The University Medical Center Hamburg-Eppendorf is one of the largest hospitals among Germany’s university medical centers, with approximately 1,500 hospital beds. Each year the medical center treats around 80,000 in-patients, 260,000 out-patients and 113,000 emergency patients. The UKE successfully implemented an electronic medical record and achieved Stage 7 of the EMRAM Model. Although these types of projects typically have a strong IT focus, success factors are demonstrated in the areas of organization, processes, and especially change management. Frequently, established processes and methods are not directly applicable, but need to be adapted to the features and technical conditions of an electronic patient record along with its requirements and constraints.

Since 1985, the international healthcare community has considered the ideal rate for caesarean sections to be between 10-15%. Since then, caesarean sections have become increasingly common in both developed and developing countries. When medically necessary, a caesarean section can effectively prevent maternal and newborn mortality. Two new HRP studies show that when caesarean section rates rise towards 10% across a population, the number of maternal and newborn deaths decreases. When the rate goes above 10%, there is no evidence that mortality rates improve.The lack of a standardized internationally-accepted classification system to monitor and compare caesarean section rates in a consistent and action-oriented manner is one of the factors that has hindered a better understanding of this trend. WHO proposes adopting the Robson classification as an internationally applicable caesarean section classification system.Dr Mike Robson, creator of the Robson classification system is also the CMIO of the Irish National Maternal and new born clinical management system, a project that will see the deployment of Cerner Millennium to the 19 maternity hospitals in Ireland. The role of the General Practitioner acting in the Pastoral role of healthcare in the United Kingdom is a model that predates the National Health Service. Primary care has been using an EPR to collect rich sources of patient information on a local level (typically list sizes of 3000-6000 patients). Until recently, we have been unable to affect a change on our populations due to the fragmentation of healthcare provision and data sharing. From learning from the lessons of neighboring boroughs about installing a system before cultural and behavioral change, we have created a rich environment whereby 36 GP surgeries, two large acute hospitals, a community provider and aspects of social care, we have been able to create an environment in which to share data responsibly, to enable a roll out of Population Health Software (supported by our local Partners, the Royal Free London Hospital), to make a significant impact on health outcomes at scale, helping the care of nearly 260,000 patients in a diverse and exciting borough of Camden.The enablers for this include:The joining of 36 GP Surgeries under a single Federated entityA mature relationship between providers based on shared values and shared governance (clinical and information governance)A software platform that we can design registries in a bespoke way that meets the needs of our unique population.A mature relationship between our respective EPR providers to concentrate on making a mature digital platform that allows all systems to contribute towards population health.The presentation will cover:The divergent history of IT platforms in our areaThe enablers for providers to aim towards a common clinical strategyThe mistakes we have made along the wayHow sometimes the obvious choice for an ACO is the wrong choice

BackgroundBlood transfusion is a frequent clinical intervention: 1.8 million red cell units are transfused in England/annum. 'Wrong' transfusion is the most frequent preventable serious transfusion incident; in the UK between 1996 and 2013, they resulted in 28 deaths and 137 cases of major morbidity, usually due to patient mis-identification. Patient safety can be improved by blood avoidance whenever possible.The main idea was to use technology to ensure:1. Care is always delivered in accordance with best practice.2. Patient safety is improved.3. Staff working environment is enhanced.4. Significant economic benefit is derived.5. Real-time guidance is offered at the point of care.Innovation:1. The addition of rules-based blood ordering guidance Clinical Decision Support System (CDSS) centred on providing the most recent blood results within Millennium to clinicians when blood orders are being made and alerting them to inappropriate orders.2. Full utilisation of the existing electronic transfusion process incorporating barcode patient identification and handheld computers at the bedside to prompt staff through every step and verify the correct blood is transfused.3. Use of remote blood fridges, speeding the delivery of blood to patients.Objectives:1. Maintain standards of patient safety throughout the transfusion process, reducing its complexity to enable staff to ‘get it right, first time - every time’, and providing end-to-end electronic control.2. To continue to meet the requirements of the UK Blood Safety and Quality regulations (2005) for the accurate documentation of transfusions using an electronic end-to-end transfusion process.3. Provide doctors with timely information and support with decision-making regarding blood use, including alerts, to show when recommended treatment is not being followed.4. Improve the integration of the transfusion process in electronic record-keeping thereby allowing the clinician full oversight of transfusion activity and blood availability for individual patients.4. Produce a transferable solution for other hospitals, and other clinical processes in order that our experiences are shared and the safest processes are promoted.

Background:In 2014, 10 Emergency Departments & 1 Urgent Care Centre in South Western Ontario embarked on a journey to implement computerized provider order entry (CPOE), electronic medication administration record (eMAR), as well as electronic triage documentation. Following this major system conversion, breakdowns involving departmental culture, technology, and process became evident. It was at this point that a stabilization project involving all of these elements became necessary. Methods: Our Emergency Department stabilization project was approached using a traditional project management framework. Our methodology: Leadership engagement, physician collaboration, along with multi-disciplinary stakeholder participation. We will review how we addressed the people, process and technology issues that are unique in Canadian Emergency Departments. We will also outline and share our stabilization project set up, planning, execution and outcomes as a result of our collaborative approach at addressing the issues raised by physicians from large academic Emergency departments. Summary: This presentation would benefit other hospitals by highlighting the steps to improve cultural, technological and process issues and in turn greatly improve user adoption and satisfaction.We believe that true transformation will only happen if it is ‘’led’’ by our staff and the power of social movement, connecting with people’s core values and motivations and mobilising their own internal energies and drivers for change. Our e-Care programme is the largest change project that our hospital has ever undertaken impacting all our patients and the majority of our staff. At a macro-level, healthcare reform has intensified the pace and associated challenges of transforming our healthcare delivery system. Hospitals need to operate at the highest efficiencies possible while delivering quality patient care and containing costs. Future payment models and measurement framework will demand this transformation. Business Intelligence, advanced analytics, and health-services econometric principle and methodologies are being applied in healthcare to aid in improved patient safety and experience; improved quality of care delivery; eliminating duplicative utilization; and lowering health care costs. Data Governance framework are being deployed in parallel, to improve data quality, prioritize data-driven organizational imperatives, and enable a structure for problem solving. This presentation will highlight key data governance strategies to support improved regulatory, pay for performance, clinical efficiency, and process measures, while enabling a system-wide modernization design with particular focus on cost and quality drivers, including population-level health management, risk models, care coordination and chronic care management.

Faith Regional Health Services started Soarian CPOE in 2010. The philosophy used was to create order sets that minimized the physician’s ‘clicks’ to ease order entry while avoiding ‘cook-book medicine’. While working towards this end we have experimented with different ways to capture nursing and medication protocols in a manner that streamlined the number of forms clinicians have to open to obtain needed information.

When we first created the nursing protocols the existing PDF documents were included in the order sets as Links. Very early in the deployment, pharmacy realized that nurses found the opening of these links too slow and cumbersome for simple information and developed a way to ‘clear-down’ an ODF form to use as a simple Protocol Document. As nursing got used to seeing medication protocols in an ODF they requested a way to decrease the amount of single-sentences and the number of PDF links in Nursing Orders which lead to the creation of nursing ODFs.

In working with physicians we found there was a need to provide them with reminders and definitions such as the criteria for healthcare associated pneumonia or the 2-midnight rule criteria. We use ‘headers’ within order sets for short definitions/guidance but have also used a combination of PDF links and ODFs to provide more in-depth information. Faith does not have PDOC (Physician Documentation) in place but needed a mechanism for physicians to perform ‘assessments’ such as VTE risk. Faith has found it useful to use an ODF within the order set to accomplish this documentation.

This presentation will walk the listeners through the evolution of Faith’s order sets and how we’ve chosen to provide clinicians with clinical and procedural information as well as gather documentation ‘up-front’ during the order entry process.

This project has taken Nash from 75 smart phones to 250 smart phones in 18 months. In 2015, Nash has gone beyond communication and secondary notification to include clinical integration and barcode scanning medication administration on a single smart phone. Carolinas Healthcare System has begun a two year journey of Clinical Optimization within their health system. They have use internal projects, a Cerner Clinical Assessment, and many of the lean initiatives that their organization has identified as the key identifiers for this work. With CHS's long standing use of the Cerner clinical platform in multiple facilities this dialogue will take the Cerner client community through the many obstacles and lessons learned in taking on such a large project.

Barnabas Health consists of 8 facilities, 7 acute care hospitals and 1 psychiatric facility. The goal of their project was to reduce visible documentation (visible sections and visible DTAs upon opening a section) by at least 35% and eliminate duplication within a 5 month timeline; the rollout of the enhancements would occur in an organized approach over a 3 month timeline. The pressures in health and social care are similar the world over. Whatever the GDP spend on health, countries cannot afford to spend more. In the pursuit of the triple aim of health care, other countries are also exploring new models of care where risk is transferred to integrated care networks to manage the health of their populations. For example, Spain has had the concept of ACOs for some time, England is trialing new care models with vanguard care networks in support of its new five year forward view strategy. However, care is organized differently in different countries and these differences will be reflected in the approaches countries will take. Public funded health systems are often fully accountable for entire geographies; including, unmet need and not just patients that are attributed to the network.
