My frustrationovermy firstyearofpresidencywasthemisquotationofthepassingrateofourexaminationsbyotherswhohadlittleknowledgeof thisCollege. I amevenmore surprised toreadanarticlebyanotherassociation recentlyregardingourlowpassingrate.Ourintermediateexamination-The HKCFP/ RACGP ConjointExamination first commenced in 1987. In theeighties,theaveragepassingratewas66.7%.Theaveragepassingrate in thepast5years (2011-2015)was64.2%.Thepassingratein2015was79.4%.TheExitExaminationleadingtoFellowshipofHKAMhasanaveragepassingrateof77%inthepast5years(2011-2015) includingfullandsupplementaryexaminations.This iscomparabletooursisterColleges inHongKongandotherGPCollegesaroundtheworld.OurexaminationsarerobustandmeticulousandarewellmonitoredbyRACGPandexternalexaminers.Nowwehavemoreexperiencedexaminersandclinicalsupervisors,butthemostimportantofall iswehavebetterandhardworkingtrainees.Allcreditshouldgotothis futuregenerationofwell-trainedFamilyPhysicians. ImustalsocongratulateDr.ChanHungChiuandDr.WendyTsuiinheadingtheBoardofExaminationandSpecialtyBoardrespectively.Thesweatandbloodoftheirteamshavenotgoneunnoticed.

Atthisverymoment,thereare444FellowsinFamilyMedicineatHKAM.ThenumberoftrainingpostinFamilyMedicineisnotdeterminedbytheCollegeorthepassingrateinprofessionalexaminations.ItisdeterminedbythebiggestemployerinHongKong-theHospitalAuthority!Anynon-specialistwith5yearsofGPexperiencecansittheConjointExaminationasCategory IIcandidates.Thebestcandidate in2013was fromthiscategory.From2015onwards,thosewhoholdintermediatequalificationcanundergofurthertraininginthealternativepathwayleadingtoExitExamination.TheCollegehasneverembracedAcademyFellowshipasthesolestandardforFamilyPhysicianstopractise inHongKong.Theprofession,thesocietyandthegovernmenthavetodecidethestandardandtrainingtheywantfor independentandcapablefamilydoctors inHongKong.

Ifyouhavereadmymessagelastmonth,Imentionedqualityassuranceofprimarycaredoctorsisofprimeimportance.Poorqualityprimarycaredoctorscandomoreharmthangood.AnimportantqualityassuranceofPrimaryCareisContinuingMedicalEducation(CME)forallprimarycaredoctorsandHKCFPcanhelptoassureCMEprogrammeisrelevanttoPrimaryCare.

AccordingtotheMedicalCouncilofHongKong,thereare2typesofregistrationforthefullyregistereddoctors,i.e.ResidentlistandSpecialistlist.Just lastweekendDr.KoWingMan,SecretaryforFoodandHealthBureauspoketothemediaregardingtheconcernofdoctorsonCMElinkedtoregistration.His latestconcept is fornon-specialist,CME isnot linkedto registration.However,hesuggests throughdisciplinarymechanismoftheMedicalCouncile.g.underthecodeofconducttorequirenon-specialisttohaveCME.ThisisamajorstepforwardandmyCouncilislookingforwardtofurtherdiscussionwiththeBureau.

Bynow Ihavevisitedall thesevenclustersand their trainees. IamdelightedDr.DavidChaohasaccompaniedmeinmostofthevisits.WehavedetailedtheworkoftheCollegeandwelovetoseethempassingtheirexaminationswithflyingcolours.

Finally, Ihave learnt fromDr.MarkMiller,Censor inChief,RACGPthegreetingfortheYearofMonkey.Iwishyouaheadbyasomersaultof9,000kilometres,72simultaneoustransformationsinscholarlinessand81kindsofcreativeskills.

Certificate Course in Family Medicine (HKCFP) 2016-17The Board is pleased to announce that the Certificate Course in Family Medicine (CFM) organized by The Hong Kong College of Family Physicians will commence in July 2016.The course consists of THREE Segments. Segment I and Segment II will be delivered by Local Distance Learning. Segment III consists of workshops. The whole course requires a HALF year of part-time studies.Details of the course are as follows:

1. Objectives:i) To provide knowledgeable, pragmatic and structured teaching in Family Medicine for medical practitionersii) To encourage professional development of practicing medical practitioners and to provide an intermediate step to attain diploma qualifications in Family

Medicineiii) To improve standards and quality in the practice of Family Medicine

2. *Syllabus: The course consists of THREE compulsory segments. Doctors who have graduated from the course are expected to have acquired:i) Current concepts about nature of Family Medicine ii) Knowledge and skills in consultationiii) knowledge and skills in some common practice procedures required in family practiceiv) Understanding regarding the role of Family Doctors as gatekeepers of the health-care system and in providing cost-effective primary care to the community

Aims: Acquire in-depth knowledge and practical skills in selected specialized areas including Medicine, Surgery, Geriatrics, ENT, Orthopaedics & Traumatology, Accident & Emergency Medicine, and Infectious Diseases

Contents: THREE Update seminars and ONE clinical attachment on selected specialties including Medicine, Surgery, Geriatrics, ENT, Orthopaedics & Traumatology, Accident & Emergency Medicine, and Infectious Diseases (subject to availability)

Module V - Practical Family Medicine (Practical Workshops)

3. Articulations: The Course allows (up to a fixed maximum percentage of the Course units) articulations or cross recognition of previous Family Medicine training programmes

that provide learning units equivalent to that of the above syllabus. Participants who wish to apply for such articulations have to submit evidence of relevant training together with their application. The granting of articulations is however, completely at the discretion and decision of the Board of DFM.

5. Admission Requirement: Medical Practitioner with Bachelor’s degree in Medicine

Kowloon Tong Club Small Discussion Group – The Past 30+ Years

Throughoutournumerousmeetings,wewerenevershortofobservers.Othercollegememberswhowereabouttostartanewgroupcametoseeksomeguidance,andoutsidedoctorswhowereinterestedindifferentsubjectswouldliketokeepabreastwithupdatedknowledgeanddevelopments.Foroveradecade,weevenhadadentalcolleaguemakinguninterruptedattendanceasanobserver,yearningtobroadenhismedicalhorizon.

30+years isnotashort time.Someofourmembershadalreadyleftusforheaven.Somegrandfathersofthegrouphad leftbecauseofaging,dwindlingstamina,andfailinghealth,andyetsomeothersleftforvariousreasons–differingideologist,timeconstraintsorcommutinginconvenience.Thevacancieswere,however, ingreatdemandandwerefilledupinnotimebycolleaguesonthewaitinglist.Theyoungergenerationbroughtalongwiththeminnovativeideasandafreshapproach.Asocialgroupinthecyberworldwasformedto facilitateconvenientand immediatecommunication,interaction,discussionandevenconsultation.

During this lengthy span of time, many younger smalldiscussiongroupsof theCollegeweredisbandedanddissolvedfordifferentreasons. TheKCTSmallDiscussionGroup,however, isuniqueasitremainsyoungandhealthy,coherentandrobust,strongandactive,monthinmonthout,yearafteryear,andisreadytostrideintoitsnext30+years.

OurCollegewasestablishedin1977.Amongthemultitudeof its major roles, the promotion of CME among itsmembers rankshighon itspriority list.Colleaguesarealwaysencouragedtoparticipateinallkindsofeducationalactivities topursuecontinuingmedical knowledgeandprofessionalexcellence.Aftertheearlyyearsofestablishingitsfooting,theCollegedecidedtoimplementtheconceptofSmallDiscussionGroups.Memberswereguidedtoformsmallgroupsfor informaldiscussiononpracticalproblemsencounteredineverydaypractice,therebyimprovingtheirmanagementskillsandapproachtodifficultchallenges.

Heeding these recommendations, Dr. Stephen Foorespondedandstartedhisendeavorsbyinvitinghisfriendsandcolleagues,togetherwithotherCollegememberswhowerealsomembersofKowloonTongClub(KTC),toformthefirstsmallgroupof itskind,theKowloonTongClubSmallDiscussionGroup(theGroup).

InitiallytherewerenostrictguidelinesastohowtheGroupshouldfunction.MembersjustgatheredforcasuallunchataconvenienttimeatKTCforelaborationondifferenttopics.Theywouldpreparethemselvesbeforehandtosharetheirexperienceandopinionswithothers. Itwasonly laterthatdoctorsinotherfieldsandspecialtieswereinvitedtogivetalksandupdatesonothertopicsofinterest.Memberswerefreetointeractandparticipateactivelythroughoutthepresentation.Itsoonbecameobviousthatthis laterapproachwasmoreinformativeandinteresting.

Toensuresmooth running, theGroup thenappointedaChairman,anAssistantandaTreasurer for itsongoingoperation.Itwasequippedwiththepurchaseofaprojectorandascreen.Gatheringsonafixeddayeverymonthwerescheduledandbecauseof itsrelationshipwiththeClub,aspaciousandcomfortableroomwasreservedreadilyforitsregularfunction. WiththestaunchsupportfromthecoregroupofKTCmembers,othermembersalsoenjoyedfriendlyatmosphere,goodcomradeship,warmandusefuldiscussionsinarelaxingenvironment,whilesharingvaluableprofessionalexperiencewiththeirpeersatthesametime.Allappreciatedtheoccasionandlookedforwardtothesemonthlygatherings,andmostwouldchoosetosupporttheGroupratherthanattendingotheracademicfunctionswhenevertherewasaclashofdates.

Many manufacturers do not make the laces longenough anymore to accommodate the varioustypesof lace tying. If the lace is tooshort,donotovercompensatebytyingtheshoelacetootight.Forpeople thathavewider feet, itmaybe required topurchaselongerlaces.

Asyoumightalreadyhavespotted,a lotof runningshoeshaveanextrahole,usually foundbehindandunderneaththelasthole.Seemingly,this looks likeauselessplacetoputahole;however, itsexistence ismeanttotakepressureoffthetopofthefoot.Withtheloop lacing lock technique,whichalsocalleda“runner’stie”,thiscaninsureatightandsnugfitandkeepingashoefromslippingontheheel.Theillustrationsbelowdemonstrateastep-by-stepmethodforthistechnique.

How to predict children’s academic performance? - Simple as a raisin and a cup!

AnewstudyrecentlypublishedintheJournalofPediatricshasfoundthatatestusingaraisinandacupcanaccuratelypredictachild’sfutureacademicperformance.

Thisstudy’smainaimwastodeterminewhetherthereisadose-responseeffectoflowgestationalageatbirthonearly inhibitorycontrol,andwhetherthese inhibitorycontrolabilitiesmediatetheunfavorableeffectsofpretermbirthonfutureattentionandacademicperformances.

558childrenbornat26-41weeksgestationwereincludedinthisprospectivestudy.Amongthem393werebornbetween25and38weeksofgestation,while165bornhealthyfullterm(39-41weeksofgestation)werechosenascontrols.

Toddlers’ inhibitorycontrolabilitieswere thenobservedearlyat theageof20months,with thenewlyintroducedraisingame.Thetaskattentionandacademicabilitieswereevaluatedat8yearsofagewithavarietyofneurological,cognitive,behaviourandparent interviewtests.Allratersandassessorswereblindedtothegestationalageoftheparticipants.

Sowhatisthisraisingameabout?Youcannotimaginehowsimpleitis.Araisinandaplasticopaquecup,thatisallthat’sneeded!Eachparticipatingtoddlerwaspresentedwitharaisinthatwasputunderanopaquecup.After3trainingtrialsduringwhicheatingtheraisinwasallowedaftershortbutincreasingtimeintervals,toddlerswereaskedtowait for60secondsintheactualtestbeforetheycouldtouchthecupandeattheraisin.Thewaitingtimeofthesubjectstilltheyatetheraisinwasmeasuredandcodedinto3categories.(1:didnotwaitorwaitedupto10seconds;2:waitedbetween11and59seconds;3:waitedfor60seconds)

Apartfrominhibitorycontrolabilities,childbehavior,attentionandacademicachievementwereassessedwithstandardizedscalesandtestsattheageof8.

Afterstatisticalanalyses, resultssupport thatpretermbirthnegativelyaffectedchildren’s inhibitorycontrolabilities,withadose-responserelationshipacrossthewholegestationalspectrum.Besides,itdirectlypredictedsubsequent lowattentionregulationandacademicachievement,afteradjustmentforotherfactors.Ahigherabilitytoinhibitunwantedbehaviorsalsopredictedbetterlaterattentionregulationandacademicachievement.Furthermore,adverseeffectsofpretermbirthonlateroutcomeswerefoundtobepartlymediatedbychildren’searlyinhibitorycontrolabilities.

Itiswellknownthatpretermbabiesmayfacevarioushealthproblemssoonafterbirthandduringgrowthanddevelopmentincludingorganfunction,behavioralandpsychomotordevelopment,andcognitivedevelopment.Thisstudyshedslightonthemechanismrelatingpretermbirthtolongtermattentiondifficultiesandunder-paracademicperformance,forwhichpotentialinterventionsmaybedevelopedinthefuture.Italsointroducestheraisingame,whichcanserveasasimpleandeffectivetoolforassessingchildren’sinhibitorycontrol.Thatcouldhelpwithidentifyingthoseatriskofattentionandlearningdifficultiesforearliertreatmentandsubsequently,abetteroutcome.

Learning PointsDr.Chan firstmentionedonevery important issue:psychological and emotional impac t of fac ialdermatoses. Lesions small and inconspicuous indoctors’eyesmaybeeye-catchinganddisfiguringfrom the patient’s point of view. Acne is a goodexample.Being themostcommondermatologicalproblem,acneaffects>85%ofadolescents.Studieshaveshown thatacnecanproducemorenegativeeffects (anxiety,depression,unemployment rates)thanasthmaandepilepsyonpatients’psychosocialfunction. Therefore physicians should thoroughlyexplore patients’ concerns and expectations tooptimizemanagementresults.

Diagnosisof acne isnotdifficultbut treatment isdiversified.Differentmedicationsactagainstdifferentpathophysiologyandsocombinationtherapy(ratherthan monotherapy) is the preferred approach.Topicalantibiotics+ topical retinoid issignificantlybetter than topical antibiotics alone. Clearing ofboth inflammatory lesionsandcomedones is fasterandbetterthanantibioticsalone.Topicalantibioticsmonotherapy is not recommended because ofincreasingantibioticresistance.Oralantibiotics,oralcontraceptivesandoral retinoidsallhavetheirownpros and cons. New treatments for acne includechemicalpeel (GApeel,SApeel), lighttherapy, lasertherapyandphotodynamictherapy.

Rosacea and per ioral dermati t is are commondifferentialsofacne.Rosaceapresentswith redorpinkpatches,visiblebrokenbloodvessels,small redbumps,andredcysts.Ithascharacteristicaggravatingfactors: emotional factors (stress, fear, anxiety,embarrassment, etc.), weather change (winds orhumidity),exercise,alcoholandspicyfood.

Perioraldermatitis is a redand slightly scaly rashthattendstooccuraroundthemouth itmayspreaduparoundthenose,andoccasionallytheeyeswhile

sparing lipmargin.Patientmay feelmild itchingorburning.

Atopicdermatitis (AD) isoneof themostcommonskindisordersininfantsandchildren.

EuropeanTaskForceonAtopicDermatitishas setgoalsofmanagement.

- Short-termcontrolofacutesymptoms

- Long-term stabilisation, f lare prevention andavoidanceofsideeffects

A multifactorial approach targeting skin barrierdysfunction(skincaretocleanseandhydrate, identifyand avoid triggers, break itch-scratch cycle) andimmune dysfunction (anti-inflammatory and anti-infection)isadvised.
