วัตถปุระสงค
ระบบประกันสุขภาพในประเทศไทย มี 3 ระบบหลักสวัสดกิารรกัษาพยาบาลขาราชการ
(Civil Servant Medical Benefit Scheme: CSMBS)ประกันสังคม (Social Security Scheme: SSS)ประกันสุขภาพถวนหนา (Universal Coverage: UC)ประกันสุขภาพเอกชน (Private Insurance)จายเงนิจากกระเปา (Out-of-Pocket: OOP)

DRG เปนระบบแบงกลุมผูปวยอยางหนึ่ง (Patient Classification System) ที่อาศัยขอมูลไมมากนัก
มาจัดกลุมผูปวยที่เขารับการรักษาในโรงพยาบาลในระยะเฉียบพลัน เพื่อบอกวาผูปวยในกลุมเดียวกนั จะ
ใชเวลานอนในโรงพยาบาลใกลเคียงกัน และสิ้นเปลืองคารักษาใกลเคียงกัน (Iso-resource group)

ระเบียบวธิกีารพัฒนา DRG
• น้ําหนักสัมพัทธ หมายถึง คาเฉลีย่ของการใชทรัพยากรในการรักษาผูปวย DRG นัน้ เทยีบกับตนทุนเฉลีย่ของการรกัษาผูปวยทั้งหมด
• การใชคาน้ําหนักสัมพัทธ - พัฒนาคามาตรฐานของแตละประเทศเอง- ยอมรับคาทีป่ระเทศอื่นพัฒนาแลวมาใช

RW = คารักษาพยาบาลเฉลี่ยในแตละกลุม DRG คารักษาพยาบาลเฉลี่ยทั้งหมด
วิธคีาํนวณน้ําหนักสัมพัทธ

กลุมวินิจฉัยโรครวม ฉบบัที ่1- เริ่มป 2536 พรบ. ผูประสบภัยจากรถ และขยายสูสวัสดิการสําหรับผูที่สังคมควรชวยเหลือเกื้อกูล และใชเพื่อจายเงินแกผูปวยที่มีคารักษาราคาสูงในโครงการสวัสดิการประชาชนดานการรักษาพยาบาล ตุลาคม 2541- ไมครอบคลุมรหัสโรคตางๆ อยางครบถวน - คะแนนน้ําหนักสัมพัทธไมสะทอนการใชทรัพยากรในการรักษาผูปวยในระดับตางๆ
การพัฒนา DRG ในประเทศไทย
กลุมวินิจฉัยโรครวม ฉบับที่ 2- แกไขจุดออนของฉบับที่ 1 โดยสํานักงานประกันสุขภาพ- ฉบับที่ 1 และ 2 อางอิงวิธีการจัดกลุม DRG ที่ใชในโครงการสวัสดิการผูสูงอายุของสหรัฐอเมริกา (Medicare)- นํารหัสวินิจฉัยโรคทั้งหมดที่มีอยูใน ICD-10 ใสลงในฐานขอมูล DRG- ปรับกลุมโรคเปน 510 กลุมตาม HCFA-DRG version 17- ไมละเอียดสําหรับกลุมโรคเด็กและโรคที่พบในเขตรอน
การพัฒนา DRG ในประเทศไทย
กลุมวินิจฉัยโรครวม ฉบบัที ่3- หลักประกันสุขภาพทั้ง 3 หลักประกัน - กลุมโรคเด็กแยกยอยมากขึ้น โดยเฉพาะเด็กทารกแรกเกิด- แยกกลุมโรคเขตรอน ใหสะทอนแบบแผนการรักษาที่มีตนทุนแตกตางกัน- แยกระดับความรุนแรงที่แตกตางกัน โดยพิจารณาภาวะแทรกซอนและโรครวม
การพัฒนา DRG ในประเทศไทย
DRG เปนเลข 5 ตัว มีรูปแบบ คือ MMDDX
- MM = เลข MDC (00-26)- DD = เลข DC
01-49 สําหรับการมี procedure (surgical DRG)50-99 สําหรับการไมม ีprocedure (medical DRG)

DRG เปนเลข 5 ตัว มีรูปแบบ MMDDX
