ชายไทยคู่ อายุ 33 ปี
อาชีพรับจ้าง ภูมิล าเนา กทม.
อาการส าคัญ: ตาซ้ายมัวลง 1 วันก่อนมารพ.
ประวัติปัจจุบัน2 สัปดาห์ก่อนขณะก าลังพูดคุยกับญาติ มีอาการพูดล าบาก พูดไม่คล่อง รู้สึกเหมือนไม่มีแขนขาด้านซ้าย เป็นอยู่ 2-3 นาทีอาการดีขึ้น แต่ยังพูดไม่คล่องเหมือนเดิม
ประวัติปัจจุบัน
แพทย์นัดท า CT brain และให้ยากัน
หลอดเลือดอุดตันไปรับประทาน ASA
วันละ 300 มก. 7 วันต่อมาอาการพูด
ล าบากดีขึ้น
ประวัติปัจจุบัน
1 วันก่อนมาโรงพยาบาล ตาซ้ายมัวลง เกิดขึ้นเองไม่ได้รับอุบัติเหตุกระแทกที่ตา แพทย์บอกมีเลือดออกในตาซ้าย hyphemaและรับไว้ในโรงพยาบาลไม่มีประวัติปวดท้องเรื้อรัง หรือปวดเป็น ๆ หาย ๆ ไม่มีถ่ายอุจจาระด าหรืออาเจียนเป็นเลือด
ประวัตอิดีตปฏิเสธโรคประจ าตัวอื่นๆ ดื่มสุรา 3-4 วันต่อสัปดาห์ ครั้งละครึ่งกลม 10 ปี สูบบุหรี่ 4-5 มวนต่อวัน ประมาณ 10 ปี
ประวัตคิรอบครัว
ไม่มีประวัตโิรคพันธุกรรมในครอบครัว
มีพี่สาว 1 คน และน้องสาว 1 คนแข็งแรงดี
Physical ExaminationBT 37oC, PR 76 /min, RR 18 /min, BP 120/80 mmHg
Good consciousness, afebrile, not pale, no jaundice, no carotid bruit, no sign of chronic liver disease
Heart: regular rate & rhythm, normal S1, S2 no murmur
Lung: Clear no crepitation
Abdomen: Soft, non tender, liver span 12 cm, spleen 2 FB lower left costal margin, no ascites
Lymph node: Not palpable
Skin: No petechia, no purpura
Neurological signs: Mild dysarthria, left pupil 5 mm not react to light, right pupil 2.5 mm react to light, cranial nerves: normal,
motor : grade V all reflex: 2-3 + ,
clonus negative
Physical Examination
CBC: Hct 43%, Hb 15 g/dL, WBC 2.9x109/L, PMN 65%, L 35%, platelet 62x109/L MCV 89 fL, MCH 30 pg, MCHC 34 g/dLCoagulogram: normalLFT: Normal
