Treat to Target initiative & EULAR recommendations
T2T EULAR
1- The primary target for treatment of rheumatoid arthritis should be a state of clinical remission
2007-10Treatment should be aimed at reaching a target of remission or low disease activity as soon as possible in every patient…
2- Clinical remission is defined as the absence of signs and symptoms of significant inflammatory disease activity
3- While remission should be a clear target, based on available evidence low disease activity may be an acceptable alternative therapeutic goal, particularly in established, long-standing disease

Final set of 12 recommendations
12- Monitoring of disease activity should include tender and swollen joint count, patient’s and physician’s global assessments, ESR and CRP.
Arthritis activity should be assessed at one to three-month intervals, as long as the remission is not achieved.
Structural damage should be assessed by X-rays every six to twelve months during the first few years. Functional assessment (eg. HAQ) can be used to complement the disease activity and structural damage monitoring.

La PR est une maladie hétérogène
• Facteurs prédictifs de progression articulaire
– Érosions articulaires précoces
– Importance de l’activité initiale • VS/CRP• Activité clinique
– ACPA (anti-CCP)

RECO 8: In patients responding insufficiently to MTX and/or other synthetic DMARDs with or without glucocorticoids, biological DMARDs should be commenced; current practice would be to start a TNF inhibitor (adalimumab, certolizumab, etanercept, golimumab, infliximab) which should be combined with methotrexate

RECO 7: If the treatment target is not achieved with the first DMARD strategy, additional of a biological DMARD should considered in case of presence of individual poor prognostic factors. In the absence of poor prognostic factors a switch to another synthetic DMARD strategy should be considered.

• NICE technology appraisal guidance 130 recommends that the response to treatment with a TNF-α inhibitor should be
increased from 3 months to 6 months after the initiation of therapy
• EULAR 2010 RECO 2: Treatment should be aimed at reaching a target of remission or low disease activity as soon as possible in every patient; as
long as the target has not been reached, adjustment of the treatment should be done by frequent (every 1-3 months) and strict monitoring
“…..the treatment target should preferably be reached, or almost reached, within 3 months and definitely attained by a maximum of 6 months…..”

Leflunomide, sulfasalasine Sels d’or Hydroxychloroquine autres
Arret biologique ? Arret MTX ? Diminution ou espacement des doses? Quand?
Stop AINS / corticoides Remission prolongée /sans progression
RECO 12: If a patient is in persistent remission, glucocorticoids should be tapered and one can consider tapering biological DMARDs# especially if this treatment is combined with a synthetic DMARD.
RECO 13: In case of sustained long-term remission, cautioustitration of synthetic DMARD could be considered, as a shared decision between patient and physician.
