Batch Pricing Model Information: Customer is required to provide and maintain current email address for the purposes of receiving batch consumption alerts. Customers exhausting all data records in their batch will automati-cally receive an auxiliary batch of 200 data records billed to the customer’s account at a published rate.

Performance is intended for a government contract and incorporates additional government contracting provisions. A copy of the corresponding contract documents is available upon request. AT&T’s wireless service is subject to the applicable Enterprise Agreement, individual consumer service agreement, rate plan brochure(s) and coverage map(s). See applicable materials for details. Batch Pricing Details

• Batches are established prior to each month by the customer through web portal or through your ProntoForms account manager1

• Unlimited users may be set up to draw from batch

• Batches will automatically be replenished each successive month and easy access allows customer to increase or decrease the size of the batch2,3

• Customer may view status of transaction volume against batch capacity via web or through pre-established e-mail alerts2,3

• Unused form submissions may be rolled into a bank for future use4

1 Initial month will be prorated for fee and batch size. Any unused form submissions will carry forward into bank for future use subject to the terms below.2 Should the batch be depleted in any month, over-usage will be charged at a rate of $350 per auxiliary batch of 200 forms.3 Additional batches may be purchased prior to depletion of purchased monthly batch amount.4 All banked form submissions must be used within a customer 12 month cycle beginning on the first day of the calendar month following initial sign-up

and each anniversary thereof. Banked form submissions may only be carried forward during a customer cycle if the customer remains at the initial batch volume or higher during the customer cycle. Upon cancellation, the account is closed and the number of banked form submissions will be set to zero.
