Report from Meeting of the Chairpersons of the four Forums.

The Chairman outlined for members, details of the meeting of the Chairpersons of the four Forums, which was held on

Thursday, 10th June 2010. Items discussed and agreed included;

Chairperson’s Allowance

The Chairs noted the position in relation to submissions made regarding the Chairperson’s Allowance and

confirmed their view that the payment of an allowance was warranted and would affirm the status of the position

of the Chair.

Centralisation of ICT aspects of medical card processing

It was agreed that the Chairs would write to the CEO seeking the regionalization of the management of the

medical card processing function.

The posting on the HSE Website of Forum members’ expenses

It was agreed that each Chair would pursue the matter at a future Committee meeting of each of the Forums.

Registration in Ireland of deaths abroad of Irish citizens

The motion, adopted by the Regional Health Forum West, was discussed in detail. It was anticipated that a

similar motion would be adopted by the other three Forums but it would be necessary, in the first instance, for

each Forum to first propose the Motion at a future meeting.

How the Regional Health Forums can be made more effective

The four Chairs discussed at length the effectiveness of the Regional Health Forums. It was agreed that the

Chairs would write to the Chair of the HSE Board on the matter.

Update from HSE on capital allocation for 2010

Report presented by HSE was welcomed and noted by the Chairs.

Update on numbers of administration staff (IV and higher), and also of clinical staff, in 2004 (last year of

Health Boards), 2006 (first year of HSE), and current numbers and need for Lean Study of HSE (D&NE)

Report presented by HSE was welcome and noted by the Chairs.

Brothers of Charity

The allocation of budgets for services provided by the Brothers of Charity was raised and is to be followed up by

the HSE.

What was the outcome of the Medical Card Motion, adopted at the February 2010 Forum meeting, which was due to

be raised at the meeting of the Chairpersons of the four Forums?

Seeking an urgent meeting with Ms. Mary Harney TD and Professor Drumm, CEO re Centralisation of Medical Cards

System - Regional Health Forum Meeting 23.2.2010

“That this Forum seeks an urgent meeting with Professor Drumm and Minister Mary Harney to discuss their intention

to centralise the processing of all Medical Card applications in April. This is a crazy decision and needs to be re-

considered in light of the problems being encountered with the over 70’s, since the processing of their Cards was

centralized.”

The Chairperson advised that this motion also received strong support from the other three Chairpersons. It was

agreed that the Chairs would write to the CEO of the HSE, in the first instance, on the matter. Any response received

from the CEO would be circulated immediately to all Forum members.

Next Regional Health Forum Meeting

The Chairperson advised members that the next Forum meeting would be held on Tuesday, 28th September, 2010 at

2pm in Room 1, Education Centre, Merlin Park, Galway.

292/28/10 Election of Chairman/Toghachán an Chathaoirligh

Councillor Padraig Conneely was proposed by Councillor Gordon Hughes and seconded by Councillor Paul Murphy.

As there were no other nominations Councillor Conneely was deemed elected and remained as Chair.

Councillor Conneely thanked his proposer and seconder and stated that it is a great honour for him to be re-elected as

Chairperson of the Regional Health Forum.

294/28/10 Formation of Regional Health Forum Committees/Bunú Coistí na bhFóraim

Sláinte Réigiúnacha

The Chairperson advised members that should they wish to move from one Committee to another, they could do so

by notifying the Regional Health Office, in advance of the Committee meetings due to be held on the 7th September,

2010.

296/28/10 Matters Arising/Nithe ag éirí

2010 HSE Business Plans

Councillor Declan Bree asked if Business Plans for county and hospital level were finalized and could now be

circulated to Forum members, as discussed and agreed at the last Forum Meeting.

Mr John Hennessy advised that it is hoped that local Business Plans would be finalized before next Regional Health

Forum meeting, preferably by mid July 2010.

It was agreed that these Plans would be circulated to Members as soon as they became available.

Medical Cards

Councillor Declan Bree acknowledged that although he has received some information regarding Medical Card

Holders as requested, that he has yet to receive information regarding Over 70s Medical Card Holders.

Mr John Hennessy agreed to provide follow up within one week.

Consultant Surgical Posts, Portiuncula Hospital

Councillor Michael Mullins requested the position regarding the advertising and filling of Consultant Surgical posts at

Portiuncula Hospital?

Ms Chris Keane advised that the interview board has been selected and the process has commenced with the Public

Appointments Service. She advised that one position will be advertised shortly. Once approval has been received to

proceed with the Competition Advertisements, Locums can be appointed. The process however could take up to 6

months or more to complete.

Lifford Hospital

Councillors Liam Blaney and Frank McBrearty requested an update regarding Lifford Hospital, highlighting the fact

that members were advised that a decision was forthcoming within 3 weeks of the last Forum meeting. Members also

highlighted the need to inform the people of Donegal regarding plans for the future of Lifford Hospital.

Mr John Hennessy advised that no decision has yet been made and there should be more certainty by the end of July

2010. He advised that as with all other counties, services are under review, under HIQA guidelines, the context of

quality and safety, nursing homes and staffing, moratorium, resignations and retirements. The HSE management

perspective is to retain frontline services, but there are questions regarding quality and safety. Mr Hennessy advised

that these elements are being monitored on a week by week basis.

General Manager - GUH

Councillor Tim Broderick asked for further details regarding the new Clinical Director, Dr O’Keeffe’s position and

stated that he felt information being provided regarding the Clinical Director’s role is contrary to what was originally

stated. Councillor Tony Ward advised that he posed a number of similar questions to Minister Mary Harney, at a

recent meeting and was advised that Roscommon County Hospital and Portiuncula Hospital will still get their own

budgets and be responsible for them, and that the reason Roscommon and Portiuncula were included in the remit of

the new Super Manager post was because it would attract a higher caliber of candidate.

Mr John Hennessy advised members that Dr O’Keeffe’s role is similar to the General Manager of Galway University

Hospitals; that he will report to the Regional Director of Operations; has a role in managing & apportioning budgets

within his area of being, i.e. Galway, Roscommon and Portiuncula hospitals and has responsibility for services within

Acute Hospitals and Continuing Care. Mr Hennessy advised that Roscommon and Portiuncula will retain their

Managers and have responsibility for budgets allocated to their respective Hospitals, as per normal practice.

Transfer of Staff to Psychiatric Services – Roscommon Mental Health Services

Cllr John Kelly referred to the transfer of staff to the Mental Health Services in Co Roscommon and requested an

update on the matter. Councillor Kelly highlighted that as there are current staff shortages, there are no psychiatric

nursing staff to redeploy, and asked if there are facilities to transfer staff across HSE areas, etc.?

Ms Catherine Cunningham advised that the HSE will meet all relevant stakeholders in Roscommon, Galway and Mayo

to look at mapping all current Mental Health staff in the services and consider options for redeployment and

reconfiguration. She advised that more information should become available in the next week or two.

Mr Francis Rogers advised that there are internal and national policies in relation to transfers and that the HSE have,

on occasion, facilitated cross area transfers. He advised that staff can and have been facilitated, and that once there

is agreement between the staff members involved, that cross-area transfers can take place.

Tearman Eanna

Councillor Catherine Connolly referred to the last position she received regarding Tearman Eanna whereby more

information would be forthcoming by the 3rd quarter.

Ms Catherine Cunningham advised that no final date has yet been agreed and that there are positive discussions

taking place.

It was agreed that Councillor Catherine Connolly would receive a further update in one week.

Brothers of Charity

Councillor Padraig Conneely referred to the cut back of funding to the Brothers of Charity and requested an update on

the matter.

Mr John Hennessy advised that discussions are ongoing between the HSE and the Brothers of Charity and that the

main objective is to retain frontline services.

Emergency Department – GUH, Changes in Recruitment of NCHDs

Ms Chris Kane and Mr John Hennessy advised members that there are currently no plans to reduce NCHD hours and

that there are some vacancies. All efforts to recruit are in place with the post being re-advertised.

Emergency Department – GUH

Councillor Padraig Conneely referred to the ongoing assessment, by the National Task Force, into Accident and

Emergency services at University Hospital Galway and questioned whether anything has changed or improved in

services since the Task Force began their work.

Mr John Hennessy referred members to the Briefing note circulated regarding the Emergency Department. He

advised that measures are being put in place to deal with A&E issues, in particular long delays, and advised that he

has had discussions with the new Clinical Director on the matter.

It was agreed that Dr O’Keeffe would be invited to attend the next meeting of the Regional Health Forum West

meeting.

Primary Care

Councillor Tony Ward requested any update in relation to Primary Care in Roscommon.

Mr John Hennessy agreed to arrange a position on the matter before the end of the meeting.

Questions and Responses from previous Forum Meetings

Members asked what the situation is regarding Responses to Questions, submitted to the three previous Forum

meetings, and for which it was agreed that written replies would be provided, now that the Work-to-Rule – Industrial

Action had now been suspended?

Members were advised that it may take up to two weeks to provide all of the outstanding written replies to the

Questions and that they were being collated at present.

The Senior Audiological Scientist post in Galway has been vacant since 2002. Numerous efforts have been made to fill this post since then but unfortunately, those attempts have proved unsuccessful, for what is an extremely scarce grade. Up until November 2009, we were providing a limited Audiological Scientist service for children on a sessional (contract) basis. This ceased when the Scientist could no longer provide the service, due to ill health. However, we have now secured the services of a Scientist from the HSE South, on a limited sessional basis. Clinics for children are held each Saturday in Galway since 22nd May 2010. The service will be reviewed at the end of June to ensure that the service is effective and provides value for money. In the meantime, Galway Primary, Community and Continuing Care (P.C.C.C.) will continue to seek a derogation from the moratorium for approval to recruit a directly employed Audiological Scientist. W28Q611 - Cllr Mary Hoade Can you outline properties in the ownership of the HSE in Galway City and County that are currently not in use? HSE Reply:

14 & 15 Barr Aille, Galway City - Vacant, shortly to be advertised for sale. 194 Castlepark, Galway City – may revert to City Council. Maam/Kilmilkin Health Centre, Co. W28Q612 - Cllr John Kelly Is there any question put down in advance of HSE meetings that management can actually answer in writing without the input of frontline staff who are involved in a work to rule? HSE Reply: Members will be aware that the Industrial Action is now suspended. The remaining backlog of unanswered questions is being dealt with expeditiously and replies will issue as soon as possible. W28Q613 - Cllr Declan Bree That the Forum be provided with a list of all private consultants, advisers and agency staff that have been engaged by the HSE in each department in each county in the region, indicating the specific function and project they are engaged on since the introduction of the public service recruitment embargo, and the costs involved? HSE Reply: This question is being considered and worked on. W28Q614 - Cllr Declan Bree What is the shortfall in funding for Sligo General Hospital in the current year and how is it envisaged that the Hospital Executive will manage the shortfall. HSE Reply: SGH is facing a Budget Deficit of €12m for 2010, this represents an enormous challenge for the hospital in maintaining services at current levels and to do so in a high quality, safe and patient focused manner. The Hospital has worked tirelessly over the past 18months or so via the Hospital Budget Working Group and in conjunction with the Specialities/Departments to put plans in place to address the budgetary shortfall. Sligo General hospital’s out-turn for 2009 was €117.5million. The Hospital’s budget allocation for 2009 was €112.4million. During the course of 2009 significant savings were targeted and achieved (outlined below) in order to minimise the out-turn deficit with a key focus on maintaining front line patient services. The savings amounted to €2.6m.

A range of areas were selected particularly focusing on Income and Non Pay etc. As part of the budgetary plan for 2009, we closed a number of beds, 72 during the course of the year and ceased elective surgery for the last two months of 2009. Notwithstanding these measures the hospital has been successful in meeting a range of HSE achievements/targets specifically around waiting lists for both inpatient and outpatients. This was only achieved through the commitment of all the staff across the hospital. In parallel with the targeted savings of 2009, reviews were undertaken in relation to a wide range of services and how we might reconfigure and provide the same service in a more effective and efficient manner and was overseen by the Hospital Budget Group which has met on a weekly basis. The measures to address the 2010 budget situation include actions across a range of headings, Pay Non pay and Income. Significant work has been completed by the hospital in terms of enhancing the management of sick leave absenteeism and supporting staff. The hospital also continues working on a range of areas to ensure that we maximise our efficiencies and savings in non frontline service areas. These include a multi-disciplinary Income Group which has introduced a range of initiatives which we expect will generate increased income from now until the end of the year. We have done this by way of reconfiguring private and semi private beds. We have also focused on Non Pay high spend areas and particularly in the areas of stock control and inventory and much work is being undertaken in this regard. The breakeven plan may have some impact on frontline but this will be minimised where possible. The proposed plan covers a wide range of areas to be addressed including reconfiguring existing services, reviewing all non pay lines including locum cover for staff, travel, no purchase of equipment for the remainder of the year, introduction of 5-day wards and the introduction of a Same Day Admission ward area to protect elective surgery. We are also reviewing our continued ability to provide services off site such as ENT, Dermatology, Ophthalmology and Neurology to Letterkenny General Hospital and other services to local peripheral hospitals, such is the crucial nature of the financial position and the requirement that the Hospital break even. In our break even plan our focus is on maintaining services in the current financial climate, minimising disruption to front line services and at the same time the hospital is striving to achieve the HSE activity targets, maximise day service usage and introduction of a Same Day Admission for Surgical specialties. Notwithstanding the reduced funding available both in 2009 and 2010 there have been many positive developments within the hospital. The Hospital is continuously looking at ways of enhancing and reconfiguring the services that we provide, to provide services in a more effective, efficient manner and with the ultimate aim of positioning the hospital for the future. W28Q615 - Cllr Declan Bree When will the planning and design work commence on the multi story extension to Sligo General Hospital announced by the Taoiseach in December, and in what month in what year will construction work on the extension commence, and how many beds will be provided in the new extension, and is it the intention to close the beds in the new extension in addition to the 62 beds currently closed in the hospital? HSE Reply: The extension to Sligo General Hospital has been approved for development as a Public Private Partnership (PPP). Work is advancing on the preparation of business case and other scoping information needed to support the cost benefit analysis required for a project of this scale. There are certain enabling works that must be dealt with before the main project can commence on site. Detailed design is in progress on the main advance enabling works - the ring road - to enable this to be tendered and completed in time for construction of the main project. We expect to submit a Planning application for the Ring Road round the end of July, 2010, and be in a position to go to tender at the end October, 2010. Construction of the Ring Road is scheduled to commence early 2011. Market Soundings are also being carried out to explore the procurement options for the construction of a multi storey car park to meet the increasing car parking requirements for the hospital including those associated with all planned developments. The lead in time to construction for the PPP, including preparation of public sector comparator, prequalification of bidders, bidding process, design of final solution and completion of financial arrangements is not likely to be less than three years.

In 2009 they received €466,004. The allocation of €466,004 in 2009 represented 50% of our total allocation to voluntary organisations for Older People of €933,029 during the year. The responsibility of the HSE is to develop services across the entire county and not just concentrate resources in one geographical area. In Ennis there are two specialist services for older people with dementia, one provided directly by the HSE and the other through funding provided to the Alzheimer’s Society. Given the need to take account of equity and access to services across the entire county, we do not view the development of a third specialized service in the Ennis area as being amongst our priorities. In the coming years the HSE will be looking to develop these services in both west and east Clare, which presently have no dementia day care service, rather than an additional one in the Ennis area. For a number of years since this project was being conceived the HSE has consistently advised the Board of Clarecastle Day Centre Ltd. that it was not in a position to provide revenue funding for their proposed stand alone dementia unit and that any consideration could only happen if new service monies became available. In an effort to assist them, the HSE worked with them to explore alternative arrangements that would allow for the new building to be commissioned in a phased manner over a number of years. In an attempt to progress a partnership arrangement between both organizations, the HSE suggested to them that they enter into a partnership arrangement with the Alzheimer’s Society who have considerable expertise in the delivery of dementia services and who are already operating a specialized dementia service here in Ennis. Unfortunately, this proposal was unsuccessful in reaching an agreement. The HSE has now reached agreement with the Alzheimer Society to develop a state of the art dementia-specific day care centre on the St Joseph’s Hospital campus. This development will serve to further the integration of services across care groups and between the hospitals and community. W28Q619 - Cllr Michael Mullins Can you please let me know when will the Audiological Scientist be appointed, that was promised to the representatives of the Galway Parents of Hearing Impaired Children, when they met with the HSE IN 2007? HSE Reply: The Senior Audiological Scientist post in Galway has been vacant since 2002. Numerous efforts have been made to fill this post since then but unfortunately, those attempts have proved unsuccessful, for what is an extremely scarce grade. Up until November 2009, we were providing a limited Audiological Scientist service for children on a sessional (contract) basis. This ceased when the Scientist could no longer provide the service, due to ill health. However, we have now secured the services of a Scientist from the HSE South, on a limited sessional basis. Clinics for children are held each Saturday in Galway since 22nd May 2010. The service will be reviewed at the end of June to ensure that the service is effective and provides value for money. In the meantime, Galway Primary, Community and Continuing Care (P.C.C.C.) will continue to seek a derogation from the moratorium for approval to recruit a directly employed Audiological Scientist. W28Q620 - Cllr Michael Mullins Can the Regional Director of Operations outline the process for consultation with all the stakeholders of Portiuncula Hospital, that will now be put in place involving the new Clinical Director, with a view to reaching agreement on the future development of the Hospital ?, and can I have a commitment to an early meeting with the Portiuncula Hospital Action Group?

HSE Reply: On the 3rd June 2010 Mr. John Hennessy, Regional Director of Operations for HSE West, announced that Dr. David O’Keeffe has been appointed as Clinical Director, Acute and Continuing Care Services for the Galway/Roscommon area. The new role encompasses the duties of the former General Manager post with additional responsibility for Portiuncula and Roscommon County Hospitals. Dr. O’Keeffe, a Consultant Radiologist from GUH, is a member of the Irish Medical Council and a former President of the Irish Hospital Consultants Association, with over 17 years as a senior clinician and contributor to the ongoing development of Irish health care. Welcoming his new role Dr. O’Keeffe paid tribute to his predecessor, “Under Bridget Howley, Galway University Hospitals flourished with major new developments including a designated Cancer Centre and the introduction of Cardiac Surgery. My hope is to build on the work done by Bridget and the management team, particularly those who worked on tirelessly after her retirement.” It is a sign of confidence that the HSE has in our hospitals that we have been chosen as the first HSE Hospital Group for clinical leadership in a management role. I am looking forward to working with the staff at GUH, Portiuncula and Roscommon and the College of Medicine, Nursing and Health Sciences at NUI Galway to build a strong, cohesive service for our patients with strong links to the community we serve.” We are in a different place now, economically and clinically with the introduction of quality standards that puts the patient, patient safety and clinical governance at the heart of everything we do. HSE Reply: The new Critical Care Block includes a significant increase in car parking on the site of the Mid-Western Regional Hospital, Limerick. The HSE also intends conducting a full Mobility Management Study for the site W28Q624 - Cllr Pádraig Conneely Due to a misdiagnosis of “missed miscarriages”, how many incidents have been recorded at UCHG, over the past 20 years, how many have contacted the Helpline in UCHG and what procedures are now in place to ensure that this will not happen again? HSE Reply: The HSE has been working with all 19 maternity facilities nationwide this week, to ensure that women with concerns about their care or treatment have access to information, support and reassurance. All hospitals have provided a dedicated response to women who attended their services who have a concern about their experience. All of these contacts will be followed up within the hospital, and where required, records will be looked at to ensure that the care provided and the diagnosis made was correct. The HSE will be supporting hospitals as required to ensure that hospitals provide all appropriate follow-up, support and care of those affected by the misdiagnosis of miscarriage. The HSE is committed to the early initiation of a review of these serious incidents to improve practice in the future. The HSE today announced details of the Miscarriage Misdiagnosis Review Team and its Terms of Reference. The Review Team is being chaired by an independent expert in Obstetrics and Gynaecology, Prof William Ledger, Vice President of the Royal College of Obstetrics in the UK who will be joined by Prof. Michael Turner, National Clinical Lead of the HSE's Obstetrics Programme and Ms. Sheila Sugrue, HSE National Lead Midwife. Service user representation in the management of this incident is being provided by Cathriona Molloy from Patient Focus. The Review Team will be fully supported by the HSE and will also have access to the input of specific expert advice in areas such as Radiology and Diagnostic Imaging Equipment, as they require it. Examine any cases previously known or now being identified where, in the last 5 years, drug or surgical treatment was recommended when the diagnosis of miscarriage has been made in error, and where subsequent information demonstrated that the pregnancy was viable

Investigate all such cases, to seek to determine the causes of and the responses to the cases, and make recommendations as required.

The 5 year timeframe has been agreed by the Review Team with a view to ensuring that the information gathered, subsequent report and recommendations can inform the Standards National Guidelines for Obstetrics, which are currently being developed, and to ultimately improve patient care in the future. However, the Review Team has indicted that in certain circumstances they will accept submissions from outside the 5 year timeframe, where the case can inform the work of the Review Team and the development of national standards. It is expected that the review will be completed within a six month timeframe. The Review Team's report will be presented to Dr. Barry White, National Director of the Quality and Clinical Care with the HSE, who has committed to publishing the report. Between last Wednesday 9thJune and 12noon today there have been a total of 240 callers to maternity hospitals around the country. To ensure that they can access information relating to their treatment as swiftly as possible, women are advised to contact the hospital where they received treatment. Contact details for each of the hospitals are now available on the HSE website, www.hse.ie and from the HSE Infoline on 1850 24 1850. A key change in practice in this area is already being implemented in all public maternity units. The HSE and the Department of Health & Children have advised all maternity facilities to immediately put in place measures to ensure that the decision to use drugs or surgical intervention in women who have had a miscarriage diagnosed must be approved by a Consultant Obstetrician. This action is being taken to ensure the safe management of early pregnancy loss across the country. A clinical programme for Obstetric care has been established as part of a major initiative by Dr. Barry White, HSE National Director of Quality and Clinical Care, to standardise care across a range of medical specialities. The Obstetrics programme will define standardised care for early pregnancy loss and other aspects of Obstetric care. This programme has recently been established and will be led by Prof. Michael Turner of the Coombe Women's and Infants University Hospital. As part of this work, a guidance document for the management of early pregnancy loss will be developed in conjunction with the Institute of Obstetrics and Gynaecology. Of over 70,000 births in Ireland each year, there are approximately 14,000 miscarriages. Miscarriage can be a very distressing experience for a woman and in a small number of cases counselling may be required.

Early Pregnancy Units have been developed to provide coordinated care for women experiencing pain or bleeding in early pregnancy. Early pregnancy care involves a multidisciplinary approach involving doctors, nurses, ultra-sonographers, midwives and support staff. Maternity units may alternatively assess patients with potential or actual miscarriage as emergencies via clinics, gynaecology wards, day-care, Emergency Departments or other mechanisms or facilities.

W28Q625 - Cllr Pádraig Conneely Why did a 94 year old Great Grandmother have to spend two nights on a trolley in A & E (Tues 8th & Wed 9th) and what procedures are in place to ensure that this will not occur again? HSE Reply: The ED was relatively quiet over the June Bank Holiday weekend, however, there was a surge in admissions on the Tuesday and Wednesday.

On the morning of Tuesday 8th June 2010 there were 12 patients awaiting admission from ED. With a large number of patients presenting to ED in the afternoon. On the morning of Wednesday 9th June 2010 there were 27 patients awaiting admission from ED. The Hospital’s Escalation Plan was put in place with patients being accommodated on the Surgical Day Ward and Elective Admissions deferred. In relation to this patient I wish to confirm as stated on the attached that the patient arrived in the ED at approximately 4pm on the 8th June 2010 and was transferred to the Surgical Day Ward at 22:45 on the 9th June 2010 and transferred to a bed on St. Mary’s Ward on the afternoon of the 10th June 2010. The Hospital has introduced a number of initiatives and is planning a number of others including:

- Opening of the Medical Assessment Unit - Direct Referrals of GPs to the Medical Assessment Unit - Facilitating contact to Senior Consultants by GPs - Proposed opening of Assessment Area for Haematology/Oncology/Radiotherapy Patients within the

next week - Proposed opening of Surgical Assessment Unit in the next few weeks - Increasing surgical pre-assessment capacity - Designation of Specialised Wards - Rapid access to Out Patients - Review of services for care of the elderly patients

W28Q626 - Cllr Catherine Connolly In view of the unanimous motion passed by the Health Forum in relation to the keeping open of St Francis Nursing Home-what decision/progress has been made in relation to making the required necessary physical changes to the building and what is the total number of beds currently empty/unoccupied and the waiting list of people waiting to get a bed. HSE Reply: The current number of residents in St. Francis CNU is 28 and there are no plans whatsoever to move these residents to any other nursing unit. The beds that have been closed to date are closed due to the moratorium on service recruitment. There are no plans to close St. Francis CNU, indeed we have just opened a Community Physiotherapy Department there to utilise the rooms. The long term viability of St. Francis CNU as a long stay unit for older people is being considered. We are looking at a range of alternative possibilities in this regard, including the use of the facility to provide enhanced Day Care, Respite Care or indeed Primary Care for the people of Galway. Staff at St. Francis CNU have been appraised of these discussions, as have their unions. W28Q628 - Cllr Catherine Connolly Please confirm a) the total number of Radiologists employed by Breast Check in Galway City and whether this is the full and required complement of Radiologists b) how many of those radiologists are currently not working for one reason or another giving the reasons c) What steps have the HSE West taken to fill the vacancies d) How many women are screened on a weekly basis giving the figures in relation to same on a weekly basis from April 2009 to date and e) What is the recommended and required number of screenings? HSE Reply:

A) BreastCheck employs three radiologists in the BreastCheck Western Unit. These are employed to provide a radiology service to all women living in the western area. Three radiologists is the full and required complement.

B) At present, two radiologists are on leave for personal reasons. One is due to return on 22 June 2010 and the other in September 2010.

C) To ensure the continued provision of the service to women living in the western area, the radiology service in the western area is supported by radiologists from the BreastCheck Eccles and BreastCheck Merrion Units.

D) It is the policy of the National Cancer Screening Service to share screening figures an annual basis in its BreastCheck Programme Report, once all statistics have been fully evaluated and verified.

E) BreastCheck offers breast screening to women in the western area from the BreastCheck Western Unit and three associated mobile digital screening units. Screening figures vary on a day to day basis as they are dependent on the number of women who attend their allocated appointment. Each mobile unit operates with a single mammography machine and two radiographers and is capable of carrying out approximately 40 mammograms per day. Invitation rates are set higher than this as women do not have to confirm they will be attending the appointment offered to them. Each static unit has three mammography screening machines and, on an ordinary screening day, has approximately three times the screening capacity of a mobile unit. The static unit holds the assessment clinics for all women re-called for further assessment following their initial screening mammogram therefore each static unit does not provide screening on at least two mornings per week. Screening capacity is also reduced at other times to accommodate procedures such as wire guided localisations, repeat biopsies etc.

W28Q631 - Cllr Laurence Fallon What effect will the decision to place Roscommon Community Services under the control of the new Super Manager in Galway, who will be covering not only Hospitals but also Community Services from Clifden to Ballyfarnon, due to the long term implications this will have in removing local decision making from Roscommon and which will see the amalgamation of Community Services with Acute Services which could result in funding being transferred from Community Services to Hospitals in contrast with the stated objectives of the HSE? HSE Reply: On the 3rd June 2010 Mr. John Hennessy, Regional Director of Operations for HSE West, announced that Dr. David O’Keeffe has been appointed as Clinical Director, Acute and Continuing Care Services for the Galway/Roscommon area. The new role encompasses the duties of the former General Manager post with additional responsibility for Portiuncula and Roscommon County Hospitals. Dr. O’Keeffe, a Consultant Radiologist from GUH, is a member of the Irish Medical Council and a former President of the Irish Hospital Consultants Association, with over 17 years as a senior clinician and contributor to the ongoing development of Irish health care. Welcoming his new role Dr. O’Keeffe paid tribute to his predecessor, “Under Bridget Howley, Galway University Hospitals flourished with major new developments including a designated Cancer Centre and the introduction of Cardiac Surgery. My hope is to build on the work done by Bridget and the management team, particularly those who worked on tirelessly after her retirement.” It is a sign of confidence that the HSE has in our hospitals that we have been chosen as the first HSE Hospital Group for clinical leadership in a management role. I am looking forward to working with the staff at GUH, Portiuncula and Roscommon and the College of Medicine, Nursing and Health Sciences at NUI Galway to build a strong, cohesive service for our patients with strong links to the community we serve.” We are in a different place now, economically and clinically with the introduction of quality standards that puts the patient, patient safety and clinical governance at the heart of everything we do. Resources may be limited but I am confident that if we work together, draw from our collective pooled experience and skills, we can meet the challenges common to all of the hospitals across the hospital group.” Mr John Hennessy, RDO, outlined that this appointment was extremely important for the people of Galway and Roscommon. tion Late Question W28Q633 - Cllr Austin Francis O’Malley Could you clarify the reasons for the closure of the Psychiatric Day Care Unit at Scared Heart Hospital, Castlebar. I would strongly urge the reopening of the unit as I believe the closure affects up to 600 patients. The closure will have a devastating effect on some of the most vulnerable people in our society. Could you please respond, if possible with an answer in writing. HSE Reply: The services at the Day Centre at the Sacred Heart ceased to operate in December 2009. As part of an overall review of adult mental health services in Mayo, a new consultant-led rehabilitation team is being developed for County Mayo. The service users of the Day Centre at the Sacred Heart Hospital have been clinically reviewed and are now being cared for under the new rehabilitation team’s outreach service or other existing services in Castlebar.

Councillor Hoade required further clarification regarding estimated bed closures at UCHG for the year end, and highlighted

a situation where a pregnant woman recently spoke on local radio about there being no beds in her local hospital, and

whereby a local hotel offered her a bed?

Mr John Hennessy advised that there was no correlation between bed numbers and performance & activity. He advised

that plans include more out-patient activity and more emphasis on day care, which is a far more efficient use of resource.

Mr Hennessy also drew members’ attention to the Briefing Document circulated regarding the Emergency Department at

UCHG, which outlined measures in place and plans for the future.

Mr Hennessy advised that he would be happy to follow up on the specific case mentioned.

Question No. W28Q610 Councillor Mary Hoade

Councillor Hoade requested confirmation regarding the advertisement of the Senior Audiologist post, as she had been

previously advised that the moratorium would not affect speciality posts, such as this one?

Ms Catherine Cunningham advised that this post was advertised on 5 occasions unsuccessfully and although there is

flexibility under the moratorium, the HSE will now have to look to other HSE areas to have this post filled.

Ms Cunningham agreed to ascertain when the post was last advertised and revert to all Forum members on the

matter.

Councillor Hoade asked if there were any proposals to sell the vacant premises listed and highlighted a premises in

Turloughmore, Co. Galway which has been vacant for 10 years.

matter.

West, on the matter, and that she will confirm for the members where exactly the proposed Dementia Day Unit will

be located at St Joseph’s Hospital.

It was also agreed that Local HSE Management would arrange a briefing for local Public Representatives and any

other interested parties.

Would like to see the new Clinical Director meet with all relevant stakeholders, including Portiuncula Hospital Action Group

to discuss the future of the Hospital.

Mr Hennessy advised members that the Clinical Director had arrangements to meet with Portiuncula staff and Unions, in

the coming weeks and that he could envisage no problem with arranging a similar meeting for all other stakeholders.

Members were advised that any individual cases could be brought to the attention of the Office of the Regional

Director.

Mr John Hennessy agreed that the incident, whereby a 94 year old Great Grandmother would have to spend two nights on

an A&E Trolley, was unacceptable and expressed an apology to her and her family for having such a regrettable

experience. Mr Hennessy drew attention to the ED Update, circulated at the meeting, outlining preventative measures

and initiatives now being introduced at the hospital.

HSE West have cut back on funding to the Brothers of Charity Services for the past 3 years. The BOC have carried a core

funding deficit of £500,000 since 2006. The effect of this is a shortfall of 2.5million this year, which will lead to severe

cutbacks in front-line services with the closure of two community homes and cutting respite services by 45% among many

other cost cutting measures. All of the 4 HSE regions got a cut of 2% for the VFM and no funding for staff increments. In

addition to this the HSE West took the exact funding for all the posts reported retirements or posts that became vacant in

2009 - the Moratorium Cut in HSE West 20 posts or £838,000. This did not happen in the other areas of the HSE. This is

a most unfair and indiscriminate cut as it affects most front line positions. I request the board to reverse this decision on

Moratorium.

with a breakdown between adults and children and further that this Forum is given the precise number of patients from

these lists who have been seen under the National Treatment Purchase Fund since it’s inception to-date and the hospitals

where they have been seen/treated.

fact that there is no follow up surveillance mammogram service now available in Sligo General Hospital, this Health Forum

expresses its outrage at the failure of the Minister and the NCCP to adhere to their commitments and calls on the Minister,

the Government and the National Cancer Control Programme to provide a follow up surveillance mammogram service in

Sligo General Hospital without further delay.

HSE West to confirm and clarify to this Forum what meetings have taken place with staff and/or patients and their families

about the closure despite the Forum’s wishes and further that the HSE West clarify the number of beds occupied out of

the total number of beds in the unit and also confirm to this Forum that the Unit will remain open given the express wishes

of the Forum and the people of Galway and further to take all necessary steps as outlined in the recommendations of the

HIQA report on the physical aspects of the building.

That the HSE West Forum condemns the failure of the HSE to date to clarify and now requests immediate clarification on

the implications of the appointment of a super-manager on services at both Roscommon and Portiuncula Hospitals; the

future of the plans for a joint department of surgery between both hospitals and the failure to provide funding for the

upgrade of sterilisation facilities to ensure additional procedures could be performed at Roscommon County Hospital.

Also, that the HSE Forum West condemns any plans for the relocation of A&E services from Roscommon and Portiuncula

Hospitals to UCHG thereby placing lives in danger.

299/28/10 Any other Business/Aon Ghnó eile Councillor Brian Meaney raised a query in relation to regulation of Shannon – doc, in particular regarding any

misdiagnosis by attending medical staff.

Mr Bernard Gloster advised that there are a number of options in relation to lodging any concerns or complaints.

As Shannon – doc is a co-operative of GPs an individual can raise any specific medical care dissatisfaction with the

Medical Council.

An individual can also raise any concerns regarding practice with the local Manager, Mr Larry Maher

Concerns can be raised with Shannon – doc’s Executive Board, and

Complaints can also be lodged via the HSE Complaints process.
