on the number of assets afterwards if the minimum amount per asset is not less than USD/EUR/AUD50/CNY350/HKD400/JPY5,000 for Intelligence Plan and

USD/EUR/AUD1,000/CNY7,000/ HKD8,000/JPY100,000 for Bright Plan.

2. The assumed rates of return used above are the annual returns of the underlying assets and after the deduction of the Plan Administration Charge of 1.44%

p.a. and any asset management charges, which has the effect of making the demonstrate rates 6.44% and 12.44% p.a. respectively. And the figures alsoassume that a constant growth rate has been achieved during the contribution payment term.

上記利回りは選択先アセットの年間リターンをそれぞれ6.44％と12.44％（アセット管理会社側手数料を含む）と仮定し、プランにかかる管理費1.44％（年率）

を差し引いています。また、積立期間中の年間利回りが一定であることを前提として計算されています。

for the actual underlying asset charges and fees.

実際の選択先アセットの手数料および費用については、メティスグローバルリミテッドが提供する「グローバルアセットセレクション」および各アセット会社が提

供する資料にてご確認ください。

4. These are not maximum or minimum amounts on encashment values. What you get back will depend on the performance of your asset choices. You should

only apply this Plan if you intend to pay the contribution for the whole of your chosen contribution payment term. Should you terminate this Plan early, youmay suffer a significant loss.

consistent with my/our financial planning horizon and objectives. At my/our own discretion, I/we confirm that I/we wish to proceed with my/our contribution in

I/We understand that I/we may choose the asset choices to which my/our Plan is to be linked, and request that Metis Global Limited (“Metis”) allocate my/our

contribution to the assets detailed in Section 4 of this Client Information Form. I/We understand that Metis does not provide asset allocation advice and that any

published asset list should not be considered a recommendation.

I/We agree to Metis acting on instructions received from me/us or my/our appointed advisor/agent. Metis shall not in any event be liable to me/us for any

liabilities, losses, damages or expenses whatsoever arising out of or in connection with any failure or delay in processing any instructions for the subscription,

switching or redemption of assets or otherwise; and Metis shall not be responsible for the performance or for any loss or liability arising from my/our choice of

asset, howsoever arising.

私/私達は、メティスが私/私達または私/私達のアドバイザー/代理人の指示に従い行動していること、アセット購入、スイッチング、売却指示において処理の不

履行や遅延により生じる如何なる負債、損失、損害および費用に対して一切の責任を負わないこと、また、パフォーマンスや選択したアセットから生じた損失、

I/We confirm that I/we understand that certain listing assets in Asset Selection may have restrictions on their ability to raise cash in the future, and that further

details are included in the Prospectus of the underlying asset issued by the respective Asset Manager and Product Issuer. Metis may sell assets linked to the Plan

without requiring my/our permission if it decides that an asset may have harmful legal or tax consequences under law or otherwise be prejudicial to the

interests of Metis or to my/our interests but Metis shall have no obligation to do so.

私/私達はメティスが提供するグローバルアセットセレクションの中に将来現金化が制限される可能性のあるアセットも含まれていること、各アセットの詳細につい

てはアセット運用会社および商品発行会社が発行する目論見書上に記載されていることを理解しています。選択先のアセットが法律上或いは税務上、有害で

あるとメティスが判断した場合や、メティスおよび私/私達の利益を損なう恐れがあると判断した場合、メティスは私/私達からの許可を得ずに選択先のアセットを

売却することができること、また、メティスがそれらを行う義務を負わないことを理解しています。

I am/We are aware that there may be fees to pay when Metis sells one or more of the assets linked to the Plan. Any fees due when selling an asset should be detailed by the Asset Manager or Product Issuer in the product documentation. I/We confirm that I am/we are aware of the fees that are payable in relation to

my chosen assets. I/We further confirm and agree that Metis or its related companies could receive trail or management fee rebate from the Asset Manager or

Product Issuer based on the selection of assets by myself/ourselves or my/our appointed portfolio manager.

私/私達はプランの選択先アセットを売却する際に手数料が発生する可能性があり、売却時の手数料についてはアセット運用会社および商品発行会社が発行

Portfolio Manager ポートフォリオマネージャー

I/We agree to Metis disclosing all information relating to my/our Plan to my/our appointed portfolio manager and servicing agent. I/We will let Metis know in

writing if I/we decide to change any appointed portfolio manager or servicing agent. I/We also understand that my/our authorized portfolio manager or servicing

agent is acting as my/our agent and not the agent of Metis.

私/私達はメティスが私/私達のプランに関連する全ての情報を私/私達が任命したポートフォリオマネージャーおよびサービスエージェントへ開示することに同

意します。私/私達は任命先のポートフォリオマネージャーおよびサービスエージェントを変更する場合、メティスへ書面にて通知します。また、私/私達が任命し

たポートフォリオマネージャーおよびサービスエージェントは私/私達の代理人であり、メティスの代理人として機能しないことを理解しています。

FATCA 米国外国口座税務コンプライアンス法（FATCA）

I/We declare that I am/we are neither (and I am/we are not making this application on behalf) a U.S. resident/citizen nor a U.S. person as defined under FATCA

(Foreign Account Tax Compliance Act). I/We further declare that I/we do not have any obligation on filing US tax. I/We may be required to provide a

self-certification or other documentation to Metis in order to establish their relevant classification under FATCA. Furthermore, if there is any change in

circumstances that would affect the FATCA classification of me/us, I/We should inform Metis immediately and provide an updated or additional certification

and/or documentation. Similarly, if there is reason for Metis to know that the self-certification is incorrect or unreliable; a new self-certification and/or additional

documentation will be required.

私/私達は、米国の“外国口座税務コンプライアンス法（以下「FATCA」とする）”にて定める米国人、米国居住者ならびに米国市民ではなく、またそれらの代理人と

して当申込みを行う訳ではないことを誓約します。さらに米国に対し、税の支払義務を負わないことを誓約します。私/私達は、FATCAに基づき、メティスへ自己申

告書および関連書類の提出を求められる可能性があることを理解しています。また、状況の変化によりFATCAに該当する状況となった際には速やかに最新の自

己申告書および関連書類をメティスに提出します。また、私/私達が提出した自己申告書および関連書類をメティスが事実に基づかない、または信頼性に欠け

ると判断した場合には、追加書類の提出が求められることを理解しています。

Commission/Referral Fee Disclosure for Intermediaries 手数料／紹介手数料の開示

I/We understand, acknowledge and agree that, as a result of my/our purchasing and taking up the Plan to be issued by Metis, Metis will pay the intermediaries

commission/referral fee during the continuance of the Plan including renewals, for arranging the said Plan. Where I am/we are a body corporate, the authorized

person who signs on behalf of myself/ourselves further confirms to Metis that he or she is authorized to do so. I/We further understand that the above

agreement is necessary for Metis to proceed with the application.

私/私達は、私/私達がメティスの発行する当プランを契約および購入した場合、メティスが仲介業者へ仲介手数料/紹介料を支払うことに同意します。また、私/

私達が法人で当プランを申込み、且つ私/私達に代わりサイン権限者が署名をする場合、メティスへそのことを事前に確認する必要があることを認識していま

Legal 法務

I/We understand that Metis is an authorized Trust Company in Hong Kong. I/We acknowledge and agree that the Plan Schedule, if issued, will be governed by

the laws of Hong Kong and subject to the jurisdiction of the Hong Kong courts. I/We fully understand the risks (including but not limited to the counterparty

risks, market and financial risks associated with assets in the Plan) and merits, as well as the legal, tax and accounting characteristics and consequences of

investing in the Plan. I/We have consulted my/our own financial, accounting, tax and legal advisors as I/we deem necessary or appropriate.

私/私達はメティスが香港にて認可を受けた信託会社であることを理解しています。私/私達はプランが実行された場合、プランの法的な解釈には香港の法律が

適用されること、ならびに香港裁判制度の管轄になることを理解および同意しています。私/私達はリスク（カウンターパーティーリスク、市場リスクおよびプランの

アセットに関連するフィナンシャルリスクを含むがこれらだけに限定しない）および利点、法務、税務、会計特性について理解しています。私/私達は必要または

適切と感じた場合、私/私達自身のファイナンシャルアドバイザー、会計士、税理士、法律顧問に助言を求めます。

Cooling-Off Rights クーリングオフの権利

I/We understand that I/we have the right to cancel and obtain a refund of any contribution(s) paid without interest less any market value adjustment, by giving

written notice. I/We understand that such notice must be signed by me/us and received directly by Metis Global Limited at 2701, 27/F., Berkshire House, 25

Westlands Road, Island East, Hong Kong within 30 days of the Plan Issue Date.

私/私達はクーリングオフの権利があること、また、書面にて申込キャンセルの意志を通知することで、市場価格調整差引後、金利なしにて積立金の返金が受け

られることを理解しています。私/私達は当該通知について、自身が署名したものが、プラン発効日から 30 日以内にメティスグローバルリミテッド（住所：Metis

Global Limited, 2701, 27/F., Berkshire House, 25 Westlands Road, Island East, Hong Kong）へ到着している必要があることを理解しています。

No Rights of Third Parties第三者権利不適用

Save as expressly provided to the contrary in this Agreement, a person who is not a Party has no right under the Contracts (Rights of Third Parties) Ordinance (Cap.

623 of the laws of Hong Kong) to enforce or enjoy the benefit of any term of this Agreement.

香港法条例第 623 章（第三者権利条例）に基づき、如何なる人物も本合意書の当事者に該当しない場合、本合意期間を通して本合意事項に基づく利益等を

享受することは出来ない。





Section 8： Personal Information Collection Statement -- Personal Data (Privacy) Ordinance

At Metis Global Limited (“Metis”), we hold as one of our core values the protection of privacy of our customer’s personal data. We are bound by the Data

Protection Principles set out in the PDPO. We respect of your privacy and the use or disclosure of your personal data by us will be in accordance with the PDPO

and this Personal Information Collection Statement. メティスグローバルリミテッド（以下「メティス」とする）はお客様の個人情報保護を重要な責務と捉え、お客様情報の管理、使用および開示について、個人情報（プライバシ

ー）保護条例および以下「お客様プライバシーに関する声明」を順守します。

1. Accuracy of Personal Data 個人情報の正確性Provision of accurate personal data to Metis by you is voluntary. From time to time, personal data of Customers is (or will be) collected by or on behalf of Metis to enable it to carry on its day-to-day business and to provide services to Customers. Failure to provide accurate personal data requested on this form may result in inability to process your application or to provide services. お客様よりご提供いただく個人情報の正確性についてはお客様側の自己責任となります。お客様へ継続したサービス提供および日々の業務遂行にあたり、メティス或いはメテ

ィスの代理人がお客様情報を収集しています。当申込書において正確な個人情報をご提供頂けない場合、お申込手続きおよびサービスを遂行できない可能性があります。

2. Purposes of collection Personal Data 個人情報収集の目的についてYour personal data held by Metis may be used for the following purposes: メティスで収集したお客様の個人情報は以下目的に利用する場合があります。(a) administration of plan business, which includes processing and evaluation of applications, identity and credit checking, suitability checking,

investigation, account/debt collection, communications, data analysis and research, internal and external audit, maintaining quality services, sales and marketing, and customer loyalty building; お申込手続き、お申込審査、身分証明および信用調査、適性審査、調査、勘定/債権回収、連絡、データ分析および調査、内部・外部監査、サービス品質の

維持、営業、顧客ロイヤルティの確立等のプラン関連業務； (b) direct marketing, which includes promoting, marketing or providing of Metis related products or services and/or any financial related products or

services provided and/or marketed by third party financial institutions by electronic or other means;メティス関連商品またはサービス或いは、第三者金融機関の金融関連商品またはサービスに関連した電子的またはその他手段によって提供された販売促進、

マーケティング等のダイレクトマーケティング；(c) to fulfill the disclosure requirements of any laws, regulations or guidelines as may in present or future be applicable to Metis within or outside Hong

Kong issued by any legal entities or other authorities, or industry bodies or associations of financial services providers. 現在また将来的にメティスに適用される香港または香港外の法律機関、当局、業界団体および金融サービスプロバイダー団体による法律、規則およびガイドラ

The personal data held by Metis will generally be kept confidential, but Metis may disclose these personal data to other third parties for plan processing purposes, including the following: メティスへ提供されたお客様の個人情報はメティスにより機密保持されますが、プラン手続きの際、以下第三者機関に開示される場合があります。

(a) any of the Metis’ affiliates; メティスの関連会社；(b) any financial institution or financial service provider who is in a position to process the payment of, or handle the payment instructions or

authorization of any monies to or by the Customer; お客様のお支払手続き、お支払依頼およびお客様より承認を受けた支払を実行する金融機関または金融サービスプロバイダー；

(c) any professional advisor, data processor, IT service provider, mailing house or third party service providers providing administration services who is engaged to provide independent advice or service in a specialized area to Metis and/or the Metis’ affiliates;専門アドバイザー、データ処理者、ITサービスプロバイダー、郵送業者、独立的助言或いはメティスおよびメティスの関連会社の専門的な分野にお

Section 9：Appointment of Portfolio Manager (if applicable)

Asset Portfolio Management ポートフォリオマネジメントサービス I/We declare that I/we have delegated asset allocation decision to the Portfolio Manager, who has complete discretionary authority, without consulting

me/us first, to make all portfolio management decisions in relation to the Plan. I/We authorize Metis to accept the written instructions of the Portfolio Manager in relation to the asset choices under the Plan. 私/私達は当プランに関するアセット配分判断を上記ポートフォリオマネージャーへ一任し、私/私達からの確認を得ずに判断およびポートフォリオ管理が行

I/We hereby authorize Metis to act upon this authority until revoked by me/us by a written notice addressed to Metis but such revocation shall not affect any liability on the Portfolio Manager’s part in any way resulting from asset allocation instructions made prior to such revocation. I/We hereby agree that if I/we appoint the Portfolio Manager arranged by Metis, Metis has the right to allocate my/our contribution into the asset(s) or portfolio(s) directly managed by my/our selected Portfolio Manager. 私/私達はメティスが本任命について書面での取消通知がない限り有効であると解釈すること、また、取消前のアセット配分指示がどのような結果を招いた場合

I/We hereby agree that Metis will not be responsible for or be liable for any losses arising in the value of the Plan resulting from advice given by or negligence or default of the third party Portfolio Manager or any switching instructions or any other unauthorized action taken by the third party Portfolio Manager concerning the Plan. 私/私達は第三者機関となるポートフォリオマネージャーの助言や過失、不履行、選択先アセットのスイッチング指示、その他当プランに関して承認されていない行為により当プランの評価額に大きな損失が生じた場合でも、メティスがそれに対して一切の責任を負わないことに同意します。

“Information”) have been provided to “the Company”, and that all such Information is true, accurate, complete and not misleading in any way, and I

/we undertake to provide any further information and documentation reasonably required from time to time.

開示する場合があることに同意します。

10. I/We agree that all Data collected by “the Company” may be disclosed to affiliates, custodian, insurers, auditors, advisors and to any government,

quasi-government, regulatory, fiscal, monetary or other authorities, bodies or persons where “the Company” is obliged to do so by any applicable law or

regulation or code or pursuant to a court order, and Data may be used for internal use, compiling statistics for internal use and marketing activities.

11. I/We consent and agree that “the Company” may receive commission, incentive or rebate from product providers in respect of any Product or Plan and

Company” in connection with any act done or transaction entered into following or as a result of such introduction or otherwise in relation to the affairs

__________________にて登録を受けていないオフショア信託プランであることを理解しています。

2. I/We confirm that any information or document provided to me/us in relation to the Plan is absolutely at my/our own request. I/We confirm and agree

to all the declarations and statements made or given by me/us in the Personal Details and Financial Needs Analysis Form. All information and details

completed by me/us in the Personal Details and Financial Needs Analysis Form are true, accurate and not misleading in any way. I/We also confirm

having read and understood the document.

私/私達は、今回申込むプランに関し提供された情報および関連書類は、私/私達の要望に基づくものであることを確認します。個人情報および財務

状況シートの中で私/私達が声明または誓約した内容は、私/私達が作成または提出したものであることを確認します。私/私達が個人情報および財務

状況シート上にて提供した全ての情報および詳細は事実且つ正確なものであり、如何なる虚偽がないことを誓約します。上記書類の内容を読み、理解

したことを確認します。

インテリジェンス (以下「本プラン」)，を契約年数年、毎月/ 四半期/ 半年/ 毎年、積立金米ドル/ ユーロ/ 豪ドル/

人民元/ 香港ドル/ 日本円を、クレジットカード/ 銀行自動送金/ 預金口座振替/ 人民

元支払/ 銀行送金にて申込みます。

Bright Plan (“the Plan”) with a lump sum of USD/ EUR/ AUD/ CNY/ HKD/ JPY as the single contribution.

ブライト (以下「本プラン」)，を一括運用資金米ドル/ユーロ/豪ドル/人民元/香港ドル/日本円にて申込みます。

4. I/We understand that fees and charges are still deductible under the Plan if the regular contribution is stopped or reduced during the term of the Plan.

私/私達は契約期間中に積立金の支払いを停止或いは減額した場合でも、手数料および所定費用は引き続き発生することを理解しています。(積立プ

ランにのみ適用)

5. I am/We are happy to accept phone call from “the Company” to confirm all the information provided is accurate.

私/私達は今回の申込にあたり提供した全ての情報に関し、「メティスおよび関連会社」より内容確認の電話を受けることを承諾します。

6. I/We understand that the return of underlying assets may fluctuate and are not guaranteed. Past performance is not necessarily a guide to future

performance.

私/私達は、アセット運用利回りが変動すること、いかなる保証もないこと、また、過去のパフォーマンスが将来のパフォーマンスの指標とはならないこと


7. I /We represent and warrant that all information and documentation (including without limitation forms required by “the Company” to be completed by

me/us, Client Information Form, declarations, risk profile questionnaires, financial needs analysis and forms in respect of any Plans (collectively

“Information”) have been provided to “the Company”, and that all such Information is true, accurate, complete and not misleading in any way, and I

/we undertake to provide any further information and documentation reasonably required from time to time.

私/私達は全ての情報および書類（今回の申込みに関連した申込書、誓約書、リスク許容度アンケート、財務状況シートに限らず提出した全ての書類）

は申込者本人が記入および完成させたものであることを保証します。全ての情報に偽りはなく、正確であり、誤解を与える情報全くはありません。また、

私/私達は今後においても情報および書類が必要な際にはそれらを提出することを承認します。

8. I/We undertake to inform “the Company” of any changes in the information.

私/私達は上記記載内容に変更があった際は「メティスおよび関連会社」へ速やかに通知します。

9. I/We agree that “the Company” can appoint its subsidiaries or affiliates (thereafter "these related companies") or third party company to provide

services, place business and agree that “the Company” can transfer or disclose the above information to these related companies or third party

company for provision of services.

私/私達は「メティスおよび関連会社」が第三者機関へサービスまたは業務を委託する場合があること、また、それに伴い第三者機関へ上記記載内容を

開示する場合があることに同意します。

10. I/We agree that all Data collected by “the Company” may be disclosed to Affiliates, Custodian, insurers, auditors, advisors and to any government,

quasi-government, regulatory, fiscal, monetary or other authorities, bodies or persons where “the Company” is obliged to do so by any applicable law or

regulation or code or pursuant to a court order, and Data may be used for internal use, compiling statistics for internal use and marketing activities.

私/私達は「メティスおよび関連会社」へ提供した全ての情報について、法律、規制或いは裁判所の命令により必要性が発生した場合には、保管機

関、保険会社、監査機関、顧問機関、政府機関、政府関連機関、監査当局、財務当局、金融当局およびその他関連当局、団体、個人へ情報を開示

することに合意します。また、内部において統計およびマーケティング資料として情報を利用することに同意します。

11. I/We consent and agree that “the Company” may receive commission, incentive or rebate from product providers in respect of any Product or Plan and

“the Company” will pay commission, incentive or referral fee to respective introducers.

私/私達は「メティスおよび関連会社」が商品およびプランに関連して商品プロバイダーよりコミッション、報奨金、手数料を受け取る事、「メティスおよび

関連会社」が紹介者にコミッション、報奨金、紹介料を支払う事に合意します。

12. I/We consent and approve to the person introducing me/us to “the Company”, to solicit and accept a commission or other advantage from “the

Company” in connection with any act done or transaction entered into following or as a result of such introduction or otherwise in relation to the affairs

or business of me/us with “the Company”.

私/私達が「メティスおよび関連会社」と業務や取引等ビジネスをするにあたり、私/私達に「メティスおよび関連会社」を紹介した者が、「メティスおよび関

連会社」よりコミッションやその他利益を受け取ることに同意します。

13. I/We authorize “the Company” to act upon my/our facsimile instructions on dealing, switching or redirection of assets only (“dealing facsimile

instructions”) and agree to indemnify “the Company” and her companies under the same group against all liabilities arising out of or in connection with

my/our dealing facsimile instructions. I am/We are fully aware of the potential risks of giving “the Company” instructions by facsimile and agree that

“the Company” have discretion to request original written confirmation of instructions from me/us. I/We would take full responsibility to prevent any

fraudulent, forged or unauthorized instructions. I/We also agree that “the Company” have its sole discretion and without any liability to refuse to act

upon my/our instructions as “the Company” think appropriate.

私/私達は「メティスおよび関連会社」に対し、私/私達によるファックス指示に基づき、私/私達に代わって行動するよう、その権限を委譲します。また、

私/私達の出した指示に関連し何か問題が発生した場合、「メティスおよび関連会社」はその法的責任を一切負わないことに合意します。私/私達は、フ

ァックスによる指示には潜在的なリスクが伴うことを理解しており、よって、「メティスおよび関連会社」が書類原本確認の要請をする際には速やかに対応

します。私/私達は不正または偽造指示等が発生しないよう責任を持ち対応します。私/私達は、「メティスおよび関連会社」が私/私達の指示に対して

妥当であると判断した場合にそれらを拒否する権限をもつこと、またそれらに対し法的な責任を一切負わないことに同意します。

I/We confirm and agree to all the declarations and statements made or given by me/us in “the Agreement”. 私/私達は、本合意書上の声明または誓約事項が、私/私達により作成または提供されたものであることを誓約します。

I/We agree to all the terms and conditions in “the Agreement”, and shall abide by all such terms and conditions. 私/私達は、本合意書上に記載されている条件・条項に同意し、全ての条件・条項を順守します。
