Tasso di gravidanze plurime (gemellari o più): varia da 9-13 gravidanze plurime per 1000 donne in Romania, Lettonia, Lituania e Polonia a oltre 20 per 1000 nella regione di Brussels, Repubblica Ceca, Danimarca, Cipro, Spagna e Malta (Italia 15.7 per 1000). Fattori come età materna avanzata e e il ricorso alle tecniche di procreazione medicalmente assistita possono in parte spiegare questa variabilità.

Sette nati su cento hanno una madre ultraquarantenne
• Si diventa madri sempre più tardi • La distribuzione delle nascite per età della madre consente di apprezzare lo
spostamento della maternità verso età sempre più avanzate, caratteristica questa ancora più evidente per le madri di cittadinanza italiana
• La posticipazione delle nascite ha contribuito al forte abbassamento della natalità osservato nel nostro Paese dalla seconda metà degli anni Settanta alla prima metà degli anni Novanta. Successivamente si è registrato un parziale recupero delle nascite precedentemente rinviate in particolare da parte delle baby-boomers, che si è tradotto in un progressivo aumento delle nascite da madri con più di 35 anni ravvisabile soprattutto al Nord e al Centro.
• Nel 2011 le donne hanno in media 31,4 anni alla nascita dei figli, circa un anno e mezzo in più rispetto al 1995 (29,8), valore che sale a 32 anni per le madri di cittadinanza italiana.
• Quasi il 7% dei nati ha una madre di almeno 40 anni, mentre prosegue la diminuzione dei nati da madri di età inferiore a 25 anni (nel 2011 sono il 10,9% del totale).
• Considerando le sole donne italiane la posticipazione della maternità è più accentuata: il 7,7% sono ultraquarantenni e solo l’8,2% ha meno di 25 anni.

Età materna: le nascite da madri di età >35 anni variano dal 10.9% in Romania a 34.7% in Italia. Invece, quelle prima dei 20 anni sono in generale ≤ 2% (1.4% in Italia), ma ci sono Paesi dove superano il 5% (Regno Unito, Malta, Slovacchia e Romania).

Procreazione medicalmente assistita (PMA): include induzione farmacologica della ovulazione, inseminazione intrauterina, e le tecniche di fertilizzazione “in vitro”. In alcuni Paesi, fino al 5-6% delle nascite si verificano dopo applicazione di qualche forma di PMA (1.9% in Italia). E’ possibile però che i dati siano sottostimati, in quanto l’uso delle tecniche meno invasive (ad es. solo trattamento farmacologico per induzione dell’ovulazione) può sfuggire alla registrazione.

Il Progetto EURO-PERISTAT Il progetto, avviato nel 1999 dalla Unione Europea, si propone di sviluppare e utilizzare indicatori validi e affidabili per monitorare la salute perinatale in Europa. I risultati della prima raccolta dati, relativa agli anni 1998-2000, sono stati pubblicati nel 2003 su un numero speciale di European Journal of Obstetrics & Gynecology an Reproductive Biology. Nel 2008 è stato pubblicato il primo Rapporto Perinatale a sè stante, contenente i dati di 25 Paesi membri della UE e della Norvegia. Il rapporto pubblicato oggi si riferisce a 26 Paesi membri con aggiunta di Islanda, Norvegia e Svizzera. Euro-Peristat è co-finanziato dal Programma Health, Directorate General for Health and Consumers della Unione Europea, e coordinato dall’Istituto per la Salute e la Ricerca Medica (INSERM) di Parigi. Fertilità : APPROCCIO COMPARATIVO• Gli approcci dei demografi e degli epidemiologi allo studio della fertilità
delle coppie, (cioè la loro attitudine a concepire), per molto tempo distinti, attualmente si sono ravvicinati.
• La demografia si interessa, tradizionalmente, soprattutto della fecondità, cioè l’ottenimento di nascite con neonati vivi, per poter prevedere l’evoluzione di una popolazione. Essa tende ad interessarsi alla fertilità, definendo i concetti come la fecondabilità o la sterilità definitiva.
• Il metodo di misurazione della fecondabilità è il tempo necessario per concepire (Time To Pregnancy: TTP), che costituisce anche lo strumento di base dell’epidemiologo.
• Mentre i demografi hanno sviluppato dei metodi per stimare la distribuzione della fecondabilità all’interno delle coppie a partire dal TTP, gli epidemiologi si sono orientati verso dei metodi per analizzare il ruolo dei diversi fattori capaci di influenzare la fertilità a livello individuale.

Il Censimento 2011
• Al 9 ottobre 2011, data di riferimento del 15° Censimento generale della popolazione e delle abitazioni, la popolazione residente in Italia ammonta a 59.433.744 unità.
• Rispetto al 2001, quando si contarono 56.995.744 residenti, l’incremento è del 4,3%, da attribuire esclusivamente alla componente straniera. Infatti, nel decennio intercensuario la popolazione di cittadinanza italiana è diminuita di oltre 250 mila individui (-0,5%), mentre quella straniera è aumentata di 2.694.256 unità.
• I cittadini stranieri risultano in crescita in tutte le regioni della Penisola, mentre gli italiani diminuiscono nel Mezzogiorno oltre che in Piemonte, Liguria e Friuli-Venezia Giulia

• In Italia, al 9 ottobre 2011, ci sono 93,7 uomini ogni 100 donne (28.745.507 uomini, 30.688.237 donne). A livello territoriale non si segnalano differenze significative, anche se nell’Italia Centrale il rapporto di mascolinità scende al 92,3% (5.568.595 uomini, 6.032.080 donne), mentre nelle regioni del Sud, nelle Isole e nel Nord-Est si attesta rispettivamente a 94,3% (6.783.667 uomini, 7.193.764 donne), 94,1% (3.219.998 uomini, 3.422.268 donne) e 94,2% (5.551.923 uomini, 5.895.882 donne).
• Il rapporto di mascolinità è più alto in Trentino-Alto Adige (95,9%), Basilicata (95,8%) e Sardegna (95,6%) mentre il più basso si registra in Liguria (89,5%).

I parametri seminali predicono la fertilità maschile ?
• Nello studio multicentrico europeo sul T.T.P. (quindi coppie che hanno concepito) tra le 76 coppie che necessitarono di >12 mesi per concepire: • solo il 37% aveva qualità seminale ridotta (conc. < 20 Mil/ml, forme
normali < 30%, MAI > 1,8) • anche escludendo le coppie con meno di 1 rapporto la settimana e
con ciclo mestruale regolare solo il 44% aveva qualità seminale ridotta.
