Einschlusskriterien• Metastatic or unresectable locally advanced/recurrent breast cancer HER2-positive
disease documented as ISH-positive and/or 3+ by IHC on previously collected tumor tissue and prospectively confirmed central laboratory prior to study enrollment. Tumorgewebe muss vorhanden sein.
• Disease progression on the last regimen received as defined by the investigator.• Prior treatment with an anthracycline, trastuzumab, a taxane, lapatinib, and capecitabine
in the neoadjuvant, adjuvant, locally advanced, or recurrent/metastatic setting and documented disease progression (by investigator assessment) after at least two regimens of HER2-directed therapy in the metastatic or unresectable locally advanced/recurrent setting.
• Patients must have had at least 6 weeks of prior exposure in the metastatic (or unresectable locally advanced/recurrent) setting to lapatinib and capecitabine (given together or separately) unless they were intolerant of lapatinib and/or capecitabine. Patients who were found to be intolerant to lapatinib can be considered eligible if they experienced disease progression during a single trastuzumab-based regimen in the metastatic (or unresectable locally advanced/recurrent) setting.
Ausschlusskriterien • Brain metastases that are untreated or symptomatic, or require any radiation, surgery, or
steroid therapy to control symptoms from brain metastases within 2 months of randomization

• Endosonography and an esophageal-gastro-duodenoscopy• Categorization of gastroesophageal junction
tumors according to the classification by Siewert (1987, cf. appendix 1)• Involved coeliacal lymph nodes of the lower esophagus are considered as M1
• Detection of an adenocarcinoma with HER2 3+ (ICH)
or HER2 2+ (ICH) with amplification proven by FISH, SISH or CISH by an accredited local pathologist (for quality assurance tumor samples have to be available for a subsequent central review)
• No preceding cytotoxic or targeted therapy

Einschlusskriterien· Are under capecitabine therapy for treatment of colon or breast cancer at a regimen of 2 weeks on and 1 off (14+7) and a daily doses 2000-2500 mg/m2 with or without trastuzumab (Herceptin®) or bevacizumab (Avastin®).· DG of PPES grade 1 in any hand or foot according to the NCI CTCAE v4.03 definition (Appendix 20.1).· Are able to apply topical medication or provide for another person to apply it.· Have a life expectancy longer than 3 months.· Patients still have to undergo at least 2 planned cycles with capecitabine therapy
Auschlusskriterien. Use of other chemotherapies for the treatment of cancer · Diagnosis of PPES grade 1 in any hand or foot according to the NCI CTCAE v4.03 definition (Appendix 20.1) for more than 2 cycles previously to inclusion in this clinical study.· Have received topical corticosteroids in hands or feet 7 days prior to planned inclusion in the study.· Need to use other emollient creams or other topical treatments in hands and/or feet during the study. · Are receiving radiotherapy.
