前　言

本書之特色為設計於某特定情境脈絡下，提供醫護人員與病人

或家屬之對話，以中英文對照方式呈現，使臨床護理人員或護理科學

生易於了解臨床常用之英語會話及各科慣用字彙，增加其對醫護英語

的實用能力。全書涵蓋七大篇，分別為初診篇、病史詢問及症狀評估

篇、住院照護篇、內外科病人指導篇、婦產科病人指導篇、小兒科病

人指導篇，及公共衛生概論篇，各篇中包含數種不同照護主題，每章

內容依據情境、對話、文章選讀及字彙編排，期使閱讀對象學習實用

性之醫護對話，並深化其對各科常用字彙之敏感度。

筆者回顧美國求學返國至今已十年，近年來各校護理科依據教

育部發展系科本位課程，教師們無不藉由臨床護理人員的需求，省思

何種課程內容最能符合臨床所需。書寫本書之構想源起於護理科與學

生的教學互動經驗，曾有多數學生表示對英文戒慎恐懼，民國89∼91

年筆者因教授醫學術語課程，發現大多醫學術語教材著重在醫學縮寫

及字詞解說，缺乏具脈絡性及實用性之對話，僅以縮寫字彙教導學生

學習，往往導致學生學過即忘。而教育部訂之英文課雖占課程許多學

分，但往往偏重一般語言學的學習，以文法結構及一般英語為其主要

內容，無法滿足護士或護理科學生醫護專業對話的需求。此外，多位

臨床護理科主管表示，臨床護理人員對於臨床醫學字彙缺乏熟悉度，

致使醫療專業成員間的溝通常常出現溝通不良之情景。因此在與幾位

教師討論後，萌生書寫一本適合臨床護理人員及專科生閱讀之護理會

話書籍。

正式開始進行本書的書寫在民國94年，為使內容中英文的表達更

具完整性與正確性，本書除護理教師的參與，更邀請英文教師成為本

書之作者群，共同負責英文內容之撰寫，以確保閱讀人員獲得正確的

學習材料。然作者群於書寫過程中因為某些事件的耽擱，使此書的出

版遲遲未能付梓，幸好五南圖書公司的積極與鼎力支持，才使其有出

版的一天，在此特為感謝五南圖書公司的用心與支持。相信本書的出

版，對於臨床護理人員及護理科學生無論在自學或是課室學習醫護英

語都能助益匪淺。

糖尿病護理指導實用對話

SituationThe 40-year-old patient, John, was sent to the Metabolism Unit of the

hospital due to the complications of DM. The nurse, Mary, is talking to him.

四十歲糖尿病病人John，因為糖尿病合併症住入新陳代謝病房，護

士Mary正與他會談。

DialogueJohn: I would like to know what the diabetes mellitus (DM) is?

約翰：我想知道什麼是糖尿病。

Nurse: DM is an endocrinopathy because of the inefficient use of, or

lack of insulin. There are two types of DM: the type I diabetes is called

insulin dependent diabetes mellitus (IDDM), and the other type, the type II

diabetes, is called non-insulin dependent diabetes mellitus (NIDDM). Your

case is the type II diabetes.

護士：糖尿病是一種內分泌病變，主要是胰島素製造不足或缺乏。糖尿

病可分兩型，一種被稱為第一型糖尿病，屬於胰島素依賴型糖尿病；另

一種被稱為第二型糖尿病，是非胰島素依賴型糖尿病。你應是屬於第二

型的糖尿病。

John: What caused DM?

約翰：什麼原因會導致糖尿病？

Nurse: There are two main causes. When the pancreas cannot supply

enough insulin or the insulin cannot function well—The insulin resistance

and the glycemic cannot get through the cells to convert to energy, it

happens. This kind of situation can be improved by medication, exercises,

health eating habits as well as weight control. Of course, when it becomes

more serious, insulin can be used to solve it.

護士：主要有兩個原因。當胰臟製造的胰島素不足或胰島素作用不良，

也就是胰島素阻抗，血中葡萄糖亦因此無法進入細胞來當作能量時，就

會產生糖尿病。這種情形可藉由藥物、運動、健康的飲食習慣與體重控

制來改善，當然嚴重時可能也需要使用胰島素來緩解。

John: What are the symptoms of DM?

約翰：糖尿病的症狀有哪些呢？

Nurse: It is not easy to find out at the beginning, so a regular health

examination is very important. When it gets worse, the patient will have

the following symptoms, such as excessive urine production, is called

polyuria, increased or excessive thirst so patients will drink more that is

called polydipsia. Patients also have an increase in food intake, it is called

polyphagia. Sometimes weight loss and weak eyesight can also occur.

護士：剛開始不易發現，所以定期健康檢查是非常重要的。當病情慢慢

嚴重時，病人會有以下症狀，如：小便頻繁（尿多）、經常口渴（喝

多），病人也會有容易飢餓的感覺（吃多）等三多症狀。有時候也會有

John: Can DM be totally cured?

約翰：糖尿病可否完全根治？

Nurse: It is impossible. But it can be controlled by food intake, exercises

and medication.

護士：不可能，但病情可藉由飲食、運動和藥物配合控制。

John: If the patient does not go to see the doctor, what will happen?

約翰：如果糖尿病患者不去看醫生會怎麼樣？

Nurse: Patient will have cataracts and retinopathy. The nephropathy of a

diabetic can cause uremia, peripheral polyneuropathy which would develop

into pain and numbness of hands and feet. Among those, the bad peripheral

circulation would cause infection of wounds and lead to amputation. It

also increases the possibility of causing coronary cardiac disease and

cerebrovascular accidents.

護士：病患易有白內障和視網膜病變。糖尿病性腎病症會引起尿毒症、

末梢神經病變和神經炎等造成手腳麻木疼痛。在上述情況下，末梢循環

不良會造成傷口易感染而截肢，也會增加冠狀動脈心臟病和腦中風的罹

病率。

John: It sounds terrible.

約翰：聽起來挺嚴重的！

Nurse: That is right. You have to take the oral hypoglycemic agents based

on doctors’ advice. Besides, you have to check for glycosuria and glycemia

regularly. Regarding how to take the medicine and check your glyconuria

and glycemia, you can read the health instructions for DM. We also want

to make an appointment with you to go over those instructions.

護士：沒錯。你必須依據醫師指示定時服用降血糖藥物，另外必須定期

檢驗尿糖及血糖。關於如何服藥及監測尿糖、血糖，你可以閱讀糖尿病

衛教手冊，另外我們也會跟你約定時間來進行實際衛教。

John: That is great! I also heard that I had to change some habits of my

food intake and daily activities. Are these all right?

約翰：太好了。另外，我也聽說必須在飲食方面及日常活動方面做一些

改變，是嗎？

Nurse: Yes, you are absolutely right. These are very important. I have

already made an appointment with the nutritionist who will give you some

helpful and valuable suggestions of your daily life.

護士：是的，你說得沒錯，這個非常重要。我已經幫你預約好營養師，

他會提供你日常生活許多有用並且有價值的建議。

John: Thanks very much!

約翰：非常謝謝你！

Nurse: Besides, I want to discuss the foot care for diabetes mellitus with

you.

護士：除此以外，我要與你討論有關糖尿病的足部護理。

John: What is it? Why should I take care of my feet specially?

約翰：那是什麼？為什麼我需要特別照護我的足部？

Nurse: The combined effects of diabetic peripheral neuropathy and

impaired circulation face a lot of problems for all diabetics, l ike the

gangrenous feet is the most popular complication of DM.

護士：在糖尿病周邊神經病變及損害循環的雙重影響下，將促使糖尿病

病患面臨許多問題，像足部壞疽便是糖尿病非常普遍的合併症。

John: It is very bad. What can I do to take care of my feet?

約翰：那非常糟糕。我該如何照護我的足部呢？

Nurse: First, be careful to protect your feet from injury is very important.

Any minor injury may allow infection to set in, which could lead to

gangrene and amputation. You have to visually inspect the foot when the

socks go on and when the socks come off.

護士：首先，小心保護你的足部免於受傷是很重要的。任何小的傷口都

可能引發感染，而導致壞疽及截肢。當要穿上襪子或脫下襪子時，你必

須要仔細觀察檢查足部。

John: Oh, I will. Is there any else?

約翰：哦！我會的。還有其他要注意的嗎？

Nurse: Second, you have to wash and dry between the toes on a daily

basis. You can apply skin cream on a daily basis to the feet and whether it’s

an over-the-counter or prescription skin softener and do it every day.

ReadingDiabetes mellitus is a medical disorder characterized by persistent

variable hyperglycemia (high blood sugar levels), resulting either from

inadequate secretion of the hormone insulin, an inadequate response by

the body’s cells to insulin, or a combination of these factors. The most

common forms of diabetes are type I, type II and gestational diabetes.

Diabetes mellitus is a chronic disease; therefore, patients should do some

change in their daily in order to copy with this disease. For example,

learning how to regular taking medicines, appropriately regular rest and

exercise, doing special interventions for preventing complications. Self-

monitoring of blood glucose should be done regularly by checking the

glucose content of a drop of blood. The results of the test can be used to

adjust meals, activity, or medications to keep blood sugar levels in an

appropriate range. Testing provides valuable information for the health

care provider and identifies high and low blood sugar levels before serious

problems develop.

糖尿病是一種醫學疾病，特徵為持續性變動的高血糖狀態，形成原

因可能是胰島素分泌不足或是身體細胞對胰島素的反應不足，也有可能

是合併以上兩種原因。其中最常見的類型分別是第一型糖尿病、第二型

糖尿病和妊娠糖尿病。糖尿病是一種慢性疾病，因此為了要因應此疾

病，病人必須要做一些日常生活上的改變，例如：學習如何規律地服

藥、適當的規律運動與休息，或是執行特殊的侵入性措施以防止合併症

產生。從血液中自我監測血糖必須規律地執行，藉由血糖值的變化可以

了解如何調整飲食、藥物和運動以控制血糖值在正常的範圍內。檢測結
