• ในUSAพบวาการสูบบุหรี่สาเปนสาเหตุสําคัญที่ทําใหประชากรเสียชีวิตกอนกําหนดถึงประมาณ 390,000 คนตอป คิดเปน 1 ใน
6 ของประชากรที่เสียชีวิตทั้งหมด เนื่องจากในควันบุหรี่ที่มีสารพิษที่กอใหเกิดโรคตางๆมากมาย
• และผลกระทบที่เกิดจากสารในควันบุหรี่ อาจเกิดจากการทําปฏิกิริยา
โดยตรงกับผนังเยื่อบุทางเดินหายใจ หรืออาจปะปนกับน้ําลายลงสู
ทางเดินอาหารและถูกดูดซึมเขารางกาย หรือเกิดจากการดูดซึมผานถุง
ลมภายในปอดเขาสูกระแสเลือดและสงผลกระทบตออวัยวะอื่น

โรคตางๆที่เกี่ยวของกับการสูบบุหรี่
• การสูบบุหรี่เปนปจจัยที่สําคัญที่ทําใหเกิดโรคกลามเนื้อหัวใจตาย
(myocardial infarction) เนื่องจากมีสวนสําคัญตอการเกิด
ภาวะหลอดเลือดแข็ง(atherosclerosis)ในหลอดเลือด
coronary โดยเฉพาะพวกที่มีความดันโลหิตสูง ไขมันในเลือดสูง

• นอกจากนี้ยังเพิ่มความเสี่ยงตอการผิดปกติของหลอดเลือดบริเวณอื่น เชน aortic aneurysm หรือ Buerger disease
• Buerger disease คือโรคที่มีการอุดตันหลอดเลือดเปนสวนๆ จากลิ่มเลือดรวมกับมีการอักเสบรวมดวย มักพบในชายอายุประมาณ 35 ป และมักเกิดกับหลอดเลือดแดงขนาดเล็กและขนาดกลาง

• มะเร็ง ในควันบุหรี่พบสารที่เปนcarcinogen และ tumor promoterจํานวนมากจึงพบวามีมะเร็งหลายชนิดที่สัมพันธกับการสูบบุหรี่
• มะเร็งปอดและมะเร็งในชองปากเปนกลุมที่สัมพันธกับการสูบบุหรี่มาก
ที่สุดเนื่องจากเปนบริเวณที่สัมผัสกับควันบุหรี่โดยตรงและสัมพันธกับ
ปริมาณบุหรี่ที่สูบ
• มะเร็งหลอดอาหาร กระเพาะปสสาวะ ไต ตับออน(มากกวาคนทั่วไป 5 เทา) มะเร็งปากมดลกู มะเร็งกระเพาะอาหาร

• โรคหลอดเลือดสมอง เกิดเปนหลอดเลือดตีบหรือแตกก็ได
• โรคระบบทางเดินหายใจเรื้อรัง
Chronic pulmonary disease(COPD) เกิดจากการทํางานมากขึ้นของ protease enzyme และยับยั้งการทํางานของ α1‐antitrypsin ทําใหเกิดการทําลายถุงลมเกิดเปนถุงลมโปงพองในที่สุด
• แผลเรื้อรังในกระเพาะอาหาร

• การสูบบุหรี่กับการตั้งครรภ พบวาในมารดาที่ตั้งครรภและสูบบุหรี่มักจะ
พบวาทารกในครรภจะมีขนาดเล็กกวาปกติ(small for gestational age)และมีน้ําหนักแรกคลอดนอยกวาทารกทั่วไป
• นอกจากนี้ยังพบวามีผลตอการพัฒนาการของทารกในอนาคตอีกดวย
• เสี่ยงตอภาวะรกลอกตัวกอนกําหนด(abruptio placenta)
รกเกาะต่ํา(placenta previa)
น้ําเดินกอนกําหนด(premature rupture of membrane)
การดื่มสุรา
• เอทิลแอลกอฮอล หรือ เอทานอล เปนสารที่ใชกันบอยทีสุด
• ทําใหเกิดการเปลี่ยนแปลงกับอวัยวะตางๆ
ตับ fatty change (reversible injury) acute alcoholic hepatitis (reversible injury) alcoholic cirrhosis (irreversible injury) กระเพาะอาหาร ทําใหเยื่อบุไดรับบาดเจ็บและการดูดซึมของสารอาหารมีความผิดปกติ เชน thiamine deficiency

การดื่มสุรา
• ระบบประสาท เกิดการเสื่อมสลายของเซลประสาทและการฝอของสมองนอย (cerebellar atrophy)
• Wernicke syndrome คือกลุมอาการผิดปกติของการเรียนรู และความผิดปกติของการกลอกตา(opthalmoplegia) ลูกตากระตุกผิดปกติ(nystagmus) แตถาเกิดความผิดปกติของการจดจําอยางรุนแรงจะเรียกวา Korsakoff syndrome
• ระบบไหลเวียนโลหิต ทําใหเซลกลามเนื้อหัวใจเสื่อมสลายทําใหหองหัวใจขยายตัวและการหดตัวของกลามเนื้อหัวใจผิดปกติ ทําใหเกิดภาวะ cardiomyopathy

การดื่มสุรา
• ระบบกลามเนื้อ ทําใหกลามเนื้อออนแรง
• ระบบสืบพันธุ ทําใหอัณฑะฝอ และเพิ่มความเสี่ยงตอการแทงในผูหญิง
• Fetal alcohol syndrome มีความผิดปกติของทารกคือ
microcephaly, facial dysmorphology, mental retard and abnormal in genitourinary system
• มะเร็ง เกิดจากacetaldehyde เปน tumor promotor • ระดับเอทานอลในเลือดมากกวา 50 mg% ถือวา เมา ตามพรบ.

ยารักษาโรค
• ยาเม็ดคุมกําเนิดและฮอรโมนทดแทนในสตรีวัยหมดประจําเดือน
เอสโตรเจน เปนฮอรโมนซึ่งใชในการคุมกําเนิดหรือเปนฮอรโมนทดแทน
ในสตรีวัยทอง ผลกระทบของการใชยาคุมกําเนิดเกี่ยวกับความเสี่ยง
ของการเกิดมะเร็งเตานมจากขอมูลปจจุบันยังไมพบความสัมพันธแต
อยางใด
ยาเม็ดคุมกําเนิดและฮอรโมนทดแทนในสตรีวัยหมด
ประจําเดือน
• Venous thrombosis โดยเฉพาะยาที่มีเอสโตรเจนปริมาณสูง • Liver tumor มักพบในสตรีสูงอายุที่ใชยาคุม เปนเนื้องอกชนิดที่ไม
รายแรง คือ hepatic adenoma
• HRT มีประโยชนมากในการปองกันโรคกระดูกพรุน ลดอาการอันเกิด
จากวัยทอง(menopausal syndrome)แตอาจเกิดผลแทรกซอนไดคือ venous thrombosis,CVD,cholecystitis
มะเร็งเยื่อบุโพรงมดลูก(endometrial CA)มักพบในรายที่ใชเอสโตรเจนเพียงอยางเดียว

• Acetaminophen ‐ การใชยาในปริมาณสูง จะทําใหเกิดการตายของเซลลตับ เรียกวา
hepatic necrosis นอกจากนี้บางรายจะพบการทําลายของเซลลในไตและกลามเนื้อหัวใจดวย
Therapeutic drugs(cont.) Aspirin • Overdose
– respiratory alkalosis followed by metabolic acidosis that may be fatal
• Chronic aspirin toxicity (salicylism) – headache, dizziness, ringing in the ears (tinnitus), mental confusion, drowsiness, nausea, vomiting, and diarrhea
• Analgesic nephropathy (renal papillary necrosis) • Erosive gastritis is a major cause of GI bleeding • May be implicated in Reye syndrome (fatty liver with encephalopathy) in children < 15 years old, especially with influenza and chicken pox
Cocaine
• Alkaloid extracted from Erythroxylon coca • เพิ่มการกระตุนของcathecolamineทําใหเพิ่มความดันเลือด ชีพจรเตนเร็ว รวมกับการหดตัวของหลอดเลือดโคโรนารีที่หัวใจ
• Acute overdose ทําใหเกิด ชัก หัวใจเตนผิดจังหวะ
• นอกจากนี้ถาใชแบบเรื้อรังทําใหเกิด atherosclerosis ของหลอดเลือดได เปนเหตุใหเกิดกลามเนื้อหัวใจตายได

– basically a scrape – superficial epidermis is torn off by friction or force – regeneration without scarring usually occurs
• Laceration vs. Incision – a laceration is an irregular tear in the skin produced by overstretching. The wound margins are frequently hemorrhagic and traumatized
– an incision is made by a sharp cutting object. The margins of the incision are usually relatively clean
• Contusion – an injury caused by a blunt force that damages small blood vessels and causes interstitial bleeding, usually without disruption of the continuity of the tissue (cf ecchymosis)

• RELATED TO RAPID ATMOSPHERIC PRESSURE CHANGES – LUNGS – VISCERA, especially GAS filled viscera
• Rupture, Hemorrhage, etc. • IMMERSION BLAST also possible, causing more of a total body compression syndrome
DECOMPRESSION • Related to GAS SOLUBILITY in divers ascending rapidly, especially the more NON‐SOLUBLE gasses, like NITROGEN, and, to a lesser extent, XENON
• AIR EMBOLISM is the common pathology – ACUTE:
• “BENDS” (peri‐articular), acute • “CHOKES” (lungs), acute • “STAGGERS” (inner ear), acute
– CHRONIC: • ASEPTIC NECROSIS: humeri, femurs
