Respected Members,
Greetings,
As we sit to put this last message together, we can’t
believe how quickly a year has passed.
Apart from scientific programs and other programs
arranged by Ahmedabad Medical Association, few require
special mention like dawn to dusk fast, cycle rally, closure of
routine work, participation in doctor’s maha panchayat at
New Delhi etc. all were about the call given by IMA HQ to
protest NMC Bill.
During all these time association had shown its unity
and strength. As one of the oldest and largest branch of India,
it is our responsibility to lead from front.
During last year, Ahmedabad Medical Association had
successfully hosted the state conference GIMACON 2017 and
an Annual conference of our association AMACON 2018.
Under Aao Gaon Chalen project, medical camp was arranged
at Dakor. PPS Zonal education seminar, CME on
Tuberculosis:“End TB initiative” were also arranged. Both
program of Gujarat State branch were hosted by Ahmedabad
Medical Association.
At this juncture, we would like to thank each one of you
for putting faith in us and supporting and guiding us
throughout the year. We are thankful to all the past presidents
for guiding us. We are thankful to office bearers, managing
committee members for their support and help. We are
thankful to office staff for their cooperation in the
administrative work.
As we are passing the baton of Ahmedabad Medical
Association to the new team, we are sure that they will carry
the Association to new height.
Ahmedabad Medical Association was, is and will
always be shaped by those who believe in betterment of
Medical Fraternity.
Our best wishes to the new team.

What is asthma?
Asthma is the disease of airways which are
hypersensitive and leads to variable airway obstruction. This
can change with time and improves with treatment.
The asthma can be presenting in many different ways and
it has to be suspected in patients
With history of allergy.
There are many risk factors for asthma.
Is genetic factor is responsible for asthma?
Yes.
Can smoking by pregnant women can affect children?
Yes. Smoking by pregnant mother leads to increase risk
of asthma in children. Even the risk of asthma may increases if
grand mother has smoked during her pregnancy.
Does caesarean section increase risk of asthma as
compared to normal delivery.?
Yes study showsthe risk of asthma increases 4 fold in
babies born by caesarean section as compared to normal
delivery.
What are the other environmental factors can be
responsible for respiratory disease?
Home wall dampness, pollution, fumes due to dhupsticks,
antimosquito coils and construction in surrounding areasetc.

What are the triggers for asthma?
There are many triggers known for asthma attacks.
Dust, fumes, ciggarate smoke, pollution, pollen etc.
There are more than 100 allergens known today.
What are the symptoms of asthma?
Patients with asthma are likely to have difficulty in
breathing, chest pain,coughing, severe coughing may be
present on exposure to allergen. Usually the patients feel
better once they expectorate sticky sputum. The difficulty in
breathing may be severe.
What is the pathophysiology of asthma?
The asthma is actually a cascade of inflammation. There
are many inflammatory mediators responsible for asthma.
Several cells, leukocytes play very important role in
development of asthma.
Research work in the field of asthma over several
centuries has lead to development of great knowledge in
understanding of asthma.
Based on knowledge we can treat the individual
inflammatory mediators and by that we can reduce the
asthma.

Yes there are many long acting molecules including
bronchodilators and steroids have came in, however they
may be useful in treatment of other respiratory conditions but
their benifits and risk are not very clear in asthma.
What is Bronchial thermoplasty?
The bronchial thermoplasty is a new way of treatment of
severe asthma.
With the help of interventional pulmonology and latest
advances we can Controll the asthma.
This treatment is done through bronchoscopy and a
radiofrequency cathater is advanced through the
bronchoscope working channel.
The catheter’send is applied at selected location.
With the help of the radiofrequency, smooth muscle
targeted will be ablated.
In this way we can reduce the smooth muscle
fibresnumbers within specific Bronchus. Not only that with
the help of thermoplasty same bronchus which has
undergone this procedure, we test the same bronchus by
exposing to allergen. The bronchus which was undergone the
procedure will not show bronchospasm. The other bronchus
which has not undergone thermoplasty will still show
bronchospasm.
This technique has started in India and in future it may
show a great impact.
However only selected patient after careful selection
should be subjected for the same.
Thus with a positive direction we should be able to
change the future of asthma patients.
Omalizumab
This is a new drug which shows good result in treatment of
asthma.
One of the important reason for triggering the asthma
attack is level of an Ig E antibody.
This antibody is also responsible for initiating the severe
bronchospasm.
The new drug is responsible for targeting this Ig E
antibody.
Omalizumabis Anti Ig E antibody. This external antibody
actually bind to the Ig E and remain bound to it so, Ig E can not
bind to the mast cell and allow to start
The inflammatory cascade, which can lead to asthma
attack.
The therapy consist of giving omalizumab in a
appropriate dosage as per the Ig E levels.
The therapy may be extended over a period if time for
more than few months.
The omalizumab can be given as a once a month or twice
a month therapy.
Thus therapy may block one of the major factor
responsible for asthma development And leads to reduction
in to asthma severity. The good news is the same drug wasnt
available in india before few years. But now the same
molecule is available in india for the treatment of asthma.
What is mepalizumab ?
The asthma cascade also contain IL-13, which is also one
of the important component of asthma inflamation.

This new molecule shows promising results in treatment
of asthma.
As it can lead to blockage of IL-13, making it unavailable
for the ashma inflammation.
As this inflammatory mediator gets out of inflammation, it
can lead to reduction in to severity of inflammation and can
lead to reduction of asthma severity.
What is fevipiprant?
This molecule is a PGD2 receptor antagonist. Newer
molecule in the treatment of asthma.
It will be helpful in treatment of asthma.
What is the latest treatment modalities for end stage lung
disease?
The best treatment modality for the endstage lung
disease is Lung transplant.
It helps to change both the diseased lung of the patient
and change them to new transplanted lungs.
This helps the patient to recover from incurable disease
and gives him a new life and a ray of hope.
