Lúcida, buen estado gral. Peso: 73 kg. talla: 178 cm. IMC: 23 kg/m2.
PyM: normocoloreadas, bien hidratadas, sin lesiones. No estrias rojovinosas.
Cuello: no se ve ,ni se palpa tiroides, ni otras tumoraciones. No adenomegalias.
CV: RR de 70 cpm. RBG. No soplos. No hipotensión ortostática.

Volumen tiroideo: 21 cc
Ecoestructura heterogénea e expensas de pequeños nódulos sólidos hipoecoicos, sin vascularización. El mayor de 4 x6 mm.
Por detrás del sector inferior del LD, nódulo de 7x9x11 mm, redondeado, hipoecoico, sin vascularización. Dos imágenes de similares características por debajo del polo inferior derecho de 8x4 mm y a izquierda de 11x4 mm, ambas en intimo contacto con la arteria carótida. Dado el antecedente planteamos que las mismas correspondan a glándulas paratiroideas aumentadas de tamaño.

Discusión
• El planteo fue la realización de cirugía 7/8 en las 4 paratiroides.
• Se hablo de la importancia del manejo de la hipocalcemia post quirúrgica dada la cantidad de tejido paratiroideo a resecar.
• Por otra parte se hablo de la importancia del seguimiento en lo clínico y bioquímica en la aparición de otras manifestaciones de la neoplasia endocrina múltiple.
