背景說明全文為： Clinical Document Architecture發行版本：

ANSI/HL7 CDA R1.0-2000 ANSI/HL7 CDA R2.0-2005

已是 ISO標準 (TC 215 / ISO/HL7 27932) 主要維護者是： HL7 Structured Documents Technical

Committee (SDTC)主要是定義資料交換之規範，使用到的技術有：

XML, HL7 Reference Information Model (RIM) Version 3 methodology Vocabulary (LOINC, SNOMED, ICD, local等 )

甚麼是電子文件從 XML角度，任何一種都是“ document”。 Documents從醫療的角度：

表現出歷史醫療記錄格式。把分散的資料及片斷病歷敍述整合在一起。

CDA的目的就是將健康照護過程中所產生的電子文件進行規範。

定義 CDA特性 Persistence ：可作為永久儲存之格式

採 XML技術可永久儲存。 Stewardship：可供文件管理系統管理

內含版本控管機制與其他文件資訊，可供DMS來使用。 Potential for authentication：可具備鑑別性

因是電子文件可被簽章。 Context：文件具有脈絡性

透過 CDA的結構描述，可將文字敘述中的前後文關係表達出來。

Wholeness：欄位具備完整性透過 CDA R2的結構可充分表達病歷文件之資訊。

Human readability：資訊可由人直接讀取透過瀏覽設計可得到與紙本病歷相同樣式，滿足閱讀習慣。

CDA 與 HL7訊息的關係 HL7 訊息是交換 CDA文檔的補足說明資料。 CDA是完整定義的資訊物件，是獨立於訊息之外。 CDA文件可用MIME編碼放入 HL7 訊息中。

XML 本文有兩大類型描述區塊

人類可讀（ Human-Readable ）敘述區塊（ narrative block ）用來表達與臨床、法律所需的內容。一般而言，仍須透過 xslt技術將其轉換成具有呈現格式的文件，如 html或 pdf。

定義上屬於 Level 2。機器可讀（ Machine-Readable ）─ Optional

屬 CDA 條目的部份，都是系統自動產生。系統可透過 CDA R2的結構關係，搜尋特定編碼，取得資訊。定義上屬於 Level 3。

setId：文件序列號，第一份文件賦予新序列號。 append時：新序列號。 replace時：同序列號。

versionNumber：版本序，辨識同一文件序列號者。 (replace 才會新增序號 )

id：文件流水號，只要新產生文件，就給予一個流水號。

可以是某系統。participant 有共同參與照顧此病患的相關醫護人員。perfomer 需與 ServiceEvent 配合。也就是指負責執行某項服務任務

的人。

custodian 負責保管此臨床文件的組織。informationRecipient

可以取得此臨床文件副本的人。

dataEnterer 實際負責將資料輸入電腦系統者。encounterParticipant

實際進行就診行為者。例如門診醫師。
