    TOURNAMENT INFORMATIONEXEMPTIONS: All former OGA Amateur Champions; Low 20 finishers in

    2013 Amateur; 2014 Mid-Am Champion and 2014 Junior Champion,2013 Senior Amateur, 2013 Four-Ball Champions (scratch), 2013 Mid AmChampion and 2013 Junior Champion.

    TOURNAMENT INFORMATION: Six sectional qualifiersChampionship field is 144. 18 holes, Tuesday, July 8, 8:00 am start.18 holes, Wednesday, July 9, 8:00 am start. Field cut to 60 and ties.18 holes, Thursday, July 10, 8:00 am start. Field cut to 40 and ties.18 holes, Friday, July 11, 8:00 am start. Low 20 exempt for 2015 OGAAmateur. Awards to winner and runner-up. Top 10 will win trophies.

    APPLICANTS: Must be (1) a member of an OGA Member Club or anAssociate Club with paid 2014 dues, (2) Determined by exemptions listedelsewhere in this application; or (3) Non members of OGA clubs may enterusing entry form. All entrants must have a 5.6 handicap index or better.Handicap must be through a USGA licensed golf club. A list can be foundat www.usga.org.

    SECTIONAL QUALIFIERS: A contestant, not exempt, (exempt list ison this app.) may compete at designated courses and dates over 18 holes.The county in which a contestants club is locat ed determines the section

    where he should qualify. Non members of OGA Clubs should choose a firstand a second qualifying location, in case of full fields. Entrant does nothave to enter qualifying in his home se ction.

    ENTRY PROCEDURE: Contestants, including exempt players, shall applyon official OGA entry blank, available at OGA Clubs, from OGA (ohiogolf.org) or from Board Members in charge of sectional qualifiers. Entry feerefund available only for medical reasons after deadline date, May 15,2014, 4 p.m. Late or incomplete applications not accepted. No telephoneor fax entries. On-line entry is available at OHIOGOLF.ORG.

    REGISTRATION: All qualifiers are required to register at Coldstream CC,Monday, July 7. A player may call the OGA to pre-register, if he is unableto get to Coldstream CC on Monday. If a player fails to register, his spotwill be given to an alternate. Club may allow alternate practice times, butwill charge appropriate fees.

    CADDIES: Contestants may carry their own bag. The followingconstitutes a condition of the competition. Appendix 1, restriction on whomay serve as a caddie is in effect: . A player is prohibited from havinga parent, guardian or another player in the same competition serve as hiscaddie during the stipulated round.

    FOOTWEAR: It is a condition of all qualifying sites and of the Amateurthat shoes with traditionally designed spikes (regardless of composition,i.e. ceramic, plastic, etc.) or spikes, regardless of design, comprised eitherentirely or partially of metal (if such metal may come in contact with thecourse) are prohibited during the stipulated round and practice rounds.Penalty for breach of this condition: Disqualification.

    TRANSPORTATION : (Ohio Amateur and Junior) Players must not rideon any form of transportation d uring a stipulated round unless authorizedby the Committee. The optional condition prescribed in Appendix 1 ofThe Rules of Golf will be in effect.

    SPECIFICATIONS OF CLUBS AND THE BALLList of Conforming Driver Heads: On its Web site (http://www.

    usga.org) the USGA periodically issues a List of Conforming Driver Headsthat lists driving clubheads that have been evaluated and found to conformto the Rules of Golf. Any driver the player carries must have a clubhead,identified by model and loft, that is named on the current List of Conform-

    ing Driver Heads issued by the USGA. Exception: A driver with a clubheadthat was manufactured prior to 1999 is exempt from this condition. *PEN-ALTY FOR CARRYING, BUT NOT MAKING STROKE WITH, CLUB OR CLUBS INBREACH OF CONDITION: Stroke play - Two strokes for each hole at whichany breach occurred; maximum penalty per round - Four strokes.

    *Any club or clubs carried in breach of this condition must be declared outof play by the player to his marker or a fellow-competitor in stroke playimmediately upon discovery that a breach has occurred. If the player failsto do so, he is disqualified. PENALTY FOR MAKING STROKE WITH CLUB INBREACH OF CONDITION: Disqualification.

    List of Conforming Golf Balls: On its Web site (http://www.usga.org) the USGA periodically issues a List of Conforming Golf Balls that listsballs that have been tested and found to conform with the Rules of Golf.The ball the player plays must be named on the current List of ConformingGolf Balls issued by the United States Golf Association. PENALTY FOR

    BREACH OF CONDITION: Disqualification.PRACTICE ROUND: Monday, July 7. NO PRE-ARRANGED PRACTICE

    ROUNDS. You must report to the first tee to arrange practice round. FIRSTTEE CLOSES AT 3:00 p.m. Players are advised to come early. No exceptions.Players who violate this or any OGA Rule will be immediately disqualified.

    PAIRINGS: Pairings will be mailed to all contestants. All entries shall bedeemed to be by invitation and are subject to OGA Board acceptance.

    HAMILTON HOUSE

    The Ohio Golf Association will send $5 per player from each 2014 entry fee to the Capital Campaignfor the new Evans Scholar House at the Ohio State University. The new house, located at 52 East14th Avenue is named the Hamilton House in honor of Earl Bones Hamilton. $5 per player of theentry fee qualifies as a charitable donation for federal income tax purposes.

    Evans Scholars Foundation administers the nations largest privately funded college scholarship program.

    Since 1930, over 8,500 deserving young caddies have earned their college degrees through EvansScholarships. Today, more than 820 caddies attend college each year as Evans Scholars.
