1.0 Project Background

Andhra Pradesh (AP) state has one of the longest coastal belts in India. Out of 67,362 Km of road network in the state, the

nine coastal districts cover 26,000 km of road length. Every year during the period of April-May and September-December,cyclonic situations set in to the Bay of Bengal. With the recent changes in climatic conditions, the catastrophic events are

increasing resulting in very devastating cyclones like Laila, Jal etc where the loss is enormous. In the 97 years ranging

from 1892 to 1989, there were as many as 67 cyclones within the State.

It is essential to provide safe residential accommodation to the vulnerable population during any eventuality or disaster due

to natural calamities such as cyclones and associated tidal waves and storm surges. The project is formulated based on

experiences of Government of Andhra Pradesh in mitigating such disasters during the past and also keeping in view the

existing facilities available in the project area in Andhra Pradesh.

The Government of India has applied for a credit of US$ 255 Million equivalent from the International Development

Association (IDA) toward the cost of National Cyclone Risk Mitigation Project (NCRMP), and it intends to apply theproceeds to payments for goods, works, related services and consulting services under this Project.

National Cyclone Risk Mitigation Project (NCRMP) Project Implementation Unit (PIU), Andhra Pradesh, headquartered at

Hyderabad has invited the consultants to submit proposals for providing the consultancy services for Third Party Quality

Audit for 9 coastal districts namely Srikakulam, Vijayanagaram, Visakhapatnam, East Godavari, West Godavari, Krishna,

Guntur, Prakasam and Nellore. The project Construction of Multipurpose Cyclone Shelters, Roads to Cyclone

Shelters, and Roads to habitations and Roads & Bridges under NCRMP is funded by International Development

Association and received through Government of India. This consultancy is envisaged to audit the quality of works being

implemented by various line departments working under PIU. The number of works under progress / for which

agreements are concluded in each district is shown in the table below.

Disaster Management & Project Director, Project Implementation Unit (PIU) NCRMP has selected Egis International in JV

with Egis India Consulting Engineers (I) Pvt. Ltd. On 31st

May2011, the Contract for Consultancy Service was signed

between The Commissioner for Disaster Management & Project Director, Project Implementation Unit (PIU) NCRMP and

the Egis International in JV with Egis India Consulting Engineers (I) Pvt. Ltd.

The district wise progress of works is shown below. The works in the West Godavari are not started and the works in

Krishna District has just started. At many places the first milestone is completed but works are not started. The line

departments are to analyze the reasons for the slow progress and if necessary penalty for the slow progress as per theagreement conditions should be imposed.

5.0 District wise status of tests conducted by TPQA:

Apart from monitoring the tests being conducted by the implementation team (contractors & Line department) at

various stages, the Third party Quality Audit team also conducted tests to confirm its compliance with the standards.

7.0 Observations in the submitted reports:

PROVIDING BT SURFACE TO THE ROAD FROM R &B ROAD TO AVASOMAVARAMIN ACHUTAPURAM MANDAL

1. Two works are there in this package:1) PROVIDING BT SURFACE TO THE ROAD FROM R &B ROAD TO AVASOMAVARAM INACHUTAPURAM MANDAL and2)PROVIDING BT SURFACE TO THE ROAD FROM R &B ROAD TO AVARAJAM INACHUTAPURAM MANDALThe individual progress of this work is 91% value of work is completed. Shoulder works are inprogress.The combined progress of this package is 91.61%.

2. Works to be done as per Common Milestone for Two works in this Package:Mile stone 1 to the end of Dt: 26.07.11- Earthwork, CD Works and GSB.Mile stone 2 to the end of Dt: 26.10.11- WBM Gr-II, Gr-III and Shoulders.Mile stone 3 to the end of Dt: 26.01.12- BT and Road furniture.

3. The formation widths and Earthen shoulder widths are verified from Km 0/0 to Km 0/775 and at somelocations shoulders have slipped and the same shall be rectified.

4.It is observed that the shoulder material of sand and gravel is not properly mixed. It shall be scarified,suitably remixed, watered to OMC and compacted to achieve the required relative compaction ofmin.98% of lab MDD as per the agreement clause.

5. At some locations the formation width is observed to be less than the estimated width. The fieldofficers are requested to furnish the reasons for the same.

6. The measurements like width and thickness of the WBM Gr.-3 Layer from Km 0/0 to Km 0/775 isverified and found tallying with the required measurements. The details are given in Annexure - 2.

7. Volumetric analysis tests were conducted for WBM layer from Km 0/0 to Km 0/775. It is fairly tallyingTo specfication of clause 405.13 of MORD.

8. Gradation is conducted for OGPC and Seal coat CA material and results are found to be within thespecified limits as per agreement specification.

9. During execution, The rate of spraying of prime coat is verified and found to be within the specifiedlimits as per Table 502.1 of MORD. The spraying temperatures are also observed during executionand found to be varying from 320C to 350 C, which is within the specified limits as per MORD.

10. During execution, the rate of spreading of tack coat is verified and found to be within the specified

limits as per Clause 503 and Table 503.1 of MORD. The spraying temperatures are also observedduring execution and found to be varying from 38C to 42C, which are within the specified limits asper MORD specifications.

11. During execution, Temparature at mixing, laying and at compaction are verified during execution andfound to be within the specified temperature range.

12. The obtained values of Bitumen Extraction Test Results for OGPC and Seal Coat in % as per IRC:SP: 20 and IRC: SP: 11 from Km 0/0 to Km 0/775 are verified and found tallying with therequired.The details are given in Annexure - 4.

13. White wash shall be done to parapet walls of the existing culverts for the clear visibility of vechiculartraffic.

14. Village name boards, cautionary and mandatory signboards at necessary points all along the outeredge of sharp curves and at school locations shall be erected.

15. Some of the KM and HM Stones are missing in this working road stretch.The field officers arerequested to erect them immediately.

8. During execution the rate of spraying of prime coat is verified and found to be within the specifiedlimits as per Table 502.1 of MORD.And the spraying temperature is observed as 300C to 360 C andwithin the specified limits as MORD

9. During execution the rate of spreading of tack coat is verified and found to be within the specifiedlimits as per Clause 503 and Table 503.1 of MORD.And the spraying temperature is observed as400C to 440 C and within the specified limits as MORD

10. During execution, Temperature at mixing, laying and at compaction are verified and found within thespecified temperature limits.

11. Bitumen Extraction Test conducted for OGPC and Seal Coat as per IRC: SP: 20 and IRC: SP: 11from Km 0/0 to Km 1/112 and the results are found tallying with the required. The details are given inAnnexure - 4.

12. The measurements like width and length of parapets and pipes of all the culverts are verified andfound tallying with the estimate.

13. Some of the KM and HM Stones are missing in this working road stretch. The field officers arerequested to make arrangement to rectify them immediately.

It is to be specified in the agrrementspecification. As it was not specified in the aggrement of this work, permission from the competentauthority is necessary.

7. The measurements like width and thickness of the WBM Grade-II and WBM Grade-III layers from Km0/0 to Km 0/860 and Km 1/163 to Km 1/492 are verified and found to be tallying with the requiredmeasurements. The details are given in Annexure - 2.

13. The obtained values of Bitumen Extraction Test Results for OGPC and Seal Coat as per IRC: SP: 20and IRC: SP: 11 from Km 0/0 to Km 0/860 and Km 1/163 to Km 1/492 are verified and found tallyingwith the required.The details are given in Annexure - 4.

14. White wash shall be done to parapet walls of the existing culverts for the clear visibility of vechiculartraffic.

5. Two sets of cubes tested for compressive strength of 7 days and 28 days seperately at laboratary. Theaverage value of compressive strength is observed for 7 days as 22.736 N/mm2 and for 28 days as30.66 & 31.99 N/mm2.

6. The thickness of the CC Road is observed as 202 to 213mm.7. The Quality Control Lab is not provided at site for testing of material. The line department officers are

requested to take necessary action to establish Quality Lab at site at early stage.8. Cores will be taken to verify depth and compressive strength after 28 days of lying.9. MORD Specifications and IRC: SP-62-2004, Guidelines for the construction and design of Cement

Concrete roads for low volume roads, need to be followed in cutting and spacing of joints and curingthe CC road as mentioned below.

1. Construction of joints shall be mechanically sawn joints spaced 2.5 m to 3.75 m2. Length of the panel in the direction of traffic shall not be less than the width of the pavement.3. Joints shall be cut when the pavement is neither too soft nor too hard and able to bear the

weight of the machine and crew. Normly it may vary from 8 to 12 hrs depending on weathercondtions

10. Construction joints shall be butt joints and placed after a days work is over or when work is suspendedfor more then 30 minutes

11. Expansion joints shall be provided near bridges and slab culverts and consists of a bitumenimpregranted joints/ premoulded synthetic joints filler board about 20 mm thick.

12. 1. After completion of the finishing operations, the surface of the pavement shall be coverd with wetHessain cloth, burlap or jute mats. They shall be maintained fully wetted and in position for 24 hrs2. Upon the removal of the wet covering out the end of 24 hrs, the slab shall be thoroughly wetted andcured by ponding or sprinklers.

13. After minimum 28 dys after casting the concrete pavement ,joints groove at contraction joint shall bewidened to a 10 mm width and to a depth 18 to 20mm and shall be sealed

2. There is a narrow slab culvert at the approach to the cyclone shelter road. At present this is the onlyapproach to the site. For movement of bigger vehicles, the approach needs to be widened dulyconstructing wing walls on four sides.

3. Another approach can be provided by constructing a pipe culvert on the southeren side of the sheltersite. Proper approaches are necessary during cyclone or other natural calamities.

4. It is noticed that ground water table is high and required to be dewatered by suaitable beams forexcavation and construction of foundation.

3. Grade -III WBM completed from Km 0/0 to Km 0/475.

4. During inspection by Site engineers, oversized metal was observed in grade III metal collected and itwas instructed to remove or rebreak the oversizes metal. During inspection on 12-11-2011, it wasnoticed tha the oversized material was removed and correct sized metal was collected.

5. The thickness of WBM Grade-III layer was verified at random location thicknesses are as perrequirement. Camber and longitudinal undulations were also measured and tabulated in annexure-I.6. The volumetric test conducted for Grade-III metal layer and it satisfied the requirements.7. Shoulders foramtion is in progress. Shoulder formation has to be done simultaneously with the WBM

Gr III layer. Hence it is instructed to construct the shoulders8. The Shoulder width's are found to be less than the width specified as per drawing at some locations in

6. The reinforcement mat for slab and beams are verified and found to be fairly tallying with drawingdetails.

7. The physical measurements for elastomeric neoprene bearings are verified and found OK.8. The field officers have furnished the test results for Water, steel and cement obtained from Department

of Civil Engineering, Andhra University, and Visakhapatnam.9. The field officers are also requested to apply cement slurry to the steel bars so as to protect them from

rusting.10. The field officers are also requested to test the samples of cement from every consignment, conduct

all the required tests on cement such as cube strength, initial setting time, Final setting time,consistency, soundness etc.

11. The field officers are requested to provide the invoices bills of the materials for verification.12. The reinforcement steel shall be neatly stored on wooden blocks, racks or platforms above the ground

in clean and suitably marked to facilitate inspection and identification as per Cl.1603 of MORTH.

1. 18.82 % value of the work is completed. WBM Grade- II is completed.2. Works to be done as per Milestone:

Mile stone 1 to the end of Dt: 14.12.11- Earthwork, CD Works and GSB.Mile stone 2 to the end of Dt: 14.03.12- WBM Gr-II, Gr-III and Shoulders.Mile stone 3 to the end of Dt: 14.06.12- BT and Road funiture.

3. The earth work done in the widening portion is not compacted properly, it is to be constructed inlayers, sectioned, watered to OMC and compacted to achieve the required relative compaction ofmin.98% of lab MDD as per the agreement clause. The field officers were instructed to rectify thesame.

4. Site registers are not available. The field officers are requested to maintain the material registers andthe same shall be furnished to the TPQA for verification.

5. The formed width is less than the approved drawing width of 7.5m. The formed width is varying from

6.0 to 7.0m. It is observed Hand pump and electrical poles are inside the road width on both sides fromCH: 0/0 to 0/390m. The field officers were asked to take necessary actions to shift them.6. The formation widths and earthen shoulder widths are verified from Km 0/0 to Km 0/390 and at some

locations shoulders have slipped and the same shall be rectified.7. The measurements like width and thickness of the WBM Gr.-2 Layer from Km 0/0 to Km 0/390 is

verified and found to be fairly tallying with the required measurements. The details are given inAnnexure-2.

8. Volumetric analysis tests conducted for WBM layer from Km 0/0 to Km 0/390. It is fairly tallying tospecfication of clause 405.13 of MORD.

9. Quality Control lab is not available at site. The Executive Engineer is requested to give necessaryinstructions to the contractor and see that the quality lab is established at site.


11. The Project informatory boards with all details were not provided as per aggrement. The same may beprovided immediately at site as per the guide lines by PIU, NCRMP.
