O2 delivered near an artificial opening in the neck (commonly called blow by O2). Tracheostomy collar provides a means for both oxygenation and humidification. The moisture tends to saturate the gauze dressing around the trach & requires frequent changing.

• Before suctioning a conscious patient, you should ask her to cough and deep breathe several times
• Many facilities require supplemental oxygen before suctioning.
• Measure the plastic suct ion catheter from the corner of the patient’s mouth to the earlobe on the same side
• Do not insert the catheter more than this distance
• Insert the catheter gently

• For adults use a size 12-14 French Catheter. Smaller catheters are associated with fewer complications. !
• Suction intermittently no more than 10 seconds at a time, including the time it takes to withdraw the catheter !
• Move the catheter in a circular motion or from side to side during suctioning
• Use a flexible catheter for only one cycle of suctions before discarding !
• Perform only as necessary to remove secretions !

Tracheostomy Care
TRACHEOSTOMY TUBES All tracheostomy tubes consist of a main shaft and a neckplate or flange. The flange rests between the clavicles. Holes on each side allow you to attach tracheostomy ties to secure the tube in place. Tubes are made of semi-flexible plastic, rigid plastic or metal. Semi-flexible plastic tubes conform to the patients’ anatomy during movement – good for patients with tracheal deviations. Rigid tubes are typically used when neck swelling is a problem. The tubes don’t bend or collapse with local oedema. Plastic tubes have a built-in 15mm adaptor that extends from the neck plate. This allows respiratory equipment e.g. an ambubag, a T piece oxygen delivery system or a mechanical ventilator, to be attached. Synthetic tubes are made from a non-irritant substance.

• The patient cannot breathe during suctioning and can quickly become hypoxic.
• If the patient has signs and symptoms of distress during the suctioning procedure, stop the procedure and call the nurse
• The vagus nerve near the pharynx may be stimulated by the suction catheter and may cause bradycardia or other arrthymias.

Cautions For Oxygen Therapy
• Oxygen toxicity – can occur with Fio2 > 60% longer than 36 hrs • Fio2>80%longer than 24 hrs Fio2>100%longer than 12hrs • Suppression of ventilation – will lead to increased CO2 and
carbon dioxide narcosis • Danger of fire • Absorbtion Atelectasia • Premature retrolental fibroplasia
