Description of organisation

Cloud2 is a software solutions company and Kinesis is our main product for CCGs. Kinesis is a secure (N3) browser based solution that directly connects referrers e.g. GP’s and other practice based personnel with specialists in provider organisations for expert online advice. It has been proven to save referrals enabling more patients to be treated successfully in the community at significantly decreased costs. NHS England have published a case study on Kinesis featuring Wandsworth CCG where the initial pilot was carried out and where it is now an increasingly used “Business as Usual” system. Some statistics to highlight include a “Saved” referral in 50% of cases where advice is requested and a greater than 75% response from specialists within 24 hours. Users also report that it is very simple and intuitive to use and fits very well with their everyday clinical practice. Governance issues are also well taken care of with secure encryption, storage and traceability of clinical conversations. EXPERT PATIENT PROGRAMME

- EPP should be a programme available to all patients, making use of their aggregated

expertise/experience as helpful but not automatically cohorting them and reinforcing their being

defined by their condition.

-the programme should be prepared to start from where patients are and not presume formal, pre-

planned stages of input. This ‘start’ will therefore be variable.

-there is huge value in the programme being delivered by ‘service users’ themselves. This is where

HAPIA is unique. We are already Experts by Experience and as genuine service users have a proven,

established, shared and strong vested interest in understanding conditions, can readily get along side

patients, act as a ready catalyst and demonstrate the mechanism of self- help and self empowerment

that the programme is there to develop. Such a model is free from patronage. External agencies or

agencies of the NHS as deliverers of a programme rarely have real authenticity. They may talk the

talk but they do not live the life.

-silos are traditionally part of the ‘problem’ therefore EPP should be independent and overarching.

Many patients have co-morbidities. Patients from ostensibly discrete pathways of care may have

common expertise – technical, administrative, presentational, experiential etc that they can re-invest

into the wider programme, creating more capacity, being self fuelling.

- the ethos of EPP is to empower. Power cannot be taught, but it can be experienced and once it is

experienced, it generates a momentum, born of confidence, conviction and a sense of achievement.

Harrogate and District NHS Foundation Trust Provider Information Harrogate and District NHS Foundation Trust (HDFT) is a high performing organisation which provides high quality integrated Hospital and Community Services. The Trust is the principal provider of hospital services to the population of Harrogate and Rural District and North and West Leeds. This represents a catchment population of approximately 200,000 which is still increasing in relation to North and West Leeds. The organisation also serves a wider population, including Harrogate and Rural District, of approximately 600,000 across North Yorkshire as it provides a range of specialist Community Services.

The hospital has an Emergency Department, extensive Outpatient facilities, Intensive Therapy Unit (ITU), Coronary Care Unit, Cardiac Catheterisation Laboratory plus five main Theatres, a 20 place Day Surgery Unit with three further Theatres and an Emergency Obstetric Theatre located within the Maternity Unit. There is also a dedicated Children’s Ward with Paediatric HDU facilities. The Sir Robert Ogden Macmillan centre which provides facilities for the treatment of patients with cancer. Dedicated purpose built facilities are also provided on site for Radiology, Pharmacy, Pathology, Endoscopy and Therapy Services.

The Trust provides a number of Community Services to the Harrogate locality and also across North Yorkshire. The services provided to the locality include Older People and Vulnerable Adults services, whilst Specialist Community Services, Children and Family Services, Minor Injury Units, GP Out of Hours services, (excluding the Scarborough locality), Smoking Cessation Services, Community Equipment and Wheelchairs stores, Salaried Dental Services, Safeguarding Children Services, Community Podiatry Services and Infection Prevention and Control/TB Liaison Services, are pan North Yorkshire services. The strategic aims of the organisation are to:-

Healthcare at Home

Healthcare at Home’s brand is synonymous with the provision of high quality home-based healthcare programmes tailored to customer and local clinician requirements. The company’s primary business focuses upon the provision of innovative clinical service models provided at a local level based. Services provided cover:

- Specialised clinical care programmes including recovery at home, admission avoidance and virtual ward services

By way of illustration HaH has run its Recovery at Home services since 2009. These support patients in their early discharge from hospital, with a view to reducing patients’ length of stay and completing the acute pathway in the home, allowing hospital clinical teams to increase their throughput of patients within the existing bed stock, and offer a patient-centred service. Patients are accepted for a wide range of conditions requiring, for example, post-surgery care for orthopaedic and spinal patients, intravenous antibiotics, complex wound care, vac dressings, catheter care, and PICC line insertion. A good example of HaH service delivery is the virtual ward service commissioned by Good Hope Hospital, Birmingham in 2012. This enables patients who are clinically stable and meet the required criteria to be transferred to the virtual ward and complete the remainder of their hospital care in the comfort of their own home. Patients remain under the care of their hospital Consultant while on the virtual ward and receive appropriate treatment from HaH’s dedicated local multi-disciplinary service delivery team which comprises nurses, physiotherapists, healthcare support workers/therapy assistants and an occupational therapist. The Virtual Ward service creates additional capacity within Good Hope Hospital and complements local health and social care and community providers by working in partnership to support the safe transition of patients out of the acute hospital setting.

HaH operates in both the UK public and private sector healthcare markets and delivers services in each of the four home countries. Its customers include the NHS, pharmaceutical companies, private medical insurers, charities and self-funding patients. HaH’s Head Office is located in Burton of Trent, Staffordshire. Around half of the company’s staff are highly skilled and experienced clinical staff. Currently, it has around 150,000 patients on service, makes 4,000 medicines home deliveries to patients every day and processes over 1,000 prescriptions on a daily basis.

Background

The Hibernian Healthcare group was founded in 2000 in Dublin and offers medical services to

individual patients and healthcare organisations. Currently providing a range of patient support and

medication services supported by an industry leading, in-house developed, IT platform which allows

access by multiple healthcare providers including GPs, Hospitals and community nurses.

Independent Prescribing Pharmacist by background with strong clinical experience. He is backed up

by a strong Head Office team and a robust clinical governance framework.

Services

Patient Support and Education

We offer visits at home to patients who self-administer medication. We also provide Telephone

Support packages backed up by mail shots to patients. Visits can be arranged at evenings or

weekends to suit patients.

Adherence & Compliance Programmes

Our IT supported programmes provide multiple touch points with patients. We can provide nurse

visits, telephone back up and support, mail shots, text messaging, and smart phone apps to meet

specific needs. Data on adherence and compliance can be tailored as required.

Follow Up services - additional flexible, person-centred support for people over 12 months (and beyond if required) following attendance at a Living Well course. Ambition RDaSH is seeking to bid as a lead provider, particularly for Care Group 3; Integrated Community Care Service. Overview Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) is an established Foundation Trust (since 2007), employing approximately 4,000 staff. We successfully operate a wide range of community healthcare, mental health, learning disabilities and drug and alcohol services across Rotherham, Doncaster, North and North-East Lincolnshire and Manchester. Experience We would utilise our experience of delivering integrated health and social care services across the range of our service provision and geography. The most relevant of these services is our Adult Community Healthcare Services in Doncaster, which has been delivered in an integrated model with social care since 2011. Our goals in Adult Community Healthcare Services are to:

- Work with our patients on prevention and wellbeing and support those with a long term conditions to self-manage;

- Deliver high quality outcomes for our patients by working across the whole system to prevent unnecessary admissions to the acute setting and facilitate speedy discharge;

We have introduced numerous new services to improve the patient experience, such as one-stop clinics that provide rapid access to diagnosis and treatment e.g. breast, bowel and certain orthopaedic specialty clinics supported by specialist nurses. Royal Voluntary Service

Royal Voluntary Service (formerly WRVS) is a charitable organisation which is in the forefront of

providing services which allow older people to live the lives they want to enable them to remain a

vibrant and connected member of their local community. We provide a range of services across

England, Scotland and Wales based on the commitment and hard work of over 35,000 volunteers.

Our range of services includes a range of Good Neighbours-type services (including befriending,

transport, escorting to appointments, shopping etc.), Home from Hospital support, On-ward

Support, delivery of hot meals, and lunch and activity clubs. We also operate a range of retail outlets

in hospital settings, using any profits to develop more wide-ranging services for older people. The

key driver for our services is what older people themselves tell us will make their lives better.

There is already a significant element of carer support within our current services. We operate a

range of befriending services and in many instances our volunteers are spending time with older

people so that a carer can be offered some respite. In some instances this is to offer respite to carers

of people with dementia.

As an organisation we are looking to develop more of a role in developing telecare services. We are

aware that older people are increasingly being asked to carry out health checks remotely and send in

information to health care professionals. We see supporting older people in carrying out these

checks as a natural development of our befriending services; ensuring that health checks are done

and appropriately reported can form part of a befriending visit. The majority of the befriending

services we deliver are in person, however we do have four telephone befriending services that we

deliver, and teleservices is an area we are interested in expanding.

Falls prevention already constitutes a significant element of our work in supporting older people,

whether informally (many of our volunteers are trained to carry out self and well checks when

visiting older people in their homes including identifying trip and fall hazards) or as part of a formal

service. As an example, in the London Borough of Hackney we deliver a Falls Prevention Service. This

is a specific scheme to prevent falls amongst older people and to reduce falls-related admissions to

hospital. It increases confidence and independence amongst those who have left hospital or who

are at risk of falls, and signposts them to the relevant service providers to ensure that the risk of

being hospitalised or re-admitted is minimised.

Volunteers trained in falls prevention visit the service user at their home, and support further with

weekly visits or telephone calls. The role of the volunteers is to ensure that the service user is aware

of the falls prevention services already available in Hackney, including the Council's telecare service.

In addition to our falls prevention work in London we also provide the Slipper Exchange project. This

addresses the fact that many older peoples’ slippers are badly worn and a contributor to falls which

result in unnecessary GP visits and hospital admissions. The project holds information events across

London in Royal Voluntary Service afternoon clubs as well as other locations where older people

come together. We explain about potential sources of falls at home and encourage people to throw

out their old slippers whilst providing them with a new pair.

Our volunteers are also aware of how issues like use of medication, and alcohol can be significant

factors in contributing to falls amongst older people.

Our urgent care centres allow patients to be seen without an appointment and, like our GP practices, have opening hours designed around the service user. Seeing and treating over 700,000 people each year Virgin Care is the second largest provider of this type of urgent care in England.

Alzheimer's Society is the UK's leading support and research charity for people with

dementia, their families and carers. The Society provides information and support to people

with any form of dementia and their carers through our publications, National Dementia

Helpline, website and more than 2,900 local services throughout England, Wales and

Northern Ireland.

Our dementia support services in Hull include:

Information, advice and signposting for anyone affected by dementia, to support and

guide people through the diagnosis period and beyond.

Dementia Cafes where people with dementia and carers can access peer support,

social interaction and support from our trained Dementia Support staff and

volunteers.

Singing for the Brain, an innovative service combining the proven therapeutic effects

of music and singing with peer support.

We support health and social care professionals by delivering high quality education,

resources and training. We influence politicians and policy-makers, and campaign for better

quality of life for people living with dementia and their carers and greater understanding of

dementia.

We also fund innovative research in the areas of cause, cure, care and prevention. We work

with scientists and people affected by dementia to ensure that the medical and social

research programmes we fund have a positive impact on people's lives.

We were a founder member of the National Dementia Action Alliance which launched the

national Dementia Declaration for England, and recently launched the Dementia Friends

campaign which aims to recruit one million Dementia Friends, who will commit to take action

The Butterflies Memory Loss Support Group is currently a voluntary organisation, soon to be a registered charity

which offers a wide variety of workshops for people fighting dementia and their carers. Our HQ is in the city centre

and accessible by public transport for people living in all areas of Hull and we have a base in East Hull.

The organisation was founded in 2010 with participants of a RemCare research project facilitated by Professor Esme

Moniz Cook which my parents took part in. I was then the carer representative on the King’s Fund development

project which worked on the recent environmental developments at The Hull Memory Clinic.

We hold a number of workshops and luncheon clubs throughout the month offering a social outlet to those who

become isolated by dementia, with a wide range of activities with a stimulating and therapeutic value, which

importantly bring fun back into the lives of our members. We pay special attention to delivering activities which are

person centred, dignified and concentrate on the skills which remain in each individual. We are not about bingo and

dominoes! Our sessions include glass engraving, wood work, candle making, music, crafts creative writing, life story

work etc in fact anything out clients show an interest in, we try to facilitate. In addition we also hold a number of

support forums for carers whilst offering an activity group for the person they are caring for.

We also work with a number of schools and youth clubs on cross generational projects aimed at providing social

events and raising awareness in dementia in the young people and breaking down barriers across the generations.

We currently have three members of paid staff and a team of excellent volunteers. All of our volunteers attend the

Dementia Academy’s Dementia Ambassador training in addition to our own in house induction training. We

encourage volunteers to complete the NCFE Certificate in the Principles of Dementia Care. This is regardless of the

role they take.

I was recently awarded by Hull and East Riding Women in Business, the judge’s award for Outstanding Contribution

in the Field of Dementia and achieved runner up in the Woman of Achievement for the voluntary sector.

The whole team take great pride in the person centred, dignified friendly service we provide and look forward to

developing and expanding to offer additional groups to other areas of the city.

Whilst we have been successful in the past in securing funding through subs, volunteers and fundraising, including

grants from Comic Relief, Community First and Smile Foundation, our aim is to secure sustainable funding from all

available sources including contracting with statutory bodies.

We have an annual turnover of approx. £6.6million. Employ 181 contracted staff and currently have 180 volunteers across the organisation. We work mainly in Hull with some focused delivery in the West, particularly within children and young people’s services.

HERIB (Hull and East Riding Institute for the Blind) is a registered charity which aims to improve and enrich the quality of life for people with sight loss in the Hull and East Riding area. Originally established in 1864, the charity now provides a range of both support practical and emotional support services as part of the rehabilitation process for almost 2,300 local people who are blind or partially sighted and living with long term sight loss conditions. It is this experience which means HERIB is recognised as the local expert in sight loss support. HERIB acts as a one stop point of contact for all advice/information about sight loss to the region and provides a wide range of support services which include: Home Visiting Scheme - HERIB’s home visiting service provides specialist advice and information for those people who can't get access our centre or would prefer to talk with someone in the privacy of their own home. Day Groups - HERIB’s Day Groups offer door-to-door transport from home and lunch followed by a variety of activities such as arts and crafts, quizzes and musical entertainment. Resource Centre - HERIB’s Resource Centre is open Monday to Friday from 9.00am to 4.00pm where staff can offer advice on a wide range of equipment such as magnifiers, talking clocks and many other daily living aids. Mobile Resource Service - Our Mobile Resource Bus regularly “drops in” at a variety of venues across the region to offer advice and assistance within the community. Sheltered Accommodation - Beech Holme Court is a sheltered housing development for visually impaired people containing twenty-two flats where tenants are encouraged to remain independent with support from HERIB. Clubs and Groups - The Beech Holme Tandem Club is based at HERIB and our visually impaired sports group holds regularly yoga classes at the centre whilst our SocialEYES group arranges outings and other activities. HERIB also hosts regular meetings by the Macular Society and local Glaucoma Support Group. WORKSHOPS & INFORMATION SESSIONS - Our informal workshops and information sessions aim to help people cope with the practical difficulties of living every day life with sight loss and are particularly useful for people who have been newly diagnosed with sight loss as well as anyone living with sight loss and their family members and/or carers. Independent Living Skills Workshops - HERIB offers a range of workshops to help anyone who is visually impaired learn specific skills which they may find useful including: Microwave cookery; Braille reading and writing; IT & technology including computers, smartphones and the use of specialist software for the visually impaired. Other Events/Activities include: Holidays; Exhibitions; Braille transcription service All services are aimed at reducing isolation, mental health issues, loneliness, anxiety, depression and increasing social interaction with a view to helping local people with sight loss live independently in the way they choose. We have operated for over 38 years mainly in Hull and Outreach to 7 localities in

East Yorkshire, a population of over 600,00, but have delivered time limited contracts

in North East Lincolnshire, West Yorkshire and training throughout England. We

have staff 48 (38 WTE), All staff receive mandatory training;

Health and Safety; Personal Safety; Mental Health First Aid; Safeguarding Adults &

Children; Equality and Diversity; Data Protection and Information Governance

Role specific requires at least NVQ level 2/3 in; health and social care, psychology,

education/training, information, advice and guidance, additionally staff are trained in

cCBT, or similar therapies as well as PTTLS. For a few roles we require Social Work

or Counsellor registration to provide support to PLO1 and PLO2 student placements

with us. Managers additionally require NVQ level 5 in leadership and Management.

We support CPD by all staff.

Hull CVS Information

NHS Hull CCG Community Services Procurement Provider Networking Event Hull CVS is a registered charity that was established in 1981 as a voluntary sector infrastructure support and development organisation, providing advice, development support, information, networking, strategic partnership work and a range of services to voluntary sector organisations and organisations using volunteers. As well as supporting the development of the local voluntary sector, we also specialise in directly delivering projects and services that are of benefit to local communities. Our current services are as follows: Volunteer Centre We are the accredited Volunteer Centre for Hull and in addition we hold the ‘Investing in Volunteers Award’. We support individuals by matching them to placements that meet their needs, as well as offering mentoring and training sessions to volunteers to prepare and support them in their volunteering roles. We have also developed strong links with local organisations, helping them to recruit volunteers. We conduct organisational health checks to make sure that they have all the necessary insurances, policies and procedures in place, and we operate a full package of training and support for organisations utilising volunteers. Volunteer Support Project This is a 5 year programme which aims to make volunteering more accessible to people who have extra support needs. This could be people who have mental health problems, those recovering from drug or alcohol misuse, people with physical and learning disabilities, ex-offenders, or people who come from an ethnic minority or whose first language is not English. Healthwatch Hull We deliver Healthwatch Hull, which was formulated from the Health and Social Care Act 2012 to give people a say in the commissioning and delivery of local health and social care services to ensure that services are tailored to meet local needs. Young Adult Carers Hull CVS delivers a service that extends the support available to young carers past the age of 16. Our service supports those who continue in their carer role past this age, supporting individuals who may face new challenges in their transition to adulthood. Direct Payments Hull CVS provides a direct payments payroll facility for people in receipt of the Individual Living Grant. Our dedicated payroll team takes the stress and worry away from individuals employing personal assistants, by processing the payroll for them and also ensuring that advice and support is provided efficiently and in line with HMRC requirements. Meeting New Horizons CIC Through our trading arm, we extend the delivery of services that are of benefit to the community to the wider Yorkshire & Humber region. Services delivered by this part of our Group include Healthwatch in the East Riding and North Lincolnshire, a Family Support service in North Lincolnshire, and a Macmillan Community Development and Support project in Doncaster. This project supports cancer survivors throughout their journey from diagnosis onwards. The service offers advice to people within their own homes, including supporting them to claim benefits they are entitled to and signposting to other organisations that offer support. The service also works with such support organisations, building their capacity to support people affected by cancer.

KIDS is a national charity providing a range of services for disabled children, young people and their families. Services currently available within Hull are: Designated Keyworker Service –The Designated Keyworker Service offers a single point of contact to families accessing more than one service or with multiple medical appointments. The Keyworker's main job is to work with other agencies (health, education, social care and the voluntary sector) to provide advice, information, emotional and practical support to families. SENDIASS (formerly Parent Partnership Service) - provides independent, impartial advice to parents and carers of children with special educational needs and helps them to make informed choices about their child’s education. The service also has a full time worker who can offer information , advice and support to young people in their own right. Independent Support – Independent supporter are available to support parents and young people through the transitional process of moving from a statement of special educational needs or learning disability assessment to an Education, Health and Care Plan. Inclusion Service, behaviour support. This service provides information and advice to parents of children with special educational needs who are at risk of exclusion from school. Holiday Playscheme and Youth Groups – a specialist service for disabled children aged 8-18 years, giving disabled children and young people the opportunity to enjoy a range of activities and outings having fun, whilst their parents benefit from a short break. Early Years – KIDS run a number of targeted play development sessions within Children’s Centres across the city for young children who are struggling to reach their developmental milestones. In addition to providing individualised learning programmes for children, the sessions are a great way for parents to become involved in their child’s learning and gain the support from other parents attending. Specialist Advocacy Service – this service supports disabled children and young people who require an advocate. The service is independent and is specifically for children and young people accessing short breaks, looked-after-children and those placed within independent settings. We can work with children from the age of eight up until the young person’s 19th birthday. Parent Participation – KIDS facilitates the parents forum, a group of parents who meet regularly with senior officers from health and the local authority to shape and improve services for disabled children in the city. Sleep Project – Providing both one to one sleep counselling support to families and the provision of Sleep Workshops. KIDS coordinates the overall service; with trained sleep practitioners working across health, social care, education and the voluntary sector. Parenting – KIDS coordinate and deliver the Stepping Stones parenting training programme for parents of disabled children. We also operate a parent to parent befriending scheme.

Background to Winner, the Preston Road Women’s Centre:

Preston Road Women’s Centre has been established in East Hull as a community anchor organisation for more than 10 years. It became a company limited by guarantee in February 2004 and a registered charity in November 2004. The organisation has delivered high quality services to local women and children in response to their expressed needs throughout its existence and is supported and led by women. Despite the challenging economic climate the organisation has continued to develop its services to respond to increased demand and need in the surrounding community. The organisation works with more than 800 women of various ages every year, enabling them to cope with crisis and develop themselves positively.

Safe dispersed accommodation: delivers safe accommodation for those in desperate need of it, the holistic wrap around services provided by the women’s centre integrated with safe accommodation delivers outstanding outcomes for those families and significant returns on investment for the public purse.

Specialist Domestic Violence and Abuse Support: This service works with women to offer practical and emotional support to help them live more safely in violent relationships and to escape from violent relationships. We assist them to deal with any Police prosecutions of the perpetrator, manage child custody issues and to rebuild their lives, including group and peer support.

Specialist Nursery childcare provision: Underpins all activities in the centre, providing registered, high quality childcare. Our staff have developed specialist expertise in looking after children recovering from the trauma of domestic violence and experiencing other significant social need associated with deprivation.

Young Women’s Participation project: Involvement and participation

opportunities for young women living locally to enable them to build skills and confidence and to find their voices. Trained staff and volunteers are able to provide a sexual health service providing advice, information and contraceptives to young people. We offer a cyberbabies project where young women are trained to look after a computerised infant so that they can understand what is involved in parenting and make informed decisions about deciding whether or not to become pregnant.

Comprehensive Volunteering programme: The women’s centre offers a range of opportunities for women to build skills and confidence through volunteering in a variety of work experience roles in the centre and the community shop.

Accredited training opportunities: Preston Road Women’s Centre is a recognised centre of learning by the National Open College Network. We offer a full programme of accredited and non-accredited learning aimed at enabling women to access training and qualifications to support them in their road to recovery and achieving their full potential. For some women, this is the first time in their lives they have had any educational achievement and the experience can be life-changing.
