คลื่นไฟฟ้าหัวใจ
(Electrocardiogram, ECG หรอื EKG) คือผลรวมของการเปลี่ยนแปลงศักย์ไฟฟ้านอกเซลล์ ที่เกิดจากการส่งผ่าน Action Potential ในหัวใจ
การบันทึกคลื่นไฟฟ้าหัวใจ(Electrocardiography) อาจใช้เครื่อง Electrocardiograph บันทึกความต่างศักย์ระหว่างสองต าแหน่งบนผิวกายหรือบันทึกที่หัวใจโดยตรง

- 6 limb leads (I, II, III, aVR, aVL, aVF); Frontal plane
- 6 precordial leads (V1-V6); Horizontal plane
Each lead has a (+)ve pole and (-)ve pole.
- Limb leads: bipolar (I, II, III) &
unipolar (aVR, aVL, aVF)
- Precordial leads: unipolar
Electrical activity approaching a (+)ve pole will as a
(+)ve [upright] deflection on ECG tracing.

ส่วนประกอบของกระดาษตรวจคลื่นไฟฟ้าหัวใน แกน X – เวลา (duration), แกน Y – ความสูง (amplitude)
•1 ช่องเล็ก = 1mm
•ความเร็วกระดาษเท่ากับ 25 mm/s ดังนั้น 1ช่องเล็กสุดในแนวนอนจึงเท่ากับ 1/25 หรือ 0.04 s
•ในแนวตั้งมาตรฐาน 1 mV = 10 mm โดยสังเกต ได้จากcalibration signal
ดังนั้นก่อนแปลผล ECG ทุกครั้งต้อง check paperSpeed (ดูท่ีเครื่องหรือดูจากECG waveformที่กว้างผิดปกติทั้งหมด) และ calibration signal (ดูท่ีtracings) ด้วย

General Tips
• There is no absolute right way to read an
ECG.
• A prior ECG is your best friend.
• Knowing the patient‟s clinical history is
essential for correct ECG interpretation
(ECG machines make squiggly lines on
paper, doctors make diagnoses).

Step Step 11: Heart Rate : Heart Rate
ดูว่าปกต ิเร็ว หรือช้า ซึ่งมีวิธีการค านวณดังนี้
1. วัดระยะห่างระหว่า R1 และ R2 หรือ R-R interval ว่าเป็นกี่มิลลิเมตร (อาจนับเป็นช่องเล็ก (ช่องละ 0.04 s )
หรือช่องใหญ่ (ช่องละ 0.04 x 5 = 0.2 s ))
2. เปล่ียน “มิลลิเมตร” ให้เป็นเวลาโดยใช้ 0.04 หรือ 0.2
คูณจ านวนช่องเล็กหรือช่องใหญ่ตามล าดับ (ขึ้นกับว่านับเป็นช่องเล็กหรือช่องใหญ่)
3. น าเวลาที่ได้ไปค านวณหาอัตราการเต้นของหัวใจ

• ( 1 small box = 0.04 s)
• Then, duration (time) per beat = 20 x 0.04 = 0.8 s
• Then, in time duration 0.8 s heart rate = 1 beat

= 75 beats
∴ Heat Rate = 75 beats per minute

= 1500/n (beats/min) , n = ช่องเล็ก
วิธีการค านวณวิธีที ่3
จดจ าตัวเลขคร่าวๆ ดังต่อไปนี้จากกฎ 300/150/100/75/60
โดยนับจากจ านวนช่องใหญ ่คือ ถ้าระหว่าง QRS
complex ห่างกัน 1 ช่องใหญ่ จะม ีHR = 300 beats/min
และถ้า 2, 3, 4 และ 5 จะมี HR = 150, 100, 75 และ 60

Step Step 22: The P wave : The P wave
• P Wave หมายถึง Atrial depolarization ปกติมีลักษณะดังนี้
– เรยีบและนูนโค้ง (smooth and round)
– กว้าง ≤ 0.12 วินาที (≤ 3 ช่องเล็ก) สูง ≤ 2.5 mV (≤ 3 ช่องเล็ก)
– Positive wave ใน I, II, aVF และ V2-6
– Negative wave ใน aVR
- Biphasic wave (อาจมีรปูร่าง Positive, Negative) ใน III, aVL
และ V1

Step Step 33: The PR Interval : The PR Interval
- PR หรือ PQ interval หมายถึง Time from atrium activation to ventricle activation (antrioventricular, AV, conduction time)
- วัดจากจุดเริ่มต้นของ P wave ถึงจุดเริ่มต้น QRS complex (ถา้วัดจากจุดสิ้นสุด P wave ถึงจุดเริ่มต้นของ QRS complex จะเรียกว่า PR segment) มีลักษณะปกติ คือ
- Duration: 0.12-0.20 วินาที (3 – 5 ช่องเล็ก), ถ้า < 0.10 วินาที: short PR syndrome (pre-excitation), ถ้า > 0.20 วินาที: AV block
- Shape: อยู่ในแนว Isoelectric line

PR > 0.2, remain the same beat-to-beat
Second degree AV block (Mobitz)
Mobitz I (Wenkebach): PR gradually increases, then fail to conduct.
Mobitz II: PR prolonged, P fails to conduct at set interval (i.e. 2:1, 3:1)
Third degree AV block (complete heart block)

First degree AV block
This is the mildest form of heart block. In this case,
the electrical signals from the SA node move
more slowly than normal to the AV node, but all
signals reach the ventricles.
PR > 0.2, remain the same beat-to-beat

Second degree AV block (Mobitz)
Some of the electrical signals are not reaching
the ventricles. This causes “dropped beats.”
Mobitz I (Wenkebach): The electrical signals become
increasingly delayed with each heartbeat,
ultimately causing a beat to be missed.
• PR gradually increases, then fail to conduct.

Second degree AV block (Mobitz)
Mobitz II: In this type of heart block, some of the
electrical signals do not reach the ventricles. This
is less common, but more serious.
• PR prolonged, P fails to conduct at set interval (i.e.
2:1, 3:1)

Third degree AV block
(complete heart block)
This is the most serious type of heart block. In
this condition, no electrical signals from the SA node
are able to reach the ventricles. The ventricles
compensate by contracting on their own.
• No relation between P and QRS (maintain separate
rates, P rate > QRS rate)

– Shape: Positive wave ใน Frontal plane ยกเว้น aVR (เป็นลบเสมอ
ถ้าเป็นบวก พบในภาวะติด lead ผิด โดยสลับระหว่างแขนซ้ายกับแขนขวา)

wave for the left and right ventricles. See rSR‟ in V1 and a deep wide S
wave in V5 and V6. QRS duration is generally > 0.12 ms and the initial
part of the QRS is fast.

แปลผลLeft Bundle Branch Block (LBBB): Slow „blending‟ of right
and left ventricular conduction. Initial part of QRS is slow, and
pattern shows a rS or QS in lead V1 and a broad, monophasic R wave
in V5 and V6. The QRS duration > 0.12 ms.

ลักษณะ T wave ปกติมีดังนี้– มีลักษณะโค้งมนและ asymmetric เล็กน้อย
– สูงไม่เกิน 5 mm ใน Limb lead หรือ 10 mm ใน
Precordial lead และไม่ควรต่ ากว่า 0.5 mm ใน I และ II
– Positive wave ใน Lead I, II, V2-6
– Negative wave ใน aVR แต่อาจ Positive หรือ
Negative ใน III และ V1

เปน็ wave ขนาดเล็กตามหลงั T wave ยังไม่ทราบกลไกการเกิด wave
น้ี เชื่อว่าเป็น repolarization ที่เกิดขึ้นที่ Purkinjae fiber มักพบในภาวะหัวใจเต้นช้า (ถ้าเห็นหัวใจเต้นเรว็เกิน 90 ครั้งต่อนาที มักไม่ค่อยเหน็ U wave) มักเหน็ไดช้ัดใน lead V2 และ V3 ปกติจะมีความสูงประมาณ 5-25% ของ T wave
โดยทั่วไปจะมขีนาด 1.5 mm และเห็นชัดในทุก lead ที่มีภาวะดังต่อไปนี้
- มีภาวะเกลอืแร่ผิดปกติ เช่น Hypokalamia, hypocalcemai, และ
hypomagnemia
- ได้รับยาต้านการเต้นหัวใจผิดปกต ิเช่น amiodarine, quindine,
procanamide, digitalis
- ภาวะ hyperthyroidism

• This type of ventricular tachycardia is defined as a
polymorphous ventricular tachycardia. The morphology of QRS
varies beat to beat with axis changes. Ventricular rate change
