Nondamaging mishaps re orced nd precautionary andings. Aircraft osses or damages which were he direct result f ombat ction re ot ncluded n this eport.

    SYNOPSIS. he OH-6A was nvolved n 1,554 mishaps rom uly 967 hrough 0 une 971. There ere 63 ccidents of hich 41 ere classified s otal osses), 57 ncidents, 87 forced andings, nd 447 recautionary andings. Based n ,229,271 lying hours eported uring this -year eriod, he ccident ate per 00,000 flying ours as 5.81. hese ccidents n- volved 3 atalities nd 62 onfatal njuries. There ere ix n-flight ires nd 4 ostflight fires.

    CONCLUSIONS. uring his eporting period, Vietnam ccounted or 9 ercent f ll H-6A reported light ime 1,166,827 hours), 99 percent of he atalities 92), 7 ercent f he njuries (351), nd 8 ercent f he ccident/incident cost $31,023,000).

    Crew error was eported as he principal cause for he ajority f amaging ishaps. ack f discipline on he part of the operatorsstretching the imitations of he aircraft plus ailure o use standard rocedureswas evident.

    Supervision-related ishaps ere inimal. The upervisory actor ay e ne f mission rather han ommission. n ctive ommand- controlled rogram s equired o generate know- ledgeable, killful, isciplined aviators.
