HIV
• Es un síndrome multisistémico con base en la alteración inmunitaria.
• Propensión a sufrir infecciones oportunistas y neoplasias.
• La afectación ocular se presenta en el 73% de los pacientes.
• Vasculopatía retiniana, retinits por CMV, herpes, toxoplasma o sifilítica. Neoplasias de conjuntiva.

Epidemiología
• Luego de la pandemia en 1981, la incidencia se mantuvo.
• En 1996 disiminuyó la incidencia de infecciones oportunistas gracias al tratamiento HAART.
• En 2002 había 886,575 casos en total; el 57% ha fallecido.

Modos de transmisión
• Relaciones sexuales.
• Uso de drogas IV.
• Transfusiones de sangre.
• Placentaria.
• Riesgo de transmisión del virus después de la exposición percutánea es del 0,3%.
• Profilaxis antirretroviral tras exposición es efectiva.

Patogenia del HIV• Daño de las células CD4+.
• Inversión de la relación CD4+ / CD8+.
• Inversión de la relación células T colaboradoras y supresoras.
• Incompetencia inmunitaria mediada por células.
• Todas las etapas se caracterizan por una alta tasa de replicación vírica.
• Los cambios del ARN del HIV-1 en plasma predice la progresión clínica.

Terapia HAART
• Un inhibidor de la proteasa
• Dos análogos de nucleósidos
• Retrasa o previene la aparición de resistencias a los fármacos, disminuyendo la replicación vírica.

Estadíos
• Infeccion primaria por HIV (período de seroconversión): fiebre, faringitis, exantema cutáneo, artralgias, malestar, ulceraciones mucosas y manifestaciones neurológicas.
• Pródromo: linfadenopatías, fiebre, sudor nocturno, pérdida de peso, diarrea.
• Sida

Signos oculares
• Se ha detectado presencia de HIV en córnea, epitelio conjuntival y lágrimas.
• Las manifestaciones oculares más frecuentes: vasculopatía no infecciosa, retinits por CMV, sarcoma de Kaposi.
• Retinits por herpes, retinitis por toxoplasma, coroiditis por P. jirovecii, coroiditis por criptococco.

• Exudados algodonosos (50%).

• 15 al 40% de pacientes con Sida, con menos de 100/mm3.
• Tratamiento invasivo para prevenir la pérdida visual grave.
• Retinitis necrotizante lentamente progresiva que puede afectar polo posterior y periferia.

• Inhibe la replicación del ADN del CMV.• Se observa respuesta a las 2 - 4 semanas de
tratamiento.• 5 mg/kg cada 12 h por 14 dias y luego 5 mg/kg/d de
mantenimiento.• Recurrencias en el 40% de los ptes, en los bordes
cicatrizados.• Valganciclovir VO: 900 mg cada 12 h por 3 semanas,
luego 900 mg/día.• Toxicidad: granulocitopenia.

• La toxoplasmosis es una infección oportunista del SNC. La afectación ocular se da en sólo el 3% de los pacientes HIV+.
• Lesiones uni o bilaterales, multifocales o diseminadas por contigüidad,
• Zonas necróticas extensas y varios focos activos.• Neuritis ópticas, panoftalmitis, iridociclis
unilaterales.• Sulfadiazina 4-6 g/día, pirimetamina y ácido fólico.
