The frameworkconceptualizes service quality on various primary and secondary dimensions, namely,infrastructure, personnel quality, trustworthiness of the hospital, administrativeprocedures, process of clinical care, social responsibility, hospital image and safetyindicators. The dimensions have been already explained in Section 10. The proposedinstrument for measuring these dimensions is provided in Appendix. Items have beenmodified largely in order to suit the context of healthcare services. One of the criticisms

poorly understood [6]. Kling writes that anumber of authors of the report ‘Computingthe Future’ encourage to envision new tech-nologies in the social contexts in which theywill be used: ‘They identify numerous exam-ples of computer applications in earth sci-ence, computational biology, medical care,electronic libraries and commercial comput-ing that can provide significant value to peo-ple and their organizations. Theseassessments rest on concise and tacit analysesof the likely design, implementation, and usesof these technologies within organizations’[7].
In this paper, we do not offer explanationsof the organizational factors that determinethe implementation process. Rather we wishto present a model that will allow researchers,educators and practitioners to identify rele-vant domains in the implementation processof information systems. The model is rootedin the approach that several authors havedeveloped to describe the adoption of infor-mation and communication technology, butalso reflects the emerging thinking about thesocio-technical dimension of information sys-tems [8,9]
2. The model
Firstly, we want to start with the assertionthat in our vision when introducing informa-tion and communication technology (ICT)into health care three domains are in need ofan ‘effective’ fit as visualized in Fig. 1 bymeans of a jig-saw puzzle. We identify thesedomains as ‘clinical work’, ‘organization’ ofmedicine and health care (system) and ICT.By clinical work, we understand all healthcare delivery activities that result from theinteraction between patient and clinician.Clinicians are physicians, nurses and alliedhealth professionals, while clinical work is
not restricted to activities taking place in ahospital or any other intramural health careorganization. Clinical work also encompassesthe activities of multi-disciplinary teams. Themulti-disciplinary approach of patient care isgrowing and, for example, already very com-mon in psychiatric care.
It is trivial to state that clinicians do theirwork in an organizational setting. They aredependent on the material and financial re-sources made available through the organiza-tions they work in or the health care systemat large. There are rules, regulations, andlaws, but also unwritten codes and practicesthat determine the clinicians’ behavior in anorganization. The triviality ends where manyapplications developed for use by cliniciansseem to ignore the organizational setting [10].
The information and communication tech-nologies are of course the third piece of thepuzzle. We consider all technologies that sup-port clinical work in practice as belonging tothat domain, whether they are advanced deci-sion support systems or more clericalsystems.
‘Effective’ fit essentially means that threedomains will mould each other to obtain thebest possible results in the delivery of care.Berg writes ‘that the work of medical person-nel is rewritten in the light of the tool andvice versa’ [11]. Work practices and ICT are

Service Blueprint - Modification
Service innovations consist of four dimensions, implying that one of these four elements or more likely a mixture of major or minor changes within these dimensions characterize and describe service innovations. Hence, a service innovation consists of a new service concept, a new client interface, a new delivery system, and/or new fields for technology application [9]. Whereas the first three dimensions are intuitive regarding innovation, the fourth isn’t. To provide an illustrative example for this dimension, one might think on a mobile boarding pass. Mobile phones as well as Multimedia Messaging Service (MMS) or email have rather been adapted and applied than developed specifically for mobile boarding passes.
C. Transformation System Shaping the transformation system itself is the most
challenging task. To cope with this challenge, the definition of service innovation needs to be considered. Service innovation comprises the co-creation between a provider and a customer, generating, designing, and exploiting valuable output [1] [49] [31] [47]. But customers are not the only ones that are integrated to co-create value. Taking a broader perspective, several other stakeholders can be identified. Besides suppliers, other companies might be participating as well [7] [31] [34]. Hence, the transformation model depicts the aspect of integrating partners explicitly, being an essential part of service innovation. In addition, the innovating service firm is represented as a part of the transformation system. The corresponding subsystems for the firm are shaped according to the McKinsey 7-S framework. This method was developed and introduced by Peters and Waterman [40], focusing explicitly on the hardware of organizations – strategy and structure – as well as on the software – style, systems, staff, skills, and shared values. This framework is utilized to analysis organizations and fits perfectly for the illustration of the innovating service
firm within the transformation system, representing seven subsystems that need to be considered. To sum up, the transformation system consists of an innovating service firm that is related to partners (e.g., customers, suppliers) through co-creation of value and divided in seven subsystems.
D. Feedback The feedback loop monitors and controls the efficiency
and effectiveness of the whole system. Therefore, two basic goals are identified. Besides monitoring the transformation system itself, the generated output has to be evaluated. Evaluating several stages of service innovations during the development phases is critical for the success of service innovation [20]. In general the evaluation of service innovation ideas has to fulfill three objectives. Evaluation has to provide the basis for stop-or-go decisions of various ideas, prepare sufficient rankings for efficient resource allocation, and identify weak points of ideas to declare improvement potentials [39]. Hence, the feedback loop is considered to be a vital part of the whole research framework, ensuring continuous monitoring, controlling, and improving of the system.
E. Environment A common approach in economic science to characterize
the environment is the PESTEL approach. Hence, the environment surrounding the transformation system can be subdivided in different influencing factors, categorized into political, economical, social, technological, ecological, and legal domains.
F. Validation The service innovation research framework is valid,
conducting a short descriptive evaluation. Hevner et al. [16] declare that informed argument is a suitable design evaluation method. Hence, the research framework is

Iyer and Muncy (2004) considered that level of trust patients had varied acrosspatient categories and segmented the patients based on the level of the trust they hadon the service provider.
This section has provided a detailed discussion on the determinants of healthcareservice quality. The next section proposes a framework to conceptualize the healthcareservice.
11. The proposed conceptual frameworkA conceptual framework for healthcare service quality, based on the literature reviewand discussions presented in previous sections is shown in Figure 1. The frameworkconceptualizes service quality on various primary and secondary dimensions, namely,infrastructure, personnel quality, trustworthiness of the hospital, administrativeprocedures, process of clinical care, social responsibility, hospital image and safetyindicators. The dimensions have been already explained in Section 10. The proposedinstrument for measuring these dimensions is provided in Appendix. Items have beenmodified largely in order to suit the context of healthcare services. One of the criticisms
