主 訴： 診所轉診至本院治療牙周病，進而希望作 復治療之評估。
醫療病史： 否認有系統性疾病及過敏病史，有重聽。

Q： Telescope denture設計的原理和一般傳統活動假牙是不同的，它有點類似一個cantilever的設計，尤其是在distal free end中，那該如何克服這種狀況，在臨床上如何設計讓它的受力可以傳到牙齒長軸(long axis)上？
A： 有考慮major connector設計成posterior strap，在穩定度上會比較好，但因為病人於posterior strap的設計感到比較不舒適，因此沒有採用這種設計，改以將比較前方的strap的寬度加寬，以增加denture的stability，bridge type的部分則是有做一些強化的結構增加剛硬性(rigidity)。
Q： 這個案例觀察了一年還不錯，但當有一些牙齒有動搖的時候，該如何處理？
A： 這類的案例是需要密集追蹤其穩定性的，在追蹤時較常發現的是假牙喪失穩定性(stability loss)而造成牙齒的傷害。若在追蹤過程發現有牙周韌帶變寬(PDL widening)等受力過大的徵兆，我們可以藉由調整內外冠間的固持力(retention)來減少該abutment受力的情形，對於受力過大的牙齒的retention做一些調整。
Q： 所謂的調整retention是將retention降低還是增加？
A： 降低。

Q： 臨床上怎麼做？
A： 在外冠的內部做一整圈的修磨，使其收據度(taper)大一點。
Q： 這樣處理後會不會使外冠和內冠的密合度(fitness)會變得不好？
或是是否要考慮增加假牙的穩定性；假牙的穩定性不好會不會是來自於它的牙 吸
收(ridge resorption)？
A： 是的。若假牙穩定性不佳，會先檢查假牙和牙嵴間的密合度(fitness)。 若有 密合度不佳的情形，reline是必要的。
Q： 若13出問題，請問後續該如何處理？ 因為若13喪失，則剩餘牙齒的排列就成了一直線。
A： 會考慮在前牙的地方加個鈎靠(rest)，若13拔除，則考慮重做，可能再加個植牙，前牙的部分在重做時會考慮將前牙splinting並加個附連體(attachment)來解決固持性(retention)和穩定性的問題。
Q： 目前這個case的設計是23, 25, 28做成 te lescope，但是臨床上28的修型(preparation)是有困難的，有沒有考慮28不做telescope，改以車個鈎靠(rest)的方式？
A： 當初是考慮多一個telescope abutment可以增加一些retention，且因為7是喪失的，將28做為telescope abutment除了可提供retention、支持力(support)，還可以藉由假牙的splinting防止28向下長的情形。
Q： 請問telescope denture的存活率(survival rate)和舒適度與傳統的局部活動假牙有什麼差別？
A： 我查到的資料是沒有比較舒適度的資料，但就以臨床經驗上來看，病人得舒適度和使用上咀嚼的狀況都是不錯的。存活率也無查到相關資料。
Q： 如果46不做crown單純做enameloplasty是否可以？
A： 因為X光片和模型上分析結果，如果要將46和45間的咬合平面打平，可能會修到牙髓，因此還是決定先做intentional endodontic treatment，再做個牙冠。
Q： 請問abutment是否有做活性測試？
A： 因為修型時沒有什麼特別的狀況，所以這部分是略過的。
Q： 如果37因為牙周的問題而要先拔除的話，還會選擇28做telescope abutment嗎？
A： 會。28狀況不錯，且多一顆牙齒來支撐，假牙的survival rate會增加，所以還是會留。
註：這個病例的設計應該名為Konus telescope較為恰當，它是一種rigid support的設計。Konus並非cantilever，黏膜也有一起參予support，其重點為長期密切的追蹤其牙 吸收的狀況。因為其為rigid support，所以mucosa也占了很重要的地位，所以free end越長是越好的。若28設計成一般的rest，則rest可能會干擾Konus telescope的retention的產生，因為Konus telescope靠的是咬力使內外冠間密合度增加以增加摩擦力來提供retention，而rest則是會阻擋假牙向下的力量，這兩者的作用機制是不同的。另外，28設計成內冠也可得到secondary splinting的效果。
