Types of CMs
2
Overview1-5
Ischemic CM*
LV dysfunction as a result of a
chronic lack of oxygen due to CAD • Dilated CM (Mostly idiopathic CM)
• Restrictive CM
• Hypertrophic CM
• Arrhythmogenic non-ventricular CM
• Hypertensive CM*
Non-ischemic CM
Myocardial disorder in which the
heart muscle is structurally and
functionally abnormal
Not related to CAD
Other CMs
Do not fit well into the general
classifications
• Stress-induced ("Takotsubo") CM
• Chemotherapy-induced CM
• Peripartum CM
• LV non-compact
• Ischemic CM
*As per AHA and ESC classification (under the auspices of the Working Group on Myocardial and Pericardial Diseases), the cardiomyopathies were classified to specify only non-ischemic CM and excludes ischemic CM (myocardial involvement
secondary to coronary artery disease), systemic hypertension, and valvular and congenital heart disease.
CM, cardiomyopathy; CAD, coronary artery disease; LV, left ventricle.

Defined as LV systolic dysfunction1,2
Pathophysiological Mechanisms
• Include depletion of ATP
• Reduction in the sensitivity to calcium and oxygen-free radical
generation
• Resulting in abnormal function of the sarcoplasmic reticulumMyocardial
infarction
CAD, coronary artery disease; LV, left ventricle; HFrEF, heart failure with reduced ejection fraction; IHD, ischemic heart disease; MI, myocardial infarction.

Defined as LV systolic dysfunction with one or more of the following:
• a history of prior myocardial revascularization or MI
• >75% stenosis in the left main stem or left anterior descending artery
• ≥2 vessels with a >75% stenosis
• A spectrum of pathophysiological states, ranging from myocardial stunning, hibernation and
scarring

beneficial effects of ACEIs, ARBs and MRAs1
• Benefits of β-blockers indicate that the SNS also plays a
key role1

Background
• Myocardial infarction (MI) and heart failure (HF) are closely linked; myocardial damage typically results in left
ventricular systolic dysfunction that ultimately leads to symptomatic HF with reduced ejection fraction (HFrEF)1
• HFrEF and MI also share many of the same treatments; ACE inhibitors and ARBs have been shown to reduce the risk
of mortality, HF hospitalization and MI in both patients with HFrEF after acute MI or in high-risk patients after MI2-4
• Sacubitril/valsartan has demonstrated superior efficacy in reducing the risk of CV death and HF hospitalization when
compared to an ACE inhibitor, enalapril in patients with chronic HFrEF5
Objective
• The current analysis of the PARADIGM-HF study examined the effect of sacubitril/valsartan, compared with enalapril,
on coronary outcomes

The post-hoc coronary composite outcome was created to:
▪ examine the effect of sacubitril/valsartan on the risk of coronary artery disease-related events, as opposed to HF-related events
▪ evaluate whether neprilysin inhibition may reduce the risk of events caused by myocardial ischemia and athero-thrombosis

Upcoming study: PARADISE-MI
Prospective of ARNI versus ACEI trial to DetermIne Superiority in reducingheart failure Events after Myocardial Infarction
Primary objective:
To demonstrate that sacubitril/valsartan is superior to ramipril in delaying the time to firstoccurrence of the composite endpoint of CV death, HF hospitalization or outpatient HF*Secondary objectives:
To compare sacubitril/valsartan with ramipril on:• delay in time to the first occurrence of CV death or HF hospitalization• delay in time to new onset of symptomatic HF (time to the first occurrence of HF
hospitalization or outpatient HF*)• delay in time to the first occurrence of CV death, non-fatal spontaneous MI, or non-fatal
stroke; reduction in the rates of the composite endpoint of CV death and total (first andrecurrent) hospitalizations due to HF, non-fatal spontaneous MI, or non-fatal stroke
• delay in time to all-cause mortality• safety and tolerability

a. Triệu chứng = NYHA Class II–IV; b. HFrEF = LVEF <40%; c. Nếu không dung nạp/chống chỉ định ACEi, dùng ARB; d. Nếu không dung nạp/chống chỉ định MRA, dùng ARB; e. Với nhập viện do suy tim trong vòng 6 tháng cuối hay tăng natriuretic
peptides (BNP > 250 pg/mL hay NTproBNP > 500 pg/mL ở đàn ông và 750 pg/mL ở phụ nữ); f. Tăng natriuretic peptide huyết thanh (BNP ≥ 150 pg/mL hay NT-proBNP ≥ 600 pg/mL, hay nếu nhập viện do suy tim trong 12 tháng gần nhất BNP ≥ 100
pg/mL hay NT-proBNP ≥ 400 pg/mL); g. Ở liều tương đương enalapril 10 mg 2 lần/ngày; h. Nhập viện do suy tim trong vòng 1 năm; i.

của điều trị can thiệp cấp cứu (giảm tử vong giai đoạn cấp)
