What If…

• You can't squat because you have bum knees?

• You can't do bench presses or overhead lifts because of lingering shoulder injuries?

• Your bad back makes barbell deadlifts a bad idea?

Use Joint Friendly Strength Training Exercises!

A list of alternative exercises (that we’ve used over the years to help clients and athletes of all levels) to work around commonly sensitive spots

and still make gains in strength and performance.

The Problem!

People want fitness, performance and/or physique related results!

Pain and limitations prevent them from performing the conventional exercises that accomplish these desired goal(s).

Many fitness professionals try to play the “corrective (therapy) exercise” game with clients or avoid exercising certain areas of the

body all together.

Clients end up frustrated, bored and less fit!

The Solution!

Joint Friendly Strength Training concepts allow us to “work‐around” sensitive spots and limitations in order to still create the necessary training effect to make fitness, physique ad performance gains!

Corrective Ex: Forward Head Posture and Pain – Claims vs. Evidence

• Forward head Posture ‐“Of 427 people examined, not even one showed a resting forwards head posture perfectly aligned with the ‘ideal’ posture postulated by Kendall” Aust J Physiother. 1997;43(1):7‐1

• Forward Head Position Kyphosis and Shoulder Impingement: “posture in asymptomatic and symptomatic people with subacromial impingement syndrome does not follow the set patterns referred extensively in medical, physiotherapy, and osteopathy textbooks and articles. The results suggest that posture may appear faulty, yet the individual maybe flexible and capable of large range of movements.” The author further goes on to point that postural assessment has a very limited role in the clinical decision making process in subjects with impingement syndrome.”  J Shoulder Elbow Surg. 2005 Jul‐Aug;14(4):385‐92

• These findings were supported by two other studies, which reported no relationship to exist between forward head posture, forward slouch posture, and the curve of thoracic kyphosis in both symptomatic and asymptomatic people.” Phys Med Rehabil, 1997; 78:1215‐23 & J Orthop Sports Phys Ther. 1995 May;21(5):287‐95

Corrective Ex: Spinal Curvatures and Pain –Claims vs. Evidence

• Spinal Curvatures & LBP: This 2008 systematic review of more than 50 studies found no association between measurements of spinal curves and pain.

• “Differences in regional lumbar spine angles or range of motion between the segments failed to show an association with the future development of LBP”(Hellsing,1988b;Burton&Tillotson,1989; Hamberg‐ van Reenen, 2007, syst. rev.; Mitchell et al., 2008

• Pelvic obliquity/asymmetry & LBP: “There is no correlation between pelvic obliquity/asymmetry and the lateral sacral base angle and lower back pain” (Dieck, 1985; Le‐vangie, 1999a,b; Fann, 2002; Knutson, 2002)

• Hamstrings and Psoas tightness & LBP: “Prospective studies of inflexibility of the lower extremities and hamstrings and psoas tightness also fail to predict future episodes of LBP”(Hellsing, 1988c; Nadler, 1998)

Corrective Ex: Special Exercises and Pain – Claims vs. Evidence

• A special (Pilates‐based) series of exercises focused on strengthening "core muscles" in a specific way is no better than general exercise for LBP in the long term.

Yamato TP, et al. Pilates for low back pain.

Cochrane Database of Systematic Reviews 2015, Issue 7. Art. No.: CD010265.

• Contrary to our expectation, motor control exercise and general exercise appear equally effective in the patient subgroup included in this study. We can conclude that the contrast between both types of intervention did not bring additional value to the shared effects… It is possible that the type of exercise treatment is less important than previously presumed; that the patient is guided to a consistent long‐term exercise lifestyle is of most importance.”

• "The results of our study support previous findings that exercise in general, regardless of the type, is beneficial for patients with NSLBP”

Saner J, et al. BMC Musculoskelet Disord. 2011 Sep 23;12:207.

3 RULES OF JOINT FRIENDLY STRENGTH TRAINING

Page 13: Joint Friendly - NSCA€¦ · Use Joint Friendly Strength ... posture perfectly aligned with the ‘ideal’posture postulated by Kendall ... contrast between both types of

• Comfort ‐‐ The movement is pain free, feels natural and works within your current physiology.

• Control – You can execute the movement technique and body positioning as indicated by the trainer/coach.

Page 14: Joint Friendly - NSCA€¦ · Use Joint Friendly Strength ... posture perfectly aligned with the ‘ideal’posture postulated by Kendall ... contrast between both types of

Rule #2: Modify to fit individual ability

To allow for comfort and control, you may need to modify (shorten) the range of motion or adjust the hand or foot placement of a

particular exercise to fit your current ability to honor the joint alignment cues provided in the

trainer/coach.

Page 15: Joint Friendly - NSCA€¦ · Use Joint Friendly Strength ... posture perfectly aligned with the ‘ideal’posture postulated by Kendall ... contrast between both types of

• In some cases, you may just have to avoid certain exercises and emphasize other options.

• If an exercise hurts you—for whatever reason—find a modification or an alternative that doesn’t hurt.

Additional (General) Programing Tips:

• Fight the sitting position: Focus on the backside of the body.

• JFST exercises aren’t just for injury, aches, or pain!• These exercises provide effective and challenging ways to build muscle, improve strength and functional capacity (one’s range of ability).

• Focus on general exercise concepts – they’re far more well‐established (in the scientific evidence) than anything else to improve health and pain measures.
