Initial Formulary Management

Select drugs on the basis of need

First line drugs: 1) angiotensin converting enzyme inhibitors (ACEIs) 2) angiotensin receptor blockers (ARBs), 3) calcium channel blockers (CCBs) หรอ 4) thiazide

Two possible interpretations for communicating to patients:

Lowering systolic BP to 120 mmHg or below for more than 3 years

1) Reduce the risk of CV events by 25% and risk of death by 27% while

increasing the risk of adverse events merely from 2.5% to 4.7%.

2) Reduce CV events from 7 out of 100 to 5 out of 100 (or 0.54% per year)

with no benefit at preventing stroke or heart attack, while increasing the

risk of hypotension, syncope, electrolyte abnormalities, and AKI by 88%.

• S-CKD-09: แนะน าไมใหซอยากนเองโดยไมจ าเปน สมนไพร อาหารเสรม และยาทางเลอกอนๆ

กลมยาทควรหลกเลยงในผปวยโรคไตเรอรง รายการยาทควรหลกเลยง ระยะของโรค เหตผล

1. ยาลดความดนเลอดทยบยง RAAS ทกระยะ ในกรณทสงสย

renal artery stenosis

fatal hyperkalemia และไตวายเฉยบพลน

2. ใชยาลดความดนเลอดทยบยง RAAS รวมกนหรอรวมกบ K-sparing diuretics

ทกระยะ fatal hyperkalemia และไตวายเฉยบพลน

3. ยาขบปสสาวะกลม K-sparing diuretics ระยะท 4 ขนไป ประสทธผลตา และ hyperkalemia

4. ยารกษาเบาหวานกลม sulfonylurea ทขบทางไต และกลม a-glucosidase inhibitor

ระยะท 3 ขนไป ทาใหเกดนาตาลในเลอดตา และเพมความ

เสยงจากผลขางเคยงของยา

5. ยารกษาเบาหวาน metformin ระยะท 4 ขนไป หรอระยะท 3b ทมความเสยง

เพมความเสยงตอการเกด lactic

acidosis

6. ยากลม NSAIDS และ COX-2 inhibitor ระยะท 4 ขนไป ไตวายเฉยบพลน นาและเกลอคง

7. ยาระงบปวดกลม opioids ระยะท 4 ขนไป neurotoxicity ชก

8. ยาปฏชวนะ nitrofurantoin ระยะท 3b ขนไป irreversible peripheral neuropathy 9. ยาในกลม oral sodium phosphate และ Fleet Phospho-soda

ระยะท 3 ขนไป ทาใหเกดไตวายเฉยบพลนจาก acute

phosphate nephropathy

10. ยาในกลม Bisphosphonate ระยะท 4 ขนไป เสยงตอการเกดภาวะไตเสอมเพมขน

กลมยาทควรระวงในผปวยโรคไตเรอรง ยาทพงใชดวยความระมดระวง เหตผล สงทควรท าเพอใชตดตามในการใชยา*

ยาลดความดนเลอด - กลม ACE และ ARB - ลดการขบ potassium ทางไต

- ลด GFR จากการเปลยนแปลงใน renal hemodynamics (efferent arteriole dilatation)

- ตดตาม K และ SCr ท 2-4 สปดาห และหยดยาเมอพบ K สงกวา 5.5 mEq/L หรอ SCr เพมขนเกน 30% โดยไมมสาเหตอน

- เรมดวยขนาดต า ในโรคไตระยะท 3b ขนไป - หลกเลยงยาทอาจเพม potassium ในเลอด

- Beta blockers กลม hydrophilic เชน acebutolol, atenolol, nadolol, sotalol

- เสยงตอการคงของยา และผลขางเคยง โดยเฉพาะภาวะ หวใจเตนชา heart block

- ปรบลดขนาดยา 50% ในโรคไตระยะท 4 - เลอกใชยาอนแทน เชน carvedilol, labetalol,

metoprolol หรอ propranolol ยาปฏชวนะ และยาตานเชอ1 - Aminoglycoside - ท าใหเกดไตวายเฉยบพลน และ

เสยงตอการเกดภาวะไตเสอมเพมขน

- ปรบลดขนาดยา ใชแบบ extended interval - หลกเลยงปจจยเสยงทท าใหเกดพษตอไตงาย - ควรตดตาม SCr ตรวจหาระดบยา (ถาท าได)

- Amphotericin B - ท าใหเกด AKI, metabolic acidosis, hypokalemia

- ปรบลดขนาดยา ในผปวยโรคไตระยะท 3 ขนไป หรอเลอกใชยาอนแทน

- Acyclovir และ Valacyclovir - เกด neurotoxicity ความรสก ตว พฤตกรรมเปลยนแปลง ชก

- ตกตะกอนใน renal tubule

- ปรบลดขนาดยา 50-75% ในโรคไตระยะท 3-5 - ตรวจปสสาวะ ถาสงสยม crystalluric tubular

damage

ตวชวดส าหรบ RDU ในผปวยโรคไตเรอรง

CKD 01 ผปวยโรคไตเรอรง ทเกด fatal hyperkalemia หรอไตวายจนตองไดรบการบ าบดทดแทนไต จากการไดรบยาในกลม ACEI รวมกบ ARB หรอยาอนทมผลตอระดบ potassium ในเลอด

CKD 02 ผปวยโรคไตเรอรง ระยะท 4 ขนไป ทไดรบยา metformin

CKD 03 ผปวยโรคไตเรอรง ทไดรบยาในกลม NSAIDs โดยไมมขอบงช หรอไดรบเกน 14 วน

ต ากวารอยละ 10 ของผปวยโรคไตเรอรงทงหมด

First line drugs: Metformin เมอไมมขอหาม ไดแก • ผทม estimated GFR < 30 มล./นาท/1.73 ม2 หรอ • ผปวยทเสยงตอ lactic acidosis ไดแก การท างานของตบ หวใจ ปอด

เสอมลงมาก ความดนเลอดต า

อาจให aspirin 75-162 มก. OD ในชายอาย >50 ป หรอหญงอาย >60 ป หรอผทม 10-year Thai CV risk score >10% ถาไมมขอหาม

หลกเลยง glibenclamide ในผสงอาย หรอผม eGFR < 60 มล./นาท/1.73 ม2

หามใชยากลม sulfonylurea รวมกบยากลม glinide (repaglinide) หามใช pioglitazone ในผมประวตภาวะหวใจลมเหลว NYHA class 3-4
