1.1 We refer to your instructions received on 13 th May 2011 to investigate into the
above accident.
1.2 After having completed our enquiries concerning the subject motor accident, we
submit our report as follows: -

Remarks
3.1 We wish to advise Insurer that our numerous attempts to secure an appointment
with the insured driver have been futile. He had informed us to refer to his police
report for the circumstances of the accident and was reluctant to meet our
Adjuster over this matter.
3.2 We have contacted the Insured for an arrangement to meet the Insured driver.
And we managed to speak to the Insured. Having explained the need for us to
meet the driver, he then forwarded us the Insured drivers phone number (010-
8838059).
3.3 We then contacted the Insured driver and having explained the need for us to
meet him. In the meantime, we also informed him that we are able to go to his
place to meet him to record a statement but he disclosed to us that he was busy
at work.
3.4 Finally he agreed to meet us and he fixed an appointment to meet us at Taman
Pelangi on 18th December 2011 which was postponed to 27th December 2011 as
he disclosed to us that he was not free.
3.5 At the material date and time, we went to Taman Pelangi, however he did not
turn up. When we contacted him, he once again postponed our appointment afterChinese New Year.
3.6 Then, we have called the Insured and the Insured driver on numerous occasions
but to no avail.

3.7 As such, we could not meet both the Insured and the Insured driver to record
statement on this matter.
4.0 Time, day and date of accident
4.1 The accident occurred at about 6.30pm, Wednesday, 26th December 2010.
5.0 Description of scene
5.1 The accident occurred at Road 1, BKE, Singapore.
5.2 Road 1 BKE measures about 12 metres in width with a road divider and metal
guard railing at the centre. It has four (4) lanes in each direction and these lanes
are separated by intermittent white lines.
5.3 The road is straight, level and its surface is noted to be in good condition. There
are no obstacles to obstruct the view of motorist.
5.4 Flanking both sides of the road are grass verge followed by foliages.
5.5 Traffic flow at this location varies between moderate to heavy, depending on
time of day. There are no speed limit signboards erected along the road.
6.0 Circumstances of accident (based on Insured drivers police report)
Our interview with the Insured driver revealed the circumstances of loss as follows:-
6.1 The Insured driver was driving the Insured motorvan bearing registration
number JCG9802 along Road 1 BKE, Singapore.
6.2 At the material time, he was travelling on the middle lane.
6.3 Here, after ensuring no vehicles approaching from his rear, he switched into theright lane.
6.4 At this juncture, the third party motorcycle which was travelling from his rear
collided into the rear portion of his motorvan.

6.5 The third party rider was conveyed to hospital by an ambulance.
7.0 Third party riders version
7.1 The third party rider succumbed to his injury
8.0 Police investigation
8.1 The Insured driver lodged a police report at the Traffic Police Division HQ, 10
Ubi Avenue 3, Singapore on the same day of the accident vide report number
T/20110401/9003.
8.2 We have applied for the relevant documents which will forwarded to yourselves
as event permits.
9.0 Independent witness / Enquiries at the scene
9.1 We were not able to make any enquiries at the accident scene or locate any
independent witnesses who could provide additional information in respect of
the accident.
10.0 Particulars of the injured third party pedestrian
10.1 We advised Insurer that till to date we are unable to meet the third party riders
next of kin since they are living in Singapore.
11.0 Our Finding
11.1 Having probed into the circumstances relating the accident and our findings with
the authorities, we wish to advise that we are unable to trace any element of a
fraudulent claim and we are satisfied that the accident is a genuine incident and
no foul play suspected.
11.2 Our investigation reveals that on the material day and time the third party motorcycle had
collided into the rear portion of Insured motorvan which was in the process of swithing the
lane.

11.3 We are still waiting for the relevant police documents and their outcome. We
will forward to yourselves as event permits.
12.0 Conclusion
12.1 Based on our investigations, the Insured driver of a motorvan bearing
registration number JCG9802 is more to be blamed for switching the lane thus
blocking the rightful path of third party rider.
12.2 Nonetheless, we leave this aspect to be advised by your solicitors on record.
13.0 Other remarks
13.1 During the course of our investigation, we noted that the Insured driver and thirdparty pedestrian involved were not known to each other.
We now submit the foregoing for your consideration and to conclude our involvement in this
matter we attached a note of fee and expenses which we trust insurer find in order.
