Menurut Corbin et al (2001) kunci untuk menjaga dan memperluas pelayanan adalah
membuat masyarakat sadar dan membangun kepuasaan dan retensi konsumen, untuk
melakukannya, rumah sakit harus exude trait mereka seperti kepercayaan, loyalitas dan
reliabilitas (Hood and Henderson, 2005). Trait ini harus menjadi fokus utamanya dalam
pemasaran karena penting bagi manajer rumah sakit untuk membuat atau mempromosikan
sebuah merek berdasarkan trait ini. Untuk memiliki organisasi kesehatan yang sukses,
pemasaean harus berusaha untuk berfokus membangun kepercayaan yang tinggi dan
kepercayaan diri pasien (Kim, Kim, Kim, Kim and Kang, 2008). Dari perspektif pasien,
kepercayaan memiliki pengaruh yang signifikan dalam pengalaman menjadi penerima
layanan kesehatan dan perkembangan kompetensi dengan manajemen ilness (Thorne and
Robinson, 2006). Kepercayaan pasien penting karena semakin percaya diri seorang pasien
pada rumah sakit dan pelayanannya, maka makin cepat penyembuhannya.
Usaha pemasaran organisasi juga harus berfokus pada membangun merek yang kuat.
. Menurut Kotler (1997) kemampuan manajemen merek menjadi pertimbangan sebagai seni
dan cornerstone pemasaran. Ketika berurusan dengan branding, organisasi encoba untuk
membangun imej tertentu dan dihubungkan dengan atribut-atribut tertentu dengn pelayanan
mereka.
Ketika membanangun sebuah reputasi, dua hal yang mendukungnya adalah identitas
perusahaan dan imej perusahaan. Davies et al., 2003; Chun, 2005 memandang imej dan
identitas perusahaan sebagai elemen reputasi dibandingkan sinonim.
Menurut Kennedy (1977) karyawan memiliki pengaruh signifikan terhadap
pandangan stakeholder eksternal terhadap sebuah organisai. Banyak perusahaan
cenderung berfokus pada apa yang dipikirkan konsumen, bukan pada karyawan pikirkan
(Davies and Chun, 2007). Karena meningkatnya tingkat interaksi antara pelanggan eksternal
dan internal, maka perlu mengkombinasikan faktor-faktor yang mendukung reputasi (Hatch
and Schultz, 1997).
Hal ini mungkin dikarenan konsumen tidak memiliki pengetahuan yang detail tentang
banyak karakteristik perusahaan dibandingkan stakeholder lain. Karyawan memilikiinformasi lebih baik dan lebih detail ketika mengevaluasi reputasi perusahaan (Page dan
Fearn, 2005). Pandangan karyawan cenderung tercermin dalam bentuk emosional kepada
konsumen; hal ini termasuk bahasa tubuh dan ekspresi wajah (Davies dan Chun, 2007).
. Dari pandangan eksternal, apa yang manajer lakukan di dalam perusahaan akan
mempengaruhi bagaimana konsumen diperlakukan dan bagaimana memandang penyedia
layanan (Davies et al., 2004).
Hubungan kepercayaan jangka panjang antara merek pelayanan dan konsumen
menginformasikan dan reinforce budaya perusaahaan yang mana merek dan pemberian
pelayanan ditanamkan (De Chernatony and Segal-Horn, 2003).

Gotri dan Wilson (2001) menyatakan bahwa karyawan dan perilakunya menggambarkan
kenyataan dari organisasi kepada konsumen, adapun bila perilaku mereka tidak seperti
yang diharapkan yang dibuat oleh kampanye komunikasi eksternal organisasi maka repuasi
organisasi seluruhnya akan rusak. Organisasi ingin memperhatikan karyawan mereka,
sehingga karyawan akan memperhatikan konsumen, sehingga terlihat bersahabat dan
penolong. Organisasi juga menginginkan rasa percaya dan rasa hormat di antara karyawan.Perusahaan harus dipercaya, dikagumi dan disukai
Martineau (1958) mendefinisikan imej sebagai sejumlah atribut kualitas fungsional; dan
psikologi pada pikiran konsumen(Cited in Gotsi andWilson, 2001).
Menurut Marwick dan Fill (1997), imej perusahaan adalah totalitas dari persepsi stakeholder
dari cara organisasi menghadirkan diri, baik secara sengaja maupun tidak.
Imej perusahaan dimulai dengan stakeholder internal perusahaan dan bagaimana mereka
menerima perusahaan. Stakeholder eksternal kemudian dapat mengembangkan imej
organisasi tergantung imej mereka terhadap stakeholder internal. Imej perusahaan ini
terkonseptualisasi, bagaimana pandangan konsumen terhadap perusahaan mereka. Imej
perusaahaan berkaitan dengan karakteristik berwujud dan tidak seperti karakteristik
fungsional, fisik dan emosional yng dihubungkan dengan perusahaan (Davies and Chun,
2002).

Brown dan Dacin (1997) menyatakan identitas perusahaan adalah pandangan orangdi dalam organisasi. Van Riel and Balmer (1997) mendefiniskan identitas perusahaan
sebagai gambaran pribadi dari organisasi, yang berasal dari perilaku individu, bagaimana
anggota organisasi mengekspresikan organisasinya secara kontinyu.
Menurut Hatch and Schultz (1997), identitas organisasi berbeda dengan identitas
perusahaan identitas organisasi lebih kepada persepsi karyawan terhadap organisasi,
merujuk pada apa yang diterima stakeholder, rasa dan pikirkan tentang organisasi. Sifatnya
kolektif, merupakan pemahaman terhadap nilai dan karakteristik organisasi. Sedangkan
identitas perusahaan merujuk pada isyarat visual, seperti nama, logo, simbol atau isyarat
strategi seperti visi, misi dan filosofi. Identitas perusahaan harus mencerminkan karakteristikatau kepribadian perusahaan yang berasal dari perilaku anggota organisasi (Balmer, 1997).
Chun (2005) menyatakan bahwa krisis reputasi dapat dicegah dengan memonitor
kesenjangan antara apa yang dipikirkan karyawan dan apa yang orang lain pikirkan.
Hibbard,
Stockard and Tusler (2005) menyatakan bahwa jika reputasi rumah sakit terpengaruh, maka
pasaran akan menurun karena pilihan konsumen atau rujukan dokter.
Pelayanan yang sukses tergantung pada komunikasi internal yang baik (Padanyi and
Gainer, 2003). Bagaimana pelayanan diberikan terlihat sebagai pendukung reputasi

karena itu menuntun kepada kualitas yang diterima lebih baik dan evaluasi konsumen positif
terhadap pelayanan.
Kennedy et al. (2002) juga menemukan bahwa terdapat hubungan yang diinginkan
antara orientasi pasar dan komitmen serta kepuasaan konsumen. Donovan et al. 2004;
Hennig-Thurau, 2004) menyatakan bahwa organisasi yang berorientasi konsumen
tinggi idak hanya menyediakan kepuasaan dan komitmen saja, tetapi organisasi juga
harus memiliki lingkungan kerja yang baik.
Perusahaan yang berorientasi tinggi pada konsumen harus fokus dalam memperkerjakan
karyawan.
Retensi konsumen secara ekonomi juga lebih menguntungkan dibanding terus
menerus mencari pasien baru, adalapun membangun dan menjaga adalah hal yang penting
untuk keseluruhan kinerja bisnis (Bove and Johnson, 2000).Homburg and Stock (2005) menyatakan bahwa tingkat kepuasaan pada pekerjan
yang tinggi akan menuntun kepada tingkat orientasi konsumen yang tinggi.
Menurut Stock dan Hoyer (2005) perbedaan antara perilaku dan sikap adalah perilaku tidak
permanen (akan berubah jika situasi berubah), sikap bersifat lebih tahan dan stabil daripada
perilaku. Sikap berorientasi konsumen diperlukan untuk menciptakan perilaku berorientasi
konsumen. Perusahaan awalnya harus berfokus pada sikap yang akan berpengaruh positif
dan akan menghasilkan strategi orientasi konsumen jangka panjang. Perilaku cenderung
tidak konsisten, bisa berubah secara cepat. Jika perusahaan ingin karyawan memiliki
perilaku konsisten, mereka harus berkonsentrasi pada sikap (Stock and Hoyer, 2005).
Menurut Stock and Hoyers (2005) sikap berorientasi konsumen mendorong perilaku
berorientasi konsumen dan juga berpengaruh langsung terhadap kepuasaan konsumen.
Karyawan pelayanan kesehatan harus banyak terlibat dalam lingkungan kerja jika
mereka ingin memberikan kualitas layanan yang baik, memuaskan pasien, dan menuntun
pada kesetiaan konsumen (Scotti et al., 2007).
Keberlangsungan dari rumah sakit swasta tergantung dari keuntungan jangka
panjang. Keuntungan dapat dipertahankan jika konsumen mendapatkan kepuasaan
dari pelayanan (Purwanto, 2010). James J. Zeboga menemukan bahwa kepercayaan
dan kepuasaan konsumen berpengaruh pada keinginan membeli ulang.
Rumah sakit swasta dipandang sebagai perusahaan yang memerlukan
keuntungan kompetitif.keuntungan kompetitif terjadi melalui kemampuan untuk
menyediakan nilai pelayanan yang baik, kemampuan untuk memuaskan pasien
dibanding kompetitornya (Pirwanto, 2010).

Orientasi konsumen membuat perusahaan menghasilkan nilai yang superior bagi
konsumennya karena kebutuhan mereka dimengerti (Narver and Slater, 1990).
Pengetahuan ketrampilan yang dimiliki karyawan adalah hal yang penting dalam
penerimaan dan seleksi karyawan (Bitner et al., 1990). Tidak dapat diasumsikan bahwa
penyedia layanan secara otomatis mengemangkan perilaku berorientasi konsumen.
Pertama, mereka membutuhkan sikap mental untuk memperkakukan konsumen dengan
empati yang tulus, perhatian, respek, kesopanan dan
Keramahan (Dubinsky, 1994). Kedua, usaha terpadu dari manajemen. Hal ini meliputi
sistem kepercayaan bahwa konsumen adalah yang paling utama, manajemen yang
mepraktekkan kepedulian dan empati, pelatihan untuk karyawan dan monitoring lingkungan
pelayanan (Dubinsky, 1994).
Orientasi konsumen dapat dinilai dari dua tingkat yaitu dari organisasi dan dari
karyawan. Pada tingkat individu, orientasi konsumen merujuk pada kecendrungan individu
atau predisposisi untuk memenuhi kebutuhan konsumen (Brown et al., 2002). Memiliki dua
dimensi yaitu dimensi kebutuhan sebagai contoh apakah karyawan memecahkan masalah
yang dimiliki dan dimensi kesenangan sebagai contoh karyawan yang mudah tersenyum
pada konsumen (Saxe and Weitz, 1982; Brown et al., 2002). Brown et al. (2002)menunjukkan bahwa orientasi konsumen mediates the relationships between more basic
personality traits and service performance. Brown et al.'s (2002).
Semakin positif asosiasi stakeholder dengan merek perusahaan, konsumen akan semakin
puas. Asosiasi ini dibangun melalui pengalaman dengan merek, dan dalam pelayanan
maksud di sini adalah karyawan; proses yang bergantung bagaimana karyawan berorientasi
terhadap konsumen. Orientasi konsumen secara positif mempengaruhi kepribadian merek
perusahaan, dan memiliki pengaruh positif terhadap kepuasaan (Whelan et al, 2009).
A brand is a symbol that encapsulates the associations made with a name (Gardner
and Levy, 1955). Brand image concerns the remembered associations held of a brand;
while brand personality is the human associations made with a brand (Aaker, 1997;
Davies, 2008; Keller, 1998). Corporate brand personality is a form of brand personalityspecific to a corporate brand, and reflects the values and actions of the corporation and
business (Keller and Richey, 2006).
A significant relationship exists between the consumer-perceived corporate brand
and consumer attitude and loyalty. Corporate brand personality is one of the most
critical and consistent predictors of both attitudinal and behavioral loyalty to that
brand (Anisimova, 2007). Preserving and reinforcing the essence of brand personality
can ensure the long-term viability of a brands equity (van Rekom et al., 2006).For example, Sung and Yang (2008) investigated corporate brand personality in a
university context. They demonstrated that corporate brand personality is positively
associated with outwardly perceived prestige and reputation, and has a positive
influence on supportive customer attitudes. Davies (2008) argued that corporate
branding and personality also impact internal stakeholders. He found, for example,
that an agreeable corporate personality predicts employee satisfaction; the combination
of perceived expertise and attractive corporate personality would enhance employeeperception; of both differentiation and loyalty.

In the face of increased competition, independent stores need (Perepelkin dan Zhang, 2011)
Terdapat banyak definisi mengenai kepuasan pelanggan,
menurut Oliver (1993) menyatakan bahwa kepuasan secara
keseluruhan ditentukan oleh ketidaksesuaian harapan yang
merupakan perbandingan antara kinerja yang dirasakan denganharapan yang ada di benak pelanggan. Spreng, Mac Kenzie,
Olshavsky (1996) mengemukakan bahwa kepuasan pelanggan akan
terjadi apabila kualitas kinerja yang dirasakan atas suatu produk
atau layanan sama dengan atau bahkan melebihi harapan dari
pelanggan. Lebih lanjut Tse dan Wilton (1988) menyatakan bahwa
kepuasan pelanggan adalah respon pelanggan terhadap evaluasi
yang dirasakan antara harapan sebelumnya dan kinerja aktual yang
dirasakan.
Woodside et al (1989) mendefinisikan kepuasan pelanggan
sebagai suatu bentuk khusus dari sikap konsumen yang merupakan
fenomena setelah konsumen tersebut melakukan pembelian yangmencerminkan sejauhmana seorang pelanggan menyukai atau tidak
menyukai pelayanan yang diberikan. Kepuasan konsumen
merupakan perasaan senang atau kecewa yang dihasilkan dari
persepsi kinerja produk atau jasa terhadap harapan yang mereka
miliki. Kepuasan pelanggan selalu melekat atau menjadi
karakteristik produk atau jasa yang sesuai dengan kebutuhan dan
harapan pelanggan, harapan tersebut antara lain tentang harga,
biaya, kenyamanan, kemudahan, keramahan, pelayanan yang
bermanfaat dan lain sebagainya. Kepuasan pelanggan merupakan
evaluasi purna beli di mana alternatif yang dipilih sekurangkurangnyamemberikan hasil yang sama atau melampaui harapan
pelanggan, sedangkan ketidakpuasan timbul apabila hasil yang
diperoleh tidak memenuhi harapan pelanggan.
Reputasi Perusahaan
Menurut Zeithml (1988) kualitas jasa yang dirasakan dari
suatu produk atau jasa erat hubungannya dengan reputasi yang
diasosiasikan dengan nama merek. Artinya pelanggan hanya akan

Corporate reputation and its componentsCorporate reputation is a many-sided component and according to Fombrun (1998),

these components are the history of human experiences. The constitution of this
history is related with the actions and activities the corporate perform in a specific time
and from this point of view reputation is defined as a concept that is based on
perception and focus on what is done and how something is done in corporations
(Bennett and Kottasz, 2000). According to Herbig and Milewicz (1993), corporate
reputation is trust that the corporate creates by keeping its promises in a decided
manner. Because trust, a result of corporate past experiences and an evaluation of itsactions as true-false, is a subjective and total evaluation in shareholders minds about
their expectations (Fombrun and VanRiel, 1997). To understand this process better, itis important that the relation between corporate reputation concept and trust should be
analyzed, because these evaluations accumulate in time and contribute to constitution
of reputation. That is, the trustworthy of a corporation is seen as the trustworthy of its
stated aims (Nquyen and Gaston, 2001). This created trustworthy constitutes customer
satisfaction together with consistency, permanency, sustainability and entirety in the
basis of reputation management. For this reason, the primary aim is to develop andstrengthen the quality of services and products. Because every shareholder, related
with the corporation, and all employers in the corporation, have an impression about
the corporates products and services and if this impression is positive, they have high
trustworthy impression (Erguden, 2003). According to Caruana (1997), in addition to
being consistent in every action, positive communication is also important. In order toconstitute reputation, it is important to use corporate communication means and mediaefficiently and consistently, to explain whats and whys properly about each activity, to
associate it with corporate vision, mission and values. This handles corporate
reputation as corporate activities, quality of service, the degree of its performance and
fulfilling its social responsibilities, the synthesis of corporations expectations related (Satir, 2006)
A corporate reputation is a stakeholder's overall evaluation of a company over time. This evaluation is
based on the stakeholder's direct experiences with the company, any other form of communication and
symbolism that provides information about the firm's actions and/or a comparison with the actions of other
leading rivals (Gotsi dan Wilson, 2001).

are self-disciplined, reliable, dutiful and tend to comply with organizations rules (Costa and McCrae, 1992), it can beexpected that they might be more committed to organizational goals. Meyeret al. (1993) proposed a three-componentconception of organizational commitment, namely:
(1) affective commitment which occurs when the employee wishes to remain with the organization becauseof an emotional attachment;
(2) continuance commitment exists when a person must remain with the organization because he or she
needs the benefits and salary or cannot find another job; and
normative commitment which derives from the values of the employee
Organizational commitment in this study represents the first category, i.e. affective attachment of an
employee to the organization. It comprises of a strong belief in, and acceptance of, organizational goals and
values; a
willingness to exert considerable and sustained personal effort on behalf of the organization; and a strong desireon the part of the employee to maintain continued membership of the organization (Mowday et al., 1979). It isan employees affective or emotional attachment to an organization (Allen and Meyer, 1990; Gifford, 2000).
Individuals scoring high on agreeableness are willing to help others (Costa and McCrae, 1992). In an employmentsituation, these individuals are willing to help their co-workers and organization by going the extra mile when
needed and are more emotionally committed to their work (Naquin and Holton, 2002). Furthermore, agreeableness
indicates how well a person gets along with those around him, thus it can be expected that agreeable individualswould derive more positive feelings from work experience than persons not so agreeable (Organ, 1994). Moreover,
research indicated that negative affectivity correlates with OCB (Organ and Lingl, 1995). Since individuals scoring
high on neuroticism (the opposite of emotional stability) are vulnerable to stress, prone to feeling inferior, self-conscious and uncomfortable around others (Costa and McCrae, 1992), it can be expected that emotionally stable
persons possess positive feelings towards their customers (patients) an important characteristic in an emotionally
charged and anxious situation such as a hospital stay. Hence, we hypothesize:
Organizational factors and COBScholars have defined organizational service climate as employees perceptions of events, practices and procedures;as well as their perceptions of the behaviors that are expected, rewarded and supported (Lytle et al., 1998; Schneider,1990; Schneider and Bowen, 1995). This concept is similar to organizational service orientation employed by
Lytle et al. (1998) which was defined as an organization-wide embracement of a set of
relatively enduring organizational policies, practices and procedures intended to
support and reward service-giving behaviors by employees that create and deliverservice excellence. Recent empirical studies show a strong support for the link between employee perceptions of their service climate and delivery of service excellence, evenamongst lowly paid employees of convenience stores (Bendapudi and Bendapudi, 2005). Put simply and consistent with Heskett et al. (1997) service profit chain
framework, a supportive service climate results in happy and committed employees,
and happy employees tend to deliver higher quality service to customers (Loveman,1998; Schneideret al., 2005).
It is generally agreed that an organization should possess two major ingredients forIt is generally agreed that an organization should possess two major ingredients for
service personnel to deliver excellent service: a genuine concern for customers (i.e. service climateall practices, and policies, should be customer-oriented), and concern for employees (how the employees are treated and supported by their management).When employees feel their organization has a climate that is not customer-oriented,
both employees and customers report less positive experiences and less commitment(Schneider and Bowen, 1995, 1993). This is especially true for high-contact services(such as healthcare) of an affective and intimate nature where a high degree offace-to-face interaction between customers and employees is necessary for the serviceto be manufactured and delivered. As noted by Hausman (2004) distinct, almost
unique, differences exist in a medical context, such as:one-to-one interactions of sometimes a highly emotive nature; intimate exchanges; and
frequent encounters with the same nurse.

Such a context is demanding and stressful for employees, and thus a supportive service climate is essential. Inother words, employees (nurses) are more likely to deliver excellent service to patients when the organizationalexpects and rewards such behavior and establishes practices that both support and facilitate (e.g. training,
providing appropriate technology) service delivery. Hence, the next two hypotheses are:
H5. Employee perceptions of service climate will have a positive relationship withCOS.
H6. Employee perceptions of service climate will have a positive relationship withCOB.As noted earlier we define organizational commitment as employees affective or emotional attachment to an
organization (Allen and Meyer, 1990; Gifford, 2000). Recent work has clearly linked service climate to employee
commitment to the organization. Employee perceptions of service climate determine how individuals behave in
influencing how they think and feel about their task environment (Salanick and Pfeffer, 1978). For example,
Schneideret al. (2005) found a positive service climate encourages employees to engage in extra-role behaviors OCBthat benefit the organization and customers. As noted earlier, a medical care context is both demanding and stressfulfor front-line employees, but especially nurses (Australian Government Report, 2005). With a positive and supportive
climate for service, nurses are better able to cope withthe daily stress of their job and hold more positive attitudes, such as commitment toward the organization
(Scanduoci, 1997). Moreover, tenets of social exchange theory suggest that when nurses perceive that their
hospital and supervisors are truly service oriented, they are likely to be motivated to become more committed
towards the hospitals service-related goals:H7. Employee perceptions of service climate will have a positive relationship withtheir organizational
commitment.
Some studies have attempted to apply the concept of organizational commitment to non-western countries (Chen and
Francesco, 2000; Tao et al., 1998) with mixed success. However, it might be expected that employees in highlycollectivist, relationship rich South-East Asian cultures such as Thailand, China, Vietnam or Indonesia for example,might exhibit higher levels of affective commitment than employees in individualistic cultures (Randall, 1993).Organizational commitment among Chinese employees for example, has been found to have a much stronger effect on
job satisfaction and turnover intention than comparative results from studies conducted in the West (Wong et al.,2000). In summary, the evidence supports the view that when employees (but especially in collectivist cultures) areaffectively committed they will put in more effort to achieve organizational goals. This effort should be reflected interms of employees attempts to satisfy customers, i.e. in their COB. Moreover, since surface traits emanate from the
interaction between self and situation, it is likely that organizational commitment will affect COS. Thus, it is
hypothesized:

Customer orientationBrown et al. (2002) suggest that customer orientation, an individual-level construct, is the key to a service organizations
ability to be market oriented. Customer orientation has been defined as the degree to which contact personnel practice the
modern marketing concept by trying to help their customers make purchase decisions that will satisfy customer needs (Saxe
and Weitz, 1982). Customer orientation is believed to be foster a number of positive marketing outcomes. Generally,
research has found customer orientation to be related positively to employee performance (e.g. Boles et al., 2001; Brown et
al., 2002; Swenson and Herche, 1994). Other research has supported a link between customer orientation and customer
satisfaction, both at the firm level (Goffet al., 1997; Humphreys and Williams, 1996) and at the individual level (Goff et
al., 1997). Most recently, in a major review of the customer orientation literature, Schwepker (2003) cites several studies
that suggest, but do not empirically test, a linkage between customer orientation and customer trust
