Differential diagnosisDifferential diagnosis
• Polyarteritis nodosa (PAN)• Antiphospholipid syndrome
Criteria : Criteria : childhood polyarteritis childhood polyarteritisnodosanodosa
• Histopathology or angiographic abnormalities (mandatory) plus one of the
five following criteria:– Skin involvement– Myalgia/muscle tenderness– Hypertension– Peripheral neuropathy– Renal involvement
Ann Rheum Dis 201069798 806; : – .
Polyarteritis nodosa (PAN)Polyarteritis nodosa (PAN)
ImpressionImpression
Polyarteritis nodosa Polyarteritis nodosa (PAN)(PAN)
• Inflammation throughout the entire arterial wall.
• Rare in childhood and although epidemiology is poorly defined.
• Hepatitis B causes a minority of cases of PAN• Manifestations are diverse and complex,
ranging from the benign cutaneous form to severe disseminated multisystemic form.
Polyarteritis nodosa Polyarteritis nodosa (PAN)(PAN)
• Untreated PAN was usually fatal within weeks to months.
• Most deaths occurred as a result of kidney failure, heart or GI complications.
• Survival rate of 75% to 80% at 5 years
Polyarteritis nodosa Polyarteritis nodosa (PAN)(PAN)
Treatment• Conventional
– high-dose steroid with an additional cytotoxic agent (Cyclophosphamide, mycofenolate mofitil)
• New treatment– Plasma exchange,IVIG,Anti-TNFa monoclonal
antibody(infliximab),Anti-CD20 monoclonal antibody(Rituximab)
