ขอมลดานความปลอดภย พบวาผ ปวยสามารถทนตอยา olopatadine และยา ketotifen ไดด (3-5)แตไมพบขอมลรายงานประสทธภาพและความปลอดภยของ antrazoline 005 + tetrahydrozoline 004(1)

1 ขอมลโดยทวไป อาการเยอบตาอกเสบจากการแพ (Allergic conjunctivitis)

เยอบตาอกเสบจากการแพเปนอาการทมกเกดในคนทอายนอย อายเฉลยของ onset ประมาณ 20 ป อาการดขนเมออายมากขน ซงกวารอยละ 50 ผ ปวยมกจะมประวตสวนตว หรอประวตครอบครวเปน ภาวะแพอยางอน เชน allergic rhinitis atopic dermatitis หรอ asthma อาการ และอาการแสดง

คน น าตาไหล เยอบตาขาวบวมแดง แสบตา กลวแสง เปลอกตาบวม อาการมกจะเกดกบตาทง 2 ขาง แตบางครงตาขางใดขางหนง อาจจะมอาการมากกวาอกขาง เยอบตาอกเสบจากการแพ แบงเปน 3 ชนด

1) Acute allergic conjunctivitisเกดอยางเฉยบพลน ซงอาจมสาเหตมาจาก การสมผสกบสารหรอ สงตางตางทกอใหเกดอาการ อาการทเกดขนเชน itching hyperemia tearing chemosis eyelid edema โดยทวไปอาการมกจะดขนภายใน 24 ชวโมงหลงจากหลกเลยงการสมผส allergen แตในบางรายอาจพบอาการรนแรงได

2) Seasonal allergic conjunctivitis (SAC) หรออาจเรยกวา allergic conjunctivitis hay-fever type conjunctivitis หรอ rhinoconjunctivitis ซงอาการทเกดขนเปนอาการแพทมอาการทางตาทไมรนแรง โดยทวไปมกสมพนธกบ การทผ ปวยมภาวะ rhinitis อยแลว อาการทเกดขนไมไดเปนอยางเฉยบพลน สามารถท านายได และมกสมพนธกบฤดกาล

3) Perennial allergic conjunctivitis (PAC) เปนอาการแพทไมรนแรง แตเรอรง มกสมพนธกบสงแวดลอมทผ ปวยสมผสตลอดทงป สวนมากจะเปน allergen ทอยภายในบาน หรอตก (indoor) เชน ไรฝ น ขนสตว รา เปนตน

INTRODUCTION mdash There are five main types of ocular allergy seasonal allergic conjunctivitis perennial allergic conjunctivitis vernal keratoconjunctivitis (VKC) atopic keratoconjunctivitis (AKC) and giant papillary conjunctivitis (GPC) VKC and AKC are chronic bilateral and severe forms of allergic inflammation affecting the ocular surface These two relatively uncommon types of allergic eye disease can cause severe damage to the ocular surface leading to corneal scarring and vision loss if not treated properly (this occurs more commonly with AKC than VKC) Type I hypersensitivity reactions are important in these diseases although they are not the only pathophysiologic mechanism VKC is reviewed in this topic AKC is discussed separately

GPC is an inflammatory disorder that represents a reaction to lid movement over a foreign substance such as contact lenses Toxic conjunctivitis is not allergic in nature but it is frequently confused with allergic ocular disease It develops with protracted use of topical medications mostly due to preservatives GPC is discussed in detail separately Seasonal and perennial allergic conjunctivitis the most common forms of ocular allergy are also discussed separately (See Allergic conjunctivitis) EPIDEMIOLOGY mdash VKC most commonly occurs in boys living in warm dry subtropical climates such as the Mediterranean the Middle East central and west Africa South America and Asian countries such as Japan Thailand and India [1] The limbal form of VKC is seen most often in dark-skinned individuals from Africa and India VKC is generally rare in cooler climates such as Northern Europe and the temperate areas of North America However the prevalence in these regions has increased probably due to immigration of individuals from susceptible populations [2] Exacerbations are common in the spring (hence the name vernal) although reactivation often occurs in the winter Initially the exacerbations are seasonal but perennial attacks increase after a few years Males are more commonly affected than females In one series the male to female ratio was 321 in patients lt20 years of age but was nearly equal in older patients [3] Age at onset is generally before 10 years with the earliest reported onset at five months of age [4] although VKC can infrequently occur in adults Patients usually outgrow the disease with the onset of puberty VKC is associated with other atopic manifestations in approximately one-half of patients [35] The most common concomitant atopic diseases are asthma and allergic rhinitis Aeroallergen sensitization by skin prick testing or allergen-specific immunoassay was reported in over 50 percent of patients in one study [3] A family history of atopy was also reported in about one-half of patients in this same study In another study atopy was more common in patients with the palpebral or tarsal form of the disease compared with the limbal form

- การศกษาแบบ prospective randomized double-blinded และ placebo-controlled ในผ ปวย SAC อาย 10 - 55 ป แบงเปน 5 กลมคอ olopadine HCI 01 ketotifen fumarate 0025 epinastine HCI 005 emedastine 005 และ fluorometholone acetate 01 โดยทกกลมหยอดตาขางหนงดวยยาทใชศกษา 1 หยด วนละ 2 ครง เปนเวลา 2 สปดาห และหยอดตาอกขางหนงดวยยาหลอก พบวา ยาทกตวยกเวน fluorometholone สามารถลดอาการคน ตาแดง น าตาไหล ระคายเคอง และเปลอกตาบวมไดมากกวายาหลอก อยางมนยส าคญ (Plt0001)(6)

-การศกษาแบบ randomized double-masked ในผ ปวย SAC อายมากกวา 18 ndash 61 ป แบงเปน 3 กลม คอกลมทไดยา Olopatadine 01 eye drop Ketotifen 0025 eye drop และน าตาเทยม โดยหยอดยา 2 หยด วนละ 2 ครง เปนเวลา 30 วน พบวากลมทไดรบยา ketotifen และ olopatadine สามารถลดอาการคนตาและน าตาไหลไดมากกวากลมน าตาเทยม แตผลลดอาการตาแดง เปลอกตาบวม ระคายเคองตาแตกตางกนอยางไมมนยส าคญ(4)

- การศกษาแบบ double-masked ในผ ปวย seasonal หรอ perennial allergic conjunctivitis แบงเปน 2 กลม คอกลมทไดรบ Olopatadine HCl 01 และ Ketotifen fumarate 0025 เปนเวลา 4 สปดาห พบวาผ ปวยสวนใหญ (81) ชอบใชยา olopatadine ซงผ ปวยรสกสะดวกสบายและมประสทธผลดในการลดอาการแพมากกวายา ketotifen (Plt00001)(5)

These agents have two main actions As mast cell stabilizers they inhibit mast cell degranulation which is the first step is the allergic cascade They

also inhibit leukocyte activity and dampen mediator release from mast cells basophils eosinophils and neutrophils

As antihistamines they competitively and reversibly block histamine receptors in the conjunctiva and eyelids thus blocking the actions of the primary mast cell-derived mediator [28] This also helps reduce the late phase of the allergic response

Dual acting agents include olopatadine azelastine HCl epinastine pemirolast potassium and ketotifen fumarate These drugs have been studied extensively in seasonal and perennial allergic conjunctivitis [29-31] However there are only a few nonrandomized studies on use of these drugs specifically olopatadine and ketotifen in patients with VKC [2732]

In one observational study topical olopatadine hydrochloride 01 percent significantly decreased most signs and symptoms of VKC including itching tearing burning mucus discharge conjunctival hyperemia and corneal involvement after two months of therapy [27] However photophobia and limbal papillae did not improve significantly In another observational study patients treated with ketotifen had greater improvement in itching tearing conjunctival hyperemia mucus discharge and photophobia after three weeks of therapy than patients treated with olopatadine [32]

Ketotifen fumarate is available in a generic formulation and is over-the-counter in the United States Common side effects of these medications include stinging upon instillation and headache The dual acting agents alone may be sufficient to treat mild acute symptoms However addition of a topical corticosteroid is generally required to gain control in patients with moderate to severe acute symptoms (See Allergic conjunctivitis section on Stepwise approach to therapy)

Topical mast cell stabilizers include cromolyn sodium [33] nedocromil sodium [3435] and lodoxamide [3637] Mast cell stabilizers have repeatedly been shown to be effective in patients with VKC [33-37] In particular efficacy of topical cromolyn was demonstrated in several small randomized trials [38] In one of these trials 4 percent cromolyn sodium demonstrated significant improvement in conjunctival and limbal injection limbal edema tearing and symptoms summary score compared with placebo [33] In this study cromolyn was more effective in patients with evidence of IgE-mediated disease Subsequent studies demonstrated that lodoxamide was more efficacious than cromolyn sodium [3637] The onset of action of these drugs is 5 to 14 days after initiation of therapy Thus these drugs are NOT useful for acute symptoms In addition dosing of mast cell stabilizers is four times daily compared to twice daily for most agents with combined actions These features may limit patient compliance

ผลการศกษารวมจาก 27 RTCs พบวา ldquocurrently available topical therapies (mast cell stabilisers immunomodulators NSAIDs and antimitotic) are significantly more effective than placebo for treating most signs and symptoms of vernal keratoconjunctivitis However there is a lack of evidence supporting the recommendation of one specific type of medication for treating this disorderrdquo (11) Result Systematic review of the initial 27 studies indicated a positive effect of common antiallergic eye-drops in reducing signs and symptoms of the disease All the drugs tested in these 27 studies were found to be safe well tolerated (except only transient mild burning and tearing upon instillation of ciclosporin eye-drops) and more effective than placebo A greater number of studies (20) evaluated the efficacy of common antiallergic eye-drops (levocabastine lodoxamide mipragoside NAAGA nedocromil sodium sodiumdisodium cromoglycate) Among these lodoxamide appeared to be the

most effective Compared with antiallergic drugs however corticosteroids (one study) were more effective while immune-modulators (1 study) did not show a statistically significant difference in reduction of signs and symptoms of the disease

To demonstrate whether the efficacy of these drugs in reducing signs and symptoms of active disease was significant a meta-analysis was conducted on the 10 trials that presented comparable data suitable for statistical analysis The combined results of these 10 RCTs included in the meta-analysis clearly demonstrated a positive effect favouring improvement of signs and symptoms in active VKC after topical therapy independent of the pharmaceutical class for the following clinical variables total signs and symptoms itching tearing corneal involvement limbal disease and hyperaemia A positive effect in reducing tarsal papillae was also demonstrated but it was only slightly significant (p=047) There was a trend towards improvement also in photophobia but it was not statistically significant (p=105) Comparison among different pharmaceutical classes and drugs was not possible due to the small number of available RCTs on the subject and the great variability in assessment of outcome measures to evaluate the effects of topical treatment for VKC

ของ topical treatments ในการรกษาseasonal allergic conjunctivitis โดยเปรยบเทยบระหวาง topical mast cell stabilizers (sodium cromoglycate nedocromil lodoxamide) กบ placebo topical antihistamines กบ placebo และ topical mast cell stabiliszers กบ topical antihistamines

ผลการศกษารวมจาก 40 RCTs (29 trials compared topical mast cell stabilizers versus placebo 9 trials compared topical antihistamines versus placebo และ 8 trials compared topical mast cell stabilizers versus topical antihistamines) พบวา topical mast cell stabilizers และ antihistamines มประโยชนและประสทธภาพ เหนอกวา placebo ในการรกษา allergic conjunctivitis แตอยางไรกตาม ยงไมมหลกฐานสนบสนนเพยงพอในการก าหนดแนวทางการรกษา allergic conjunctivitis ทจะก าหนดใหใชเฉพาะยาเพยงกลมใดกลมหนงเทานน นอกจากนนยงไมมรายงานเกยวกบอาการขางเคยงทส าคญของยาใดๆ ดงนนการเลอกใชยาจงควรขนอยกบความสะดวก ลกษณะอาการของผปวย และราคาของยา(2) (Treatment preferences should therefore be based on convenience of use (with reduced frequency of instillation for some preparations) patient preference and costs especially as important side effects were not reported with any medication) Results A meta-analysis of six trials showed that patients using sodium cromoglycate were 17 times (95 confidence interval [CI] = 4 to 78) more likely to perceive benefit compared with those using a placebo although this estimate may be partially influenced by publication bias Five trials indicated that those patients using nedocromil were 18 times (95 CI = 13 to 26) more likely to perceive their allergy to be moderately or totally controlled than those using a placebo Four trials showed that those using antihistamines were 13 times (95 CI = 08 to 22) more likely to perceive a lsquogoodrsquo treatment effect than those using mast cell stabilisers although this beneficial effect was not statistically significant Limited evidence suggests that antihistamines might have a faster therapeutic effect compared to mast cell stabilisers

ผลการศกษารวมจาก 40 RCTs (29 trials compared topical mast cell stabilizers versus placebo 9 trials compared topical antihistamines versus placebo และ 8 trials compared topical mast cell stabilizers versus topical antihistamines) พบวา topical mast cell stabilizers และ antihistamines มประโยชนและประสทธภาพ เหนอกวา placebo ในการรกษา allergic conjunctivitis แตอยางไรกตาม ยงไมมหลกฐานสนบสนนเพยงพอในการก าหนดแนวทางการรกษา allergic conjunctivitis ทจะก าหนดใหใชเฉพาะยาเพยงกลมใดกลมหนงเทานน นอกจากนนยงไมมรายงานเกยวกบอาการขางเคยงทส าคญของยาใดๆ ดงนนการเลอกใชยาจงควรขนอยกบความสะดวก ลกษณะอาการของผปวย และราคาของยา(2) (Treatment preferences should therefore be based on convenience of use (with reduced frequency of instillation for some preparations) patient preference and costs especially as important side effects were not reported with any medication) Results A meta-analysis of six trials showed that patients using sodium cromoglycate were 17 times (95 confidence interval [CI] = 4 to 78) more likely to perceive benefit compared with those using a placebo although this estimate may be partially influenced by publication bias Five trials indicated that those patients using nedocromil were 18 times (95 CI = 13 to 26) more likely to perceive their allergy to be moderately or totally controlled than those using a placebo Four trials showed that those using antihistamines were 13 times (95 CI = 08 to 22) more likely to perceive a lsquogoodrsquo treatment effect than those using mast cell stabilisers although this beneficial effect was not statistically significant Limited evidence suggests that antihistamines might have a faster therapeutic effect compared to mast cell stabilisers
