Zika virus: following the path of dengue and chikungunya?

On May 7, 2015, the Pan AmericanHealth Organization issued an alertabout potential Zika virus (ZIKV)transmission in northeast Brazil.1 Thishas now been confirmed with widespread of the disease, underscoringthe potential for ZIKV to spread

globally, similar to dengue (DENV)and chikungunya (CHIKV) viruses.

ZIKV is an emerging arthropod-borne virus (arbovirus) that was firstisolated from a Rhesus monkey inUganda, in 1947. This arbovirus isrelated to DENV and they have similarepidemiology and transmission cyclein urban environments. Until recently,only sporadic human ZIKV infectionswere reported. In 2007, ZIKV emergedoutside of Asia and Africa for the first

time and caused an epidemic onYap Island in the Federated States ofMicronesia,2 which was followed bya large epidemic in French Polynesiain 2013–14.3 Subsequently, ZIKV spread to several countries in Oceania(figure).4

The clinical presentation of ZIKVinfection is not specific (mild fever,rash, arthralgia, and conjunctivitis)and can be confused with otherdiseases, especially dengue and

ジカウイルス感染症の症状

感染しても約8割の人は症状が出ません。2割の人は蚊に刺されてから2～7日後くらいに発症します。

ジカウイルス感染症の検査所見

✤ 血液検査では特徴的な所見はない

✤ 血液や尿の遺伝子検査、または血液の抗体検査で診断する

✤ 発症から数日で血液からジカウイルスは消えるが、尿からは2週間程度までジカウイルスが見つかる

ジカ熱の治療と予防

✤ 現在のところジカ熱に対する特異的な治療はない

✤ それぞれの症状に対して対症療法を行う

✤ 有効なワクチンも今のところない

✤ 蚊に刺されないための対策が重要

ジカウイルス感染症と小頭症

✤ 妊婦さんが感染するとお腹の中の赤ちゃんが小頭症になることがあると言われています。

✤ ある研究によると、通常の分娩に比べてジカウイルス感染症に感染した妊婦さんから小頭症の赤ちゃんが生まれる可能性は約50倍と言われています

どんなときに ジカウイルス感染症を疑う？

✤ ジカウイルス感染症が流行している地域から帰国後1週間以内に目の充血、微熱、全身の皮疹などの症状があるとき

✤ 頭痛や全身の節々の痛みを伴うことも

✤ 中南米だけでなく、東南アジアでもジカウイルス感染症は流行している点には注意が必要

日本におけるマラリア

✤ 年間60～80例前後（全て海外感染例）。

✤ 現在でも年間1~2人程度死亡例がみられる。診断・治療の遅れが主な原因と考えられる。

● 約10日間～30日間。まれに、半年以上の場合もあります。● 旅行が1週間以内であれば、現地で発病することはありません。

マラリアの臨床症状

✤ 潜伏期間は12～35日

✤ 発熱以外の症状は多彩

✤ 頭痛、関節痛、黄疸、吐気、下痢など

✤ 症状が発熱と下痢だけのこともあり、旅行者下痢症と判断され診断が遅れ重症化することがある。重症マラリアの死亡率は15-20%
