Step 1. Decide upper occlusal wax rimCheck the maxillary occlusal plane of wax rim with occlusal fox plate. Anteriorly, the occlusal plane was parallel with the interpupillary line and posteriorly, the occlusal plane was parallel with Camper's line.

█Neutral zone technique 小簡介 ● 早在1931年，Sir Wilfred Fish 首先提出了polished surfaces 對於假牙retention 和stability的影響。

● 他也表示假牙應該如何建立在這個所謂的“dead space” 當中 , 這也就是之後我們所指的 “neutral

zone – 中立區”。

● 之後更有如Beresin and Schiesser等學者提出，人工牙齒應該排列在這“中立區”內，達到假牙在口腔

內一個平衡的狀態，維持其穩定性。

█Neutral zone 概念 ● 當所有的自然牙喪失後，口腔會存有一個潛在性的假牙空間，這個空間是藉由舌頭往外推擠的力

量和臉頰往外推擠的力量來使假牙可以達到一個互相制衡穩定的狀態。

● 對於ridge嚴重atrophy，臉部或舌頭muscle tone大 ，甚至像是CA開過刀顎骨被切除的病人， 都是

neutral zone的indication。

neutral zone technique 的適應症？若這個case不做neutral zone technique，用其他方法會有何差別？

適用於atrophic ridge、muscle tone 比較強、tongue比較大、有神經肌肉方面問題的病人。有些癌症的病人齒槽骨有被切除也可以利用

neutral zone technique。此病例未以傳統方式製做全口假牙，所以沒有去比較兩者之間的差

異。

1. 如何決定centric position，上下顎關係如何決定？為何先molding後才決定VD並取bite，而非先決定VD才Molding，有何原因？

2. green compound 適合作為Molding的材料嗎？要如何判斷 molding時，哪些是多的compound，哪些是molding出來的？

1. 我們先決定上顎的wax rim，先決定病人以後的美觀，之後lower compound rim放入口中，利用吞嚥方式取得他的VD；如果先決定VD再molding也是可以的。

2. neutral zone所使用的材料大不同, 有使用soft liner、compound⋯.等。我們使用green compound 的原因在於它的軟度不至於太軟, 有些病人muscle tone大的時候使用太軟的材料很容易變形崩解， 加上此材料是可以重複使用的, 所以方便反覆check。Molding finish後，通常會在buccal side再打上mobile material(soft liner etc.)作為recheck，必要時再調整。多的compound，會讓病人在function時變的不穩定，可用以上recheck方法做調整。此病例病人之後的追蹤回診情況都還滿理想的。

病人呈現Class III的malocclusion，lower排牙又較buccal，且上下顎occlusion又排成normal，那upper在使用上有沒有問題？

upper CD使用上是很穩定的，整體來說upper的retention還是比lower好。此病例lower molding出來較偏buccal，但upper posterior的牙嵴比較寬加以選用較小顆的上顎牙齒，且上下牙 水平差距並不大，

所以義齒還是可以排在ridge上方；但若差距很大時，可能必須排成lingualized occlusion 或是cross-bite。

為何選擇balanced occlusion，有無考慮其他occlusion？

選擇balanced occlusion，穩定性比較好。 也可以考慮使用零度牙排成Monoplane occlusion ，可排成cross bite，且stability也很好，對於牙 條件比較差的病例可考慮使用。

若排成monoplane，前牙要如何set up？packing後要如何調整？

可以排成edge-to-edge. 通常只會有不到0.5 mm的誤差，可以用砂紙在咬合器上或直接在病人口內做調整。

使用 neutral zone technique的方法，在病人retention 比較差的條件下，functional movement 時夠stable嗎？

對於這個case追蹤後的使用上都還算滿意, 如果真的可以試試用假牙黏著劑輔助。

upper occlusal rim的寬窄，在swallow時會限制tongue的活動範圍，如此會影響lower neutral zone嗎？

1. Upper occlusal rim寬窄對前牙區域的影響：正常吞嚥時，tongue應頂著palate，不會碰到upper occlusal rim 前牙區域，所以upper occlusal rim 前牙區域之寬窄，對tongue影響較不顯著， 但它可能會影響upper lip位置，及可能影響lower lip在做吞嚥或噘嘴時之位置，進而影響lower neutral zone。

2. Upper occlusal rim 寬窄對後牙區域的影響：Upper & lower occlusal rim後牙區域內緣的位置，與tongue運動的範圍息息相關，所以tongue會影響upper & lower neutral zone範圍。但因upper CD後牙的排列須考慮支點及槓桿原理，為避免咀嚼時rocking，大多盡可能排在ridge上方。而lower CD retention 較差，盡可能排在neutral zone以內為佳。
