosis. • Ácidos (pH<4) o bases (pH>11). • TAR: “titrable acid
reserve/tritrable alkaline reserve”. Si TAR↑ ↑daño.
• Bases (NH3, NaOH): necrosis liquefactiva (úlceras profundas).
• Ácidos (HCl, H2SO4): necrosis coagulativa (escara).
• Lesión: agente, cantidad, concentración, estado, duración de contacto.

• Cutáneo, ocular, respiratorio o digestivo.• frecuente: ingesta:
– accidental (niños) poca cantidad ↓grave. • Pilas botón: álcalis!!!
– suicida (adultos) cantidad grave. • Accidental (“creía que era agua”).
– Lejía (HClO 5%): poca gravedad. • Suicida:
– Bases (NaOH, KOH): productos de limpieza. – Ácidos: (ClH, H2SO4, H3PO4): desengrasantes, piscinas…
• Daño: – Álcalis: esófago. – Ácidos: estómago. – Realidad: pH, capacidad oxidante, concentración y cantidad.

• Clínica: – Odinofagia, sialorrea, dolor retroesternal, epigastralgia,
nauseas, vómitos hemáticos. • Exploración:
– depapilación lingual, lesiones en descamativas /escaras en labios/mucosa oral.
• Graves: – no degluten, tos persistente (aspiración), disfonía,
sialorrea. Dolor palpación abdominal. – Peritonismo + shock perforación gástrica (esofágica
menos frecuente). – Dolor retroesternal + fiebre mediastinitis. – Hexafluorosilicatos: quelan Ca2+ arritmias graves.

• Diagnóstico: fibrogastroscopia. – Lesiones bucales, síntomas y/o corrosivo
concentrado o cantidad.– Ojo: no lesiones en boca ≠ no lesiones esófago-
gástricas. – Criterios de Zargar. – Contraindicaciones TC toracoabdominal.
• Inestabilidad hemodinámica. • Lesiones orofaríngeas graves. • Alta sospecha de perforación.

• Criterios de Zargar: – 0: normal. – I: edema de mucosa e
hiperemia. – IIA: úlceras superficiales,
sangrado, exudado. – IIB: úlceras profundas
focales o circunferenciales.
– IIIA: necrosis focal. – IIIB: necrosis extensa.
• Pronóstico: – I-IIA: buen pronóstico. – IIB-IIIA: estenosis 70-100%.– IIIB: 65% mortalidad

• Nunca: – Eméticos.– Neutralizantes exotérmico. – Sonda nasogástrica perforación.
• Edema laringe traqueostomía. • Ayuno. • AntiH+ (pantoprazol/esomeprazol). • Antibioterapia amplio espectro.• Corticoterapia 5-15d. Controvertido (grado III?)• Nutrición: parenteral gastro/yeyunostomía.

• Obvio: perforación (alta mortalidad).
• No tan obvio…. prevención de perforación, necrosis, septicemia, hemorragias tardías.
• Cirugía precoz: grandes ingestas (>150 ml), shock, acidosis metabólica, shock, acidosis grave, coagulopatía.
• Lapatorotomía, visualización, reparación (si se puede…). Intervención muy agresiva.

– 2 semanas- 6 semanas-años. – dilataciones /cirugía.
• Carcinoma esofágico:– riesgo x1000.– Media 41 años (13-71). – Endoscopias de control 15-20 años tras evento.

basal: 94%. Consciente, orientado, ↑afectación estado general, lesiones lengua y labios. AR: abdomen blando, depresible, doloroso, pero no peritonítico. Ruidos peristálticos abolidos.

• Priorizar estado del paciente.• Valorar vía aérea (laringe y estenosis!!)• Analgesia y sedación.• Factores de complicación temprana. • Casos graves –> cirugía precoz.
